Learning Objective

• Examine the rationale and trigger point treatment of core musculature including handling techniques

3

Course Outline

• Physiology review of trigger points

• 3 Key trigger point treatments

• Improving sensory‐perceptual function to improve work, sleep, and cognition

• Core activation initiation techniques

5

Travel and Simons define trigger points as, “a highly irritable localized spot of exquisite tenderness in a nodule in a palpable taut band of muscle tissue.”

Trigger Point Physiology & Symptoms

• Local and Remote Inflammation• Acidic environments

• Hypoxia• Tissue Sensitization

• Referred pain – Active TP’s

• Stiffness / Tightness – Latent TP’s

• Compression of Vessels and Nerves• Sensation changes (numbness, tingling, burning)

• Autonomic Effects• Eye redness, blurred vision, nasal secretions, respiratory changes, nausea, etc…

9

Referral Patterns

• Somatic to somatic• Erector spinae to gluteal

• Somatic to visceral• Iliopsoas to testicle

• Visceral to somatic• Heart to shoulder, jaw, back

• Visceral to visceral • Lungs to stomach, esophagus

10

Trigger Point Formation

• Physical Trauma• Slow – overwork, repetitive movement, poor posture

• Fast – MVA

• Emotional Trauma• Abuse, anxiety, depression, stress, etc…

• Exposure to Harsh Climates• Cold drafts, dehydration, heat stroke

• Metabolic Disorders• Hypoglycemia, thyroid inadequacies, anemia, etc..

11

Trigger Point Treatment

• Ischemic Compression• Mechanical pressure releases acidic “bubble”

• Capillary vasodilation flushes chemicals and normalizes circulation

• Nervous system threshold normalization

• Static• Hold 6‐10 seconds x 3 repetitions• 5‐7 intensity rating level

• Dynamic• Do warm up strokes

• 6‐12 short, slow, deep, repeated “milking” strokes in direction of fibers

• 5‐7 intensity rating level

12

3 Trigger Point Treatments

• Erector Spinae• Spinalis, Longissimus, Iliocostalis

• Quadratus Lumborum

• Multifidus

13

Erector Spinae

Spinalis Thoracis

‐Spinous process to spinous process

Longissimus (Lumbar)

‐ Ilium to L1‐5 transverse processes

Iliocostalis Lumborum

‐Sacrum, Iliac Crest, TL Fascia to L1‐4 transverse processes, ribs 5‐12

Tips

Superficial spinal muscle TP’s cause more diffuse type referral

Chronic back stiffness/tightness is sign of latent TP’s in superficial spinal mm

14

15

Quadratus Lumborum

Iliac crest, iliolumbar ligament to transverse processes of L1‐4 and inferior border of rib 12

TipsReferral can occur in hips, buttocks, SI joint, groin, and anterior thigh.

Referral symptoms commonly mistaken for spinal arthritis, disk problems, sciatica, hip bursitis

Coughing and sneezing can cause stabs of intensity

16

Page 10: 3 Key Trigger Point Treatments to Enhance Core Activation

Iliac crest and transverse process to spinous processes 2‐5 segments above.

Bilateral spinal extension

Unilateral lateral flexion, contralateral rotation

TipsTP’s can misalign vertebrae restricting motion in all directions

TP’s can compress vertebrae causing foraminal stenosis and subsequent symptoms

Referral can be local, to abdomen, or buttocks

Multifidus

17

Improving Sensory‐Perceptual Function

• Work• Low back pain is the leading cause of work relateddisability, 577 million people affected

• Sleep• 58.9% of chronic low back pain sufferers have disturbed sleep

• Cognition• 45‐55% prevalence of anxiety and depression in people with chronic low back pain
