• In this scenario, the family pays a total of $11,400

• Since there is an embedded deductible, each individual is responsible for paying the individual out-of-pocket max up to the total family out-of-pocket max

Example: $2,850 Individual / $5,700 Family deductible, and 70% coinsurance. Richard, his spouse, and child are enrolled in this plan. Let’s say Richard has a hospital claim that costs $100,000:

Since the plan has an embedded deductible, Richard must pay the individual deductible of $2,850, and then 30% of the remaining $97,150 up to the individual out-of-pocket maximum or an additional $2,850, for a total of $5,700

Mom must pay the individual deductible of $2,850, and then 30% of the remaining $17,150 up to the individual out-of-pocket maximum or an additional $2,850, for a total of $5,700

The plan covers the child’s claim at 100% since the family out-of-pocket maximum has been met.

FSA/HRA/HSA

An employer funded health benefit that reimburses

teammates for out of pocket expenses

• Teammates enrolled in the $1850 Plan will receive HRA funding from Service King

• Service King contributes $250 for single coverage and $500 annually for family coverage

• While FSA and HSA allow teammates to contribute pre-tax dollars through payroll, an HRA is solely funded by Service King. Teammates cannot make contributions to an HRA and funds are not portable

An individual medical savings account that lets you save towards eligible healthcare

expenses

• Eligible expenses include coinsurance and prescriptions

• Teammates enrolled in the $2,850 Deductible Plan can contribute up to $3,500 single or $7,000

• SK contributes $250 annually for single coverage and $500 annually for family coverage.

• Contributions are made quarterly and teammates must have an open HSA account to receive funding

• HSA accounts can accumulate year over year and are portable

401(k) Plan – Fidelity Investments

• Enroll anytime after 90 days of employment and age 21.

• Company provides discretionary match annually, currently at 50% of the first 8% contributed. Teammate must be active on December 31 to receive annual match.

• That means: For every $1 you save, Service King puts in $.50

• Three year vesting: 33% / 66% / 100%

• You may elect a percentage of pay to save or a specific dollar amount through payroll deduction.

• Amount you contribute is tax-deferred. You pay taxes only when you withdraw the money. You’ll pay a penalty if you withdraw your account prior to age 59-1/2.

• Changes can be made at anytime to your account by logging into your Fidelity account at www.401k.com or by calling the Retirement Benefit Line @ 1-800-835-5097.
