What’s New for 2018 You, our employees, are Harmonia's greatest resource and your HR and execu�ve team strives to provide the most compe��ve benefits package in the industry to each of you. To this end, we constantly review our benefits to see how we can make them even beter. For 2018, we are pleased to present to you the following changes and addi�ons:

• Aetna Medical Plans: You have a choice of three plans, two of which are HSA eligible. • Mutual of Omaha Voluntary Life Insurance: You may elect coverage during open enrollment for up to $150000

for employee, $30000 for spouses, and $10000 for dependent children without any health questionnaire unless you were previously declined.

• Upon termination of employment with Harmonia, we will pay you your PTO balance unless you were terminated for cause.

Who’s Eligible In accordance with the Affordable Care Act, all employees who work more than 28 hours per week are eligible for medical, dental, and vision benefits. You must be a salaried employee to be eligible for all other benefits.

Key Issues to Factor into Your Planning for Medical, Dental and Vision Insurance If you have health insurance coverage elsewhere (through your spouse’s employer-sponsored plan, for example), consider whether it would be more practical for you to cover your dependents (or yourself) under that plan (based on your family’s health needs) instead of Harmonia’s medical plan. Remember that benefits payable under Harmonia’s plans are coordinated with benefits payable under your spouse’s employer-sponsored plan. Generally, the benefit, if any, of having dual coverage varies widely based on the plans available and should be carefully evaluated for exclusions or limitations prior to enrolling into one or more plans.

Always review factors like co-pays, coinsurance, deductibles, etc. prior to selecting a benefits package. Don’t assume your benefit options are the same as prior years and make sure to select the plan that best addresses your current health and welfare needs. Here are some things to keep in mind:

• Medical insurance of dependents is included up to age 26 regardless of dependency status. • No pre-exis�ng medical condi�ons apply to any new or renewing plans. • Preven�ve services are covered at 100%. • Check your current medical and dental providers to make sure they are in your new network.

Changing Your Benefit Selections During the Year In some cases, you may need to cancel or change your benefit selections during the year. By law, a number of the benefits cannot be canceled or changed unless a qualifying event occurs. If you want to decline a benefit, select a different plan, or add family members to your plan, refer to the chart that follows to determine when you can make the changes and how.

Qualifying Events Most benefits can be modified only during open enrollment or if you experience a qualifying event. The following are considered qualifying events:

• Change in legal marital status, including marriage, death of a spouse, divorce, legal separation, and annulment.

• A change in the number of dependents, including birth, death, adoption, and placement for adoption. • A change in employment status of the employee, or the employee’s or retiree’s spouse or dependent,

including termination or commencement of employment; a commencement of or return from an unpaid leave of absence; a change in worksite; and a change in working conditions (including changing between part‐time and full‐time or hourly and salary) of the employee, the employee’s or retiree’s spouse or dependent which results in a change in benefits they receive under a cafeteria plan or health or dental plan.

• A dependent ceasing to satisfy eligibility requirements for coverage due to attainment of age, student status, marital status, or other similar circumstances.

• A change in place of residence of the employee, retiree or their spouse or dependent and the current carrier is not available.

• Significant cost or coverage changes (including coverage curtailment and the addition of a benefit package). • Family Medical Leave Act (FMLA) leave. • A change in coverage of a spouse or dependent under another employer’s plan. • Open enrollment under the plan of another employer. • Health Insurance Portability and Accountability Act (HIPAA) special enrollment rights for new dependents and

in the case of loss of other insurance coverage. • A COBRA‐qualifying event. • Loss of coverage under the group health plan of a governmental or educational institution (a state’s children’s

health insurance program, medical care program of an Indian tribal government, state health benefits risk pool, or foreign government group health plan).

• Entitlement to Medicare or Medicaid. • Any other situations in which the group health or dental plan is required by the applicable federal or state law

to allow a change in coverage.

