purposes of this request, separation of employment may either be voluntary or

involuntary, including ret irement. Please specify the number of separation for each

year, the job classifications of each separation, and whether the separation was

employee or employer initiated.

As used herein, the term writing shall mean handwriting, typewriting, printing, photostating,

photographing, and every other means of recording any form ofcommunication or representation, including

letters, words, pictures, signs, or symbols or combinations thereof, and any and all papers, maps, magnetic

or paper tapes, photog raphic films and prints, magnet ic or punch cards, disks, drums, and other documents.

Demand is hereby made that within ten days from receipt of this request a determination is made

whether the City of Oak land will produce, in whole or in part, the aforementioned requested public records

in your possession. Should you deny any of the above requested records, please set forth the names, tit les,

and positio ns of each person responsible for the den ial and the reaso ns therefo r. Further, please advise as

to the estimate of any photocopy ing expense necess itated by this reques t.
