Our Community Health Needs Assessment

In 2015, IU Health Goshen prepared a Community Health Needs Assessment (CHNA) for Elkhart County, Indiana, as required by the Patient Protection and Affordable Care Act of 2010. The purpose of the report was to understand and prioritize the health needs of the community and develop strategies reported to the needs. IU Health Goshen’s prior CHNA Report and corresponding Implementation Plan were adopted in 2012.

The 2015 CHNA Report is based on quantitative and qualitative community health information and provides a comprehensive understanding of health improvements that are needed. The health needs of Elkhart County were identified and prioritized through a systematic data informed process and with the input of individuals throughout Elkhart County and in the Elkhart County Health Department.

IU Health Goshen began assessing community health needs long before the Affordable Care Act of 2010, developing programs to benefit the community.– Susan Franger, Vice President, Cancer Services & Marketing

Surveys in English or Spanish were completed by over 600 members of the community, providing a broad cross-section of perspectives by local residents regarding community health issues. The IU School of Medicine, Bowen Research Center, Department of Family Medicine and the Indiana University School of Medicine analyzed the survey data, as well as demographic, health and economic secondary data for Elkhart County. Elkhart County Health Department officials identified critical public health-oriented priority concerns in the community.

After carefully evaluating this information, a comprehensive picture of the community’s health needs emerged, as well as the community’s prioritization of these needs.

Partnership with Maple City Health Care Center

Since 2012, IU Health Goshen has worked to increase community access to health services.

A leading initiative is its partnership with Maple City Health Care Center.

We have enhanced healthcare access through our partnership with the Maple City Health Care Center in Goshen.– Mark Lindemood, Chief Philanthropy Officer IU Health Goshen Foundation

IU Health Goshen and Maple City Health Care Center (MCHCC) have created a partnership committed to expanding access to primary care for an additional 5,000 members of the community. In November 2013 IU Health Goshen purchased the property located in North Goshen, often referred to as the Abshire Mansion. Through renovation of the existing structure and new clinical construction in 2014, a second location for MCHCC has been developed.

Community Health Programs

In 2012, IU Health Goshen enhanced its community programs in order to aggressively address other health needs identified in the 2012 Community Health Needs Assessment.

In response to community needs identified as priorities through the 2012 assessment, IU Health Goshen undertook initiatives and enhanced programming in a variety of areas. Among these were an array of fitness classes offered in a dedicated fitness center as well as fitness assessments for children and adults, obesity screenings and prevention programs. Educational workshops addressed nutrition, weight management and health literacy topics. Diabetes came particularly into focus as the topic of health fairs, preventive educational seminars and ongoing support group meetings. Cancer screening and education programs were offered with an emphasis on tobacco cessation.

Health Priorities for IU Health Goshen

After taking into account the health needs identified by the 2015 assessment of community and secondary data, IU Health Goshen has determined that its efforts can have the greatest impact on the following needs in the coming months and years.

Obesity. As the leading need of concern across the community, IU Health Goshen plans on continued and growing focus offering fitness, wellness, healthy lifestyle choices, and where appropriate, clinical interventions, to promote optimal health and quality of life for all members of the community.

Access to health services. Additional sites for providing convenient, excellent care are planned, such as the urgent care clinic under way, as well as adding more providers to serve the community. Efforts to educate all community members as to their options for health insurance and financial aid will also continue.

Mental health services. Tele-health access to behavioral health professional services is being arranged, to provide sorely needed services while offering convenience to the community and efficiency for the limited providers available.

Treatment of chronic conditions, with a focus on diabetes, hypertension and cancer risks. Early detection and vigilant management are keys to enjoying high quality of life in the face of chronic conditions. IU Health Goshen will continue evaluating and implementing highest-impact initiatives driving success, such as regular screenings, supportive groups and coaching, and medical management where appropriate.

Tobacco use. Community classes, individual education, remote educational sessions and outreach events and activities aimed at helping people quit, or never start smoking, are ongoing and expected to grow with staff dedicated to the effort.
