OCES Acquires Mayflower Retired & Senior Volunteer Program

OCES acquired Mayflower Retired & Senior Volunteer Program (RSVP) as of April 1, 2015. Mayflower RSVP was founded 41 years ago with the mission of helping adults age 55 and older remain active and healthy by providing them with volunteer opportunities in their communities.

Recently OCES was awarded a three-year grant from the Corporation for National and Community Service to operate the RSVP in Plymouth County with focus on Economic Opportunity, Education, Veteran and Military Families and Healthy Futures.

Cultural Competency

In 2014 OCES convened a Cultural Competency Committee consisting of 7 employees representing a variety of OCES programs. The goal of this committee was to first develop a Welcoming Statement to support the OCES mission and to illustrate that OCES welcomes and serves all individuals. In 2014, the committee was proud to announce the statement: “OCES aspires to welcome everyone without exception! Whether you are looking for help or are interested in helping others, we are here to support you”. To illustrate this powerful statement, the committee worked with Shields Design of Plymouth to create a plaque that could be posted in our offices and is pictured on the front of this Annual Report. The committee continues its work on incorporating this statement into all areas of agency practice such as our website, brochures, forms, and other formats to visually present our commitment.

Aging and Disability Resource Consortium

ADRCs serve people age 60 or older, and individuals with disabilities of any age. With a single contact to one of the member agencies, staff work together to connect individuals with the appropriate services and supports to help people live independently in the community. To the consumer this is a seamless process where No Wrong Door is really All the Right Doors!

The core functions of ADRCs are:• Information and Referral (I&R)• Transition Planning and Coordination• Options Counseling (OC)

The Aging and Disability Resource Consortium of Southeastern and Southern Massachusetts (ADRC SE SM) is a partnership between Aging Services Access Points (ASAPs) and Independent Living Centers (ILC). Together we ensure there is No Wrong Door when an individual contacts any one of the six member agencies for assistance or information about available services. The six agencies include Bristol Elder Services, Coastline Elderly Services, Independence Associates, OCES, South Shore Elder Services and Southeast Center for Independent Living.

Plymouth Supportive Housing

OCES is proud that the Executive Office of Elder Affairs (EOEA) awarded us a contract to expand our current Supportive Housing Program (SHP) in Plymouth, MA! The Massachusetts SHP, a collaboration of the EOEA and the Department of Housing and Community Development (DHCD), is available in 41 senior public housing locations statewide and serves 6,360 senior residents.

OCES and the Plymouth Housing Authority (PHA) work collaboratively, providing an “assisted living like“ environment, with on-site service coordinators as well as access to 24-hour on-call assistance provided by one of our provider agencies. In addition to daily services, OCES staff coordinate a variety of social, educational and recreational onsite events to increase resident interaction and to decrease social isolation. For example, the SHP reached out to the local Salvation Army Sergeant to coordinate food distribution at all 5 sites. This program is now serving 165 residents on a monthly basis! Another fun and meaningful event that has been spurred on by resident interest and SHP support is the development of a community garden. These are just a few examples of exciting events that are taking place.

Intensive Care Management

ICM is a new program developed with EOEA to respond to the increased need for intensive services to support individuals in either retaining or accessing home care services. ICM is a care management level that provides a more rigorous and time intensive delivery of support to individuals who may be resistant to services or with behavioral health needs that are putting them at risk. Through ICM, OCES has provided services to 13 consumers since the program began in January 2015.

Money Follows the Person

The overall purpose of the MFP Demonstration Program is to support and assist aging individuals and people with disabilities as they transition from long-term care facilities to community settings of their choosing. During FY15 OCES transitioned 53 people into a variety of locations including apartments, family homes, group homes, and Adult Family Care. The length of time people lived in facilities prior to transition back to the community ranged from 6 months to 8 years. OCES Transition Support Team Advisors and Registered Nurses worked collaboratively with each consumer and other community supports to make these transitions successful.

One situation involved Mr. N. who had resided at an area Skilled Nursing Facility for many years. Mr. N. had a history of homelessness, substance abuse, behavioral health issues and has physical disabilities; public housing was not an option. OCES worked with Mr. N. to develop a transition plan, taking into account his preferences, needs and history. Mr. N. is very social and decided he wanted the opportunity to live with a family; he was matched with a family in an Adult Family Care program. Preparing to move, Mr. N. shopped with OCES staff to purchase household items and furniture funded by MFP. Mr. N. described the shopping day as “the happiest day of my life”.

In 2015, at age 64 and after 8 years in the SNF, Mr. N. returned to the community. He has an Individual Supports Person through Massachusetts Rehabilitation Commission who provides Mr. N. with socialization opportunities and other community supports. Mr. N. is grateful and happy to have a second chance at living in the community.

Pets Eat Too The Pets Eat Too Program is in its second year. The program is made possible by a grant from the Banfield Charitable Trust; this year 79 consumers were served totaling 43 dogs and 69 cats. All of our consumers really appreciated the help. Pets make wonderful companions, offer unconditional love and can play an important role in healthy aging.

Thank you for bringing the cat and dog food to our home.I feel very blessed to have you in our lives.

I cannot find enough words to express how it feels to be able to hear again and to hear my great grandchildren talk to me. You are an asset to
