symptoms; Use assessment skill for the collection of subjective and objective data

Access: Look up results of tests that have been completed; Review patient’s medication list & allergies

Analyze: Use critical thinking to determine nursing problem list and compile interventions

Apply: Carry out the interventions

Assess: Review effectiveness or plan of care and adjust accordingly