Dental Insurance Harmonia offers two Dental Plans through Anthem BlueCross BlueShield. Both plans offer orthodontia coverage. The plans offer the same coverage, but the Low Option pays up to $1000 of covered expenses per year and the High Option pays up to $1500. Maximum Carryover: Employees can carryover a limited amount of their remaining maximum to the next coverage year. How it works: • There must be at least one claim paid during the coverage year. • If total paid claims do not exceed $500, they can carryover up to $250 to the next coverage year. • Amounts carried over can accumulate to a total of $1000. • Carryover starts to accumulate only after one full coverage year. • A break in coverage will reset the balance to zero. • Claims payments are made first from the standard annual maximum. Once the annual maximum has been

reached, then claims begin paying from the available accumulated annual maximum carryover.

Why Should I Have Vision Insurance?

Per Gallup, approximately 70% of adult Americans report wearing some type of corrective lenses. A visit with your eye doctor can determine whether you need corrective lenses and, if so, the correct prescription. Other eye concerns that will be addressed in an eye exam include checking for conditions or diseases such as glaucoma and cataracts, which can lead to vision loss.

Health Savings Account Save on this Year’s Health Care Expenses—and for the Future If you are enrolled in a qualified high-deduc�ble health plan, you are automa�cally enrolled in Health Equity’s health savings account (HSA). This account can help you save and pay for medical expenses tax-free. With an HSA, you can choose to spend the funds today or build them for the future.

You can have HSA money deducted pre-tax from your paycheck or send contribu�ons straight to Health Equity and file on your taxes as a botom line tax deduc�on. Health Equity charges a $3.95/month fee to manage your HSA; the fee is automa�cally deducted from your contribu�on.

HSA Advantages You own it. You own the funds in your account from day one. You decide when and how to use them. And if you change jobs, you can keep it.

Health Reimbursement Arrangement Harmonia is excited to offer a Health Reimbursement Arrangement (HRA) to employees enrolled in our HSA plans. This is an employer-funded, tax-advantaged plan that can be used to reduce your out-of-pocket healthcare expenses each year.

Eligibility You are automatically enrolled in the HRA if you are enrolled in any of the HSA plans.

Availability of Funds Once you have met $1350/$2700 for your out-of-pocket expenses—the portion you pay—you can begin requesting reimbursement for any additional medical, hospital, and/or prescription drug costs you incur. Harmonia will reimburse you up to $1105/$2227.50 for any additional expenses you incur after the HRA deductible. This may not be used to cover dental or vision expenses.

Reimbursement from your HRA must be substantiated by a claim. Expenses shown on your Explanation of Benefits (EOB) statement are typically required to show that you have met the HRA deductible. Claims are generally processed within one week. Reimbursement is by check or direct deposit from Flexible Benefit Administrators.

Active employees have a 90-day period each plan year to file claims. If you leave Harmonia, you will 60 days from the date of separation to file claims. Coverage can continue under COBRA.

Harmonia HRA Contributions Harmonia will pay 33% of each claim after the HRA deductible shown below is met and up to the contribution limit specified below per year:

Parking and Transit Benefit Program By participating in the Parking and Transit Program, you can set aside pre-tax dollars to be used to cover certain transit or parking expenses.

Qualified Transit Passes “Transit pass” means any pass, token, fare card, voucher, or similar item that entitles you to transportation (or transportation at a reduced price), provided that such transportation is on mass transit facilities, or provided by an entity in the business of transporting persons if such transportation is provided in the type of highway vehicle eligible for use in vanpooling. Examples include bus, light rail, streetcar, subway, vanpool, or ferry. TOLLS are not included. Maximum Election: $260.00 per month per calendar year, Tax Free

Qualified Parking Qualified parking is that which is provided to you at or near Harmonia's business premises. It also includes parking at or near a location from which you travel to work by mass transit, vanpooling, in a commuter highway vehicle, or by carpool. Parking at or near your home doesn’t qualify. Maximum Election: $260.00 per month per calendar year, Tax Free

No Use It or Lose It Rule Any money you do not use from your transportation benefits account for expenses incurred during a Plan Year will be rolled over to the next year.

