OFFICE MEMORANDUM

Subject: Supply of medicines on credit basis to CSIO Staff, Pensioners and their dependent family members - reg.

On the recommendations of duly constituted Committee, the Director CSIR-CSIO has been pleased to empanel the following chemists for supply of medicines to CSIR-CSIO staff, pensioners and their dependent family members on the prescription of Senior Medical Officer/lady Medical Officer of CSIO Clinical Centre on credit basis for a period of three months w.e.f. 01.11.2016 to 31.01.2017 on the following terms and conditions :

1. M/s. Jan Aushadhi Generic Drug Store Indian Red Cross Society Shop No. 5, Govt. Medical College Sector 32, Chandigarh

2. M/s. Apna Chemists Near Emergency, Under Ramp, Block A Govt. Medical College & Hospital Sector 32, Chandigarh

T~!rms and Conditions :

1. The Chemists will make the supply of the day to day requirements of medicines to the staff, pensioners and their family members directly on production of prescription slips issued ' by Medical Officers of CSIR-CSIO Clinical Centre and also on daily basis at CSIR-CSIO Dispensary on demand as per the convenience of the patients or as directed by the Medical Officers of CSIR-CSIO.

2. The Chemists will issue medicines to staff members/pensioners and their family members and get the signatures of patients (along with ID Nos of the Staff Members/pensioners) on reverse of the prescription slips and invoices/bills.

3. All the prescription slips and invoices along with pre-receipted and stamped consolidated bills in triplicate would be submitted in CSIR-CSIO Dispensary for certification/verification that the medicines supplied to the patients are as per prescription and

invoices have been duly recommended by him/her. After certification/verification by the concerned Medical Officers of CSIR-CSIO, the bills will be processed for payment by the office. The consolidated bills would be prepared by the Chemists separately for regular staff/their family members and pensioner I their family members. The paymek nts would be credited to the Bank Account of Chemist through electronic mode within 30 days of receipt of the certified bills.

4. The period of contract will be for three months w.e.f. 01.11.2016 and can be curtailed/extended at any time on 07 days' notice by the Director, CSIR-CSIO.

5. Substitute medicines will not be accepted unless permitted by the Medical Officers of CSIR-CSIO Dispensary.

6. Cut strips and loose medicines will be accepted only after showing batch number and expiry date.

7. The Chemists will record a certificate that the price charged m each bill is not more than the printed price (MRP).

8. The supplied medicines should be of latest batch and their expiry date should not be less than six month from the date ofi supply.
