Community Mobiliza8on
Come Together Mobilizing your community involves bringing the right people together and working collabora8vely. This includes youth and families and people from a range of diverse groups in your community.
Make a Plan Assess community readiness for change and explore strengths, needs and issues that are most important. Work together to create a formal ac8on plan that includes sustainability.
Get Going Community mobiliza8on is more than planning. Take your plan, put it into ac8on and bring change in your community.
Evaluate Your Efforts Evalua8on allows you to assess what you’ve done and plot a course for ongoing improvements. It should take place throughout the process.
WHAT WE FUND HEALTHY COMMUNITIES
Community Environment -‐ Our physical and social surroundings -‐ the places where we live, work, play and learn. Community Mobiliza8on -‐ A process through which ac8on comes from by a community itself, or by others, that is planned, carried out and evaluated on a par8cipatory and sustained basis to improve health. Mul8-‐Sectoral Collabora8on -‐ Partnerships between different sectors (e.g. government, private and community) and fields (e.g. ,health, educa8on and transporta8on).
What We Mean -‐ The following common terms are generally used in Healthy Communi8es efforts. We feel these are an important framework for applicants to have a shared understanding of terms used.
Public Will Campaign -‐ Organized, strategic ini8a8ves designed to legi8mize and garner public support for social problems as a mechanism of achieving policy or ac8on change. Policy -‐ The rules that guide the ac8vi8es of government or organiza8ons and provide authority for the use of resources including ins8tu8onal prac8ces and public policy. The Equity Approach – The means to ensure that everyone has the opportunity to par8cipate and prosper when building strategies to improve community health.

Fiscal Agents HCF connects nonprofit grantees who do not have an annual audit with fiscal agents. For more informa8on, contact your program officer or call 816-‐241-‐7006.

Grant Review HCF invites prospec8ve applicants to visit our office to review past successful proposals. If you are interested in reading successful grants, please contact HCF to set up an appointment.

Feedback Sessions HCF invites applicants who were not awarded funding to receive feedback from the Program Officer(s) who reviewed their request.