You Can Make Changes to Your Election You are not locked in to your election throughout the Plan Year. You may stop, start, increase, or decrease your election as needed.

Obtaining A Reimbursement From Your Parking & Transit Benefit Accounts To obtain a reimbursement from your Parking & Transit Benefit Accounts, you must complete a Claim Form. This form is available from Harmonia. You must attach a receipt or bill from the service provider which includes all the pertinent information regarding the expense such as date of service, amount charged, provider’s name, and nature of expense OR you can use your Benefits Card to pay for expenses from your Parking & Transportation Benefit account.

The Parking & Transit Benefit Account Is Not A Pre-Funded Account Harmonia will deduct pre-tax money from your paycheck and deposit it into your account. You will be reimbursed only up to your account balance at the time you submit your claim. If your claim is for more than your account balance, the unreimbursed portion of your claim will be tracked by Flexible Benefit Administrators. You will be automatically reimbursed as additional deductions are taken and deposited into your account, until your entire claim is paid out.

Income Replacement As a part of Harmonia’s benefits, we provide you with Life and Accidental Death and Dismemberment (AD&D), Long and Short Term Disability Insurance coverage, and the opportunity to enroll in supplemental voluntary life insurance. These programs offer you and your family financial protection against some of the uncertainties life can bring.

Life and Accidental Death and Dismemberment As an eligible Harmonia employee, you will be automatically enrolled in our Basic Life and AD&D Insurance program. This benefit provides $50000 of Basic Life and AD&D coverage.

Voluntary Life and Accidental Death and Dismemberment The Voluntary Life Insurance Plan is a voluntary, employee-paid group life insurance plan which you can elect to supplement your employer paid Life Insurance of $50000. You may select coverage for yourself, your spouse, and your dependent children. Payments are conveniently handled through payroll deduction on a post-tax basis. Unlike the employer provided Life Insurance, voluntary Life Insurance is totally portable and you can take it with you if you terminate employment.

Long Term Disability As an eligible Harmonia employee, you will be automatically enrolled in our Long-Term Disability (LTD) Insurance plan. In the event of disability, this program will replace 60% of your monthly, pre-disability earnings to a maximum of $5000 per month. Benefits may begin after the Elimination period of 90 days due to a covered accident or sickness. This policy will provide a benefit as long as you are disabled up to the Social Security normal retirement age.

Short Term Disability As an eligible Harmonia employee, you will be automatically enrolled in our Short-Term Disability (STD) Insurance plan. In the event of disability, this program will replace 60% of your weekly, pre-disability earnings to a maximum of $1,500 per week. Benefits may begin after the Elimination period of 8 days due to a covered accident or sickness and last for 90 days.

Retirement Plan Harmonia offers all full-time employees the opportunity to participate in the company-sponsored 401(K) plan through John Hancock Pensions. For full details, please contact Human Resources.

Eligibility You may begin participating on the first day of the month following your start date.

Contributions and Matching Contributions to your 401(K) are made via pre-tax payroll deductions. Please note the following:

Time Off Harmonia strives to provide flexibility with time away from the office. Harmonia offers paid personal time off, paid holidays, jury/witness leave, bereavement leave, FMLA leave, unpaid leave, and personal leaves of absence.

Paid Time Off (PTO) We believe taking PTO from time to time is vital to your continued productivity and we encourage every employee to take PTO on a regular basis. Eligible employees begin accruing PTO on your first day of employment. PTO is earned every day. Thus, if you earn 16 days per year, you will accrue .35 hours per day x 365 days. Voluntary PTO Donation Program Each of us may, at some point in our lives, have a family emergency or a personal crisis that causes a severe impact to lives, resulting in a need for additional time off more than our available PTO. This is when we most need our friends and family. To address this need, you may donate PTO from your unused balance to a co-worker provided they have requested to receive such donations and have met certain conditions.

