The conference will take place on November 13-15, 2019 in Memphis, T ennessee. Putting Care at the Center, co-hosted this year by the National Center and Regional One Health, is the central event for the emerging field of complex care. It attracts thought leaders, innovators, and those engaged in frontline clinical work from around the country, as well as consumers, policymakers, researchers, and data analysts.The conference reinforces the key concepts that link people from a range of disciplines while also sharing practical skills and knowledge that can be immediately applied in local communities. The National Center is excited to partner with Regional One Health as a conference co-host in Memphis, Tennessee to convene complex care stakeholders from across the country and feature work from the Memphis area. Putting Care at the CenterThe Camden Coalition of Healthcare Providers’ National Center for Complex Health and Social Needs is seeking proposals for conference workshops for its 2019 national conference, Putting Care at the Center. The conference will take place on November 13-15, 2019 in Memphis, Tennessee.

Putting Care at the Center, co-hosted this year by the National Center and Regional One Health, is the central event for the emerging field of complex care. About the WorkshopsWorkshop sessions are intended to expose participants to a particular model, teach a specific skill, or discuss a topic that is immediately actionable for attendees. We are seeking a range of formats including panel presentations, podium talks, experiential training, facilitated work sessions, or small group discussions, but have a preference for experiential training and other interactive approaches. We anticipate providing continuing education units for workshops and will require workshop organizers to cooperate with conference organizers to ensure continuing education certification.

Workshops will run for 75 minutes for groups between 20-120 participants, depending on the modality. Please note that some workshops will run twice to accommodate additional participants.

We are particularly interested in workshops that address one of the following topics:

Care Delivery • Clinical Models - e.g., site or subpopulation-specific interventions, forming interdisciplinary care

teams, care coordination workflows• Core Competencies - e.g., trauma-informed care, reducing stigma toward complex populations,

harm reduction

Data & Evaluation • Data-driven methods for design and assessment• Effective use of data with limited resources• Population segmenting and patient identification• Sharing data across systems

Finance and Payment • How to use data to make a business case for complex care interventions• Implementing alternative payment models• How to leverage partnerships with managed care organizations

Policy & Advocacy• Leading consumer-driven programs & advocacy• Policy tools to advance complex care

Program Design and Operations • Workforce retention and resilience• How to integrate consumers in program design and evaluation• Cross-system process improvements

Application Process Please provide the following information for each proposed workshop. The below information is for reference only - please submit all applications through the online portal. Only proposals with all required information will be considered. The point of contact should be a presenter in the session and have substantial knowledge of the material to be presented.

7. Workshop description (up to 500 words) including: a. Format (panel, podium, participatory, etc.) b. Proposed target audience(s) (clinicians, researchers, policy, consumers, etc.) and populations/subpopulations that could be supported through the workshop. c. Summary of content d. How the content responds to the needs of the complex care field and what problem is addressed through the session. e. Workshop applicants are encouraged to specifically address the selection criteria listed below.

8. Workshop learning goals a. Knowledge or skills that participants will leave the session able to apply b. Each goal should have only one measurable action verb and should specify what the learner will be able do once the goal has been completed (the outcome of attaining the goal)

Selection CriteriaConference organizers will select workshops based on the following criteria: a. Applicability - the extent to which the knowledge applies to others and can make a difference in how others do their work. Related note: there will be preference for workshops that feature free, open source tools and resources. b. Quality - the material is professional and well organized c. Innovation - the knowledge is innovative in the way we think about complex health and social needs d. Incorporation of Key Tenets: i. As a person-centered field, complex care must incorporate individuals with lived experience in the development, implementation, and assessment of complex care programs or efforts. Priority will be given to workshops that include a community member/patient/consumer of complex care as a co-equal presenter and content expert. ii. The Blueprint for Complex Care1, unveiled at the 2018 Putting Care at the Center conference, introduced a comprehensive definition of complex care, an assessment of the state of the field, and prioritized recommendations to move the field forward. We encourage workshop applications to incorporate concepts from the Blueprint, particularly the ecosystem of care that seeks to engage a wide variety of sectors, disciplines and professions. e.
