Editor’s DeskDear Readers,

We are thankful to all those who appreciated our efforts and coved their best wishes .This

issue is devoted to our profession.

On 8th of September we celebrated the World Physiotherapy Day.

World Physical Therapy Day is the day which gives an opportunity to all physical therapists from

all over the world to raise awareness about the crucial contribution the profession makes to

keeping people well, mobile and independent.

During its history the profession of physiotherapy has successfully evolved and developed,

and the role of physiotherapists has diversified and expanded considerably in recent years,

particularly in response to current health and social care reform.

The physiotherapist profession has come a long way on its road of experience. Initially it was

more focused on patient care. This was inclusive of building a balanced therapeutic relationship

which consist of trust, respect and understanding for patient’s ideas, beliefs, knowledge and

values. Patients were involved in decision making and patient’s autonomy was emphasised. In

short, therapists were focused in working within the “Biopsychologicalmodel” of care.

During this journey within a short time the profession has given due importance to the academic

development as well. By practicing the greater use of evidence and clinical guidelines, more

emphasis is laid down on understanding patient care. Not only there is growth in numbers

of clinical Scope Practitioners & Consultant Physiotherapist, but there is also proportionate

increase in numbers of Clinical Researchers. Clinical speciality itself has grown considerably

in all areas due to technological advancement. The profession had solely but firmly made its

space in the medical field and made its identity in society.

It is correctly said that ‘Doctors add years to life and Physios add life to the years’.

Scientific Writing

Neuro Developmental Therapy - An InsightBitter are the tears of a child: Sweeten them.

Deep are the thoughts of a child: Quiet them.

Sharp is the grief of a child: Take it from him.

Soft is the heart of a child: Do not harden it.

Cerebral palsy (CP) is defined as a non-progressive, but often clinically changing motor impairment due to an abnormality of the developing brain. It is a symptom complex or a descriptive term rather than a specific disease. Intellectual, sensory and/or behavioural problems may exist although the primary abnormality must be a motor deficit.

Postural alignment is negatively impacted by abnormalities in motor control, muscle tone and weakness. Weakness resulting from neuromotor deficiencies and lack of use of muscle in a variety of ways decreases the capacity to control posture against gravity and ground reaction force on the body. This results in delayed milestones.

Neuro developmental techniques aim at improving the postural control, stabilizing the trunk and improving balance and eliciting proper motor response. During therapy, input can be used through application of manual techniques to increase feedback and control in treatment and to progressively strengthen core postural musculature. Specifically, the direction of input is often applied to anchor the body and give perception of the base of support and guide the direction of desired movement.

When I treat children of such types, I am always reminded of what Bobath says,

”Since we began our therapy in 1943 we have been learning constantly, and experience has taught us to change our approach and our emphasis on certain aspects of treatment. However, the basic concept has not changed…we have been guided by the child’s reactions to our handling… improved our knowledge…tried to avoid repeating mistakes. We have learned to test the value of a particular technique by the child’s response to it, and. on the close interplay between child and therapists.’

NDT works on postures, meaning postures that reduce abnormal reflex activity. It uses theory of replication and gives the brain a “second chance”. It sets the neural clock to “correct time”. It works on specific stereotyped movements. Once begun should not be abandoned mid stream. This needs careful and qualified supervision.

When using manual points of contact the therapist can apply or direct their input to increase feedback and direct weight shift or active muscle control, limit direction of patient movements to strengthen activation of a muscle group.

Today manual contact is viewed as a method of feedback for the patient scaling information so the patient can actively produce movements that are components of functions. Principles of manual contact can be analyzed in the direction of input, the point of contact and the load the input is giving for what purpose.

For instance if manual contact would be applied from superior to inferior direction in sitting, you would attempt to align the body and add information through the shoulder superiorly and in inferior direction to facilitate the individual to activate their posture off the base by erecting the trunk. If there was no response, shifting the individual off the base laterally or in antero-posterior direction may facilitate activation of hip into the base to control the shift. In this instance the shift would move toward infero-anterior or infero-posterior or infero-diagonal to create a lateral weight shift. This is how motor control is achieved

v Orientation/Foundation Program – I BPT

The orientation/foundation program for the 15th batch (2013-14) of BPT students was held from 10th to 14th September, 2013. Students were welcomed with inspiring words and they were explained about the core values of our institute, Vision & Mission of institute, importance of Physiotherapy in health care and emphasized on its growth as a profession. Dr. Shirish Srivastava explained about the rules and regulations of university regarding the attendance and examination. An interactive session of students with Shri Sandeep Desai, CEO, CAM was arranged concerning the core values of professionalism, moral values and introduction to vision and mission of the institution. An interactive session with the parents was held to familiarize them regarding history, functioning and rules and regulation of the institution. Senior medical and physiotherapy faculty delivered special talks on various topics viz. Role of physiotherapy and practices in KMPIP, Communication skills, Professionalism, Time management, Decoding medical terminology, etc.

Visit to campus, medical college & hospital, Physiotherapy staff orientation, interaction with class co-ordinator / mentor, health check up / vaccination, village visit and patients’ interaction with students also comprised of a major part of this orientation/foundation program.