Jury and Witness Leave A leave of absence for Jury Duty will be granted to you if you have been notified to serve. During this leave, if you are an exempt employee you will be compensated your regular salary for up to five days. After that, you may take PTO or leave without pay.

Bereavement Leave In the unfortunate circumstance of a death in your immediate family, you may, if you are an exempt full-time employee, take up to three days off to attend the funeral or make funeral arrangements. If you are a part-time employee, the paid time-off will be prorated when the funeral occurs on a scheduled work day.

Bonuses Performance/Spot and Annual Bonuses In general, we give performance/spot bonuses throughout the year based on the recommendation of your manager and at the sole discretion of Harmonia’s management. A performance/spot bonus is awarded when your contributions exceed expectations for exceptional performance.

We give annual bonuses during the holiday season depending on Harmonia’s performance for the year, the length of your association with the company, your team’s performance, your individual performance, your position and level of responsibility.

Other Benefits Gym Membership and Fitness Class Reimbursement Harmonia will reimburse you for gym membership or fitness classes up to $45 a month. Some of our locations have special deals with local gyms. Check with Human Resouces for your location.

Workplace Giving and Matching Gift Program Harmonia offers a Workplace Giving and Matching Gift Program. As part of our corporate social responsibility, we make contributions to several causes throughout the year. We also have a Matching Gift Program, through which the Harmonia will match donations made by you to your favorite charitable causes.

To be qualified for a matching contribution, the organization you donate to must:

• Be a legitimate 501(c)3 organization that is a social service, charitable, or an environmental organization and is focused on bringing about positive social change

• Be not-for-profit, not politically motivated, and non-religious • In no way benefit you or your firends and family directly or indirectly

CRC Hokie Passport If you work in our offices in Blacksburg which are located within the Virginia Tech Corporate Research Center, you are eligible for the CRC Hokie Passport identification card. Glossary Deductible The amount that must be paid per benefit period before insurance begins to pay its por�on of claims. The deduc�ble amount varies based on whether it is individual or family coverage.

Coinsurance The percentage or amount the member is required to pay in conjunc�on with their insurance plan a�er the deduc�ble has been met.

Copayment The amount a pa�ent pays up-front when services are received from a provider during an office visit.

Embedded Deductible On a medical plan with more than one covered person, the plan contains two components: an individual deduc�ble and a family deduc�ble. Having two components to the deduc�ble allows for each member of your family to have their medical bills covered prior to the en�re amount of the family deduc�ble being met. The individual deduc�ble is embedded in the family deduc�ble. For example, you and your daughter are on a plan with a $3000 family-embedded deduc�ble, and the individual deduc�ble is $1000. If your daughter incurs $1000 in medical bills, her deduc�ble is met and any subsequent medical bills for your daughter that year will be paid even though the family deduc�ble of $3000 has not been met yet.

Inpatient Hospitalization This is care given to a person who has been admited to the hospital and staying one or more nights.

Out-of-Pocket Maximum This is the maximum amount a member will pay out of pocket during the benefit period, which includes deduc�bles, coinsurance, and copayments. Once this amount is met, the plan will pay 100% of the allowed amount for the rest of the benefit period for covered services.

Out of Network An out-of-network provider is one not contracted with the health insurance plan. In many cases, the insurance company will either pay less or not pay anything for services you receive from out-of-network providers. These providers may also be subject to balance billing.

Qualifying Events Change of coverage under another employer’s plan, loss of coverage, marriage, child-birth, adop�on, death, legal separa�on, and divorce. This list is not all-inclusive of possible qualifying events. Some qualifying events such as the loss of dependent status (divorce, death) are mandatory events and you must no�fy HR immediately. Contact HR if you have ques�ons about qualifying events.
