COMPLAINT-AFFIDAVIT
I, EMETERIO C. OCAYA, of legal, single, a resident of No. 75,
San Vicente,
Baguio City after having been sworn to in accordance with law,
do hereby depose and
state:
1. That I am filing a complaint against DIOSDADO MARAPAO
and CAMILO E. TAMIN (“Respondents”), for Serious Physical
Injuries, punishable under Art. 263 of the Revised Penal Code
who
are both residents of No. 85 Engineer’s Hill, Baguio City;
2. That at about 11:00 o'clock in the evening of March 29, 2016,
while I was
walking along Legarda rd., Baguio City, I was approached by
the
Respondents and asked if my name is Emeterio, I answered in
the
affirmative and asked in return who they were.
3. Suddenly, they all looked at each other and laughed
boisterously
and then came rushing and started beating me. I never had a
chance to defend myself because it was fight of four (4)
againstone (1).
4. Due to the said incident, I suffered many injuries and the
following
are:
a. Swollen left eye and cracked left eye socket. Requires
confinement and treatment for 45 days.
b. Cracked ribs, requires confinement for 30 days.
c. Dislocated shoulder ball joint, requires treatment for 20
days.d. Crushed Kneecaps, requires surgery and replacement,
and
rest for 60 days.
e. Left hand pinkie crushed beyond repair requires
amputation.
5. I am executing this complaint-affidavit in order to file a
case of Serious
Physical Injuries against DIOSDADO MARAPAO and CAMILO
E. TAMIN
IN WITNESS WHEREOF, I have hereunto set my hand this 2nd