The response of the students and parents to this orientation program was overwhelming and feedback taken from the parents and students at the end of the program suggests their complete satisfaction for the same.

v Orientation/Foundation Program – II, III & IV BPT:

Orientation/Foundation program for II, III & IV BPT students was held on 5th September, 2013. Special talk on various topics like communication skills, stress management, hospital acquired infection, anti ragging importance were delivered by the senior medical and physiotherapy faculty.

This workshop has grown three months old by getting a new shape after adoption by KMPIP family under the dynamic guidance of our Principal- Dr. R. We have fabricated 34 orthotic appliances within last three months to enhance the quality of life of patients.

Another milestone of KMPIP…

Our paediatric physiotherapy unit receives lot of physically challenged children for rehabilitation. They receive full fledge physiotherapy, occupational therapy and other medical services as a part of treatment protocol. Among these children, there are many mentally retarded but can improve with proper guidance, keeping in view this scenario, with the approval from our CEO the institute will commence Special Education by qualified special educators to these children to carry their need and to empower them. This will be a “Small leap for a giant commitment”.

v A Multidisciplinary Camp was organized at Gamadi Senior Citizen Hall on 7th August, 2013 by business development department of our organization. Total three disciplines i.e Medicine, Orthopedic and Physiotherapy provided its services in this camp. Dr. Ashish Gupta (PT), faculty of KMPIP along with PG students participated in this camp. Total 40 patients received physiotherapy treatment during this camp. All the patients were educated about their conditions and basic home exercise program was give with advice for follow up at K. M. Patel Institute of Physiotherapy. The response to the camp was overwhelming and feedback collected from the patients showed their complete satisfaction about the services provided in the camp

v Our institute participated in the camp organized by Rotary club- Baroda Metro, Rotary Foundation and Rotary club of ASHIA (Japan) and Bhagwan Mahaveer Viklang Sahayata Sammitee Kendra at Borsad on 21st and 22nd September, 2013. Dr. R. Harihara Prakash and Dr. Daxa Mishra accompanied by Dr. Deepak Ganjiwale (OT) and Dr. Nirali (PT, Petlad Extension Centre) along with 10 PG students participated in the camp. Around 200 patients were screened and assessed for prescription of free orthotic and prosthetic aids for orthopedically handicapped. The beneficiaries were from surrounding villages and were referred to our Petlad and Bhadaran Centres for follow-up and further management.

v Community visit by I BPT Students

A community visit was arranged for newly joined freshers (I BPT) on 13th September, 2013 at Mehlav village. The students were accompanied by Dr. Vinod Ravaliya and Dr. Hardik Parekh. The visit was scheduled with objective to sensitize them regarding village life, economic background, educational level, sanitary facilities, health services, government related schemes for village beneficiaries etc.

The students were assigned a task of collecting detailed information related to framed objectives by interacting with the family members. The students also visited a Community Health Centre where they were explained about the services provided by the government to the village people.

WORLD PHYSIOTHERAPY DAYK. M. Patel Institute of Physiotherapy celebrated “WORLD PHYSIOTHERAPY DAY” on 7th September, 2013. The theme was “Physiotherapy for Preventive and promotive health”.

Institute celebrated this day in the following ways:

• Tree plantation program was arranged in the morning and 20 plants were planted in the pots by Principal, Student Support Group and Group of students.

• Poster competition among the students was arranged on the topic “Physiotherapy for Preventive and promotive health” and the winners of this competition were Surbhi Mehta, Krupali Damor and Anjali Shah (II BPT).

• Speech was delivered by Dr. R. Harihara Prakash on the day of world PT day and oath taking ceremony for physiotherapists was conducted.

•A Radio talk was arranged on “Prevention and Management for low back ache” by Dr. R. Harihara Prakash and Dr. Daxa Mishra.

• An article on “Role of physiotherapy in cancer patient” by Dr. R. Harihara Prakash was published in Gujarati news paper.

v Farewell Function- PG

Farewell function for outgoing PG students was arranged on 26th July, 2013. Dr. R. Harihara Prakash (Principal, KMPIP), Dr. Daxa Mishra (PG coordinator), Dr. Yojana Sharma (HOD Dept of ENT), Mr. Ajay Phatak and Mrs. Jayshree Ganjiwale (members of CRS), all the Physiotherapy faculty and all PG students attended the occasion. Dr. R. Harihara Prakash, Dr. Daxa Mishra delivered inspiring words and wished them all the best. Mrs. Jayshree Ganjiwale shared her experience on the process of dissertation review for PG students. Best dissertation award was given to Dr. Amreen Palejwala and best academic award of the year was given to Dr. Shailja Mashroowala. As a memory of this function, Mementoes were prepared by second year PG students and were distributed to all outgoing PG by their respective Mentors

v Quiz competition

K.M. Patel Institute of Physiotherapy organized a quiz competition for physiotherapy students as a part of Academic activity on 23rd September, 2013. The quiz was planned as an innovative practice to boost learning and participation among students. Dr. R. Harihara Prakash, Dr. Daxa Mishra and other faculty members attended the competition to motivate the students.

Total of 10 teams (4 students per team) participated out of which 4 teams qualified at the end of Qualifying round. The entire quiz was thoroughly enjoyed by all the students with active participation. The winners and runners-up of this quiz competition were as follow:
