Directions for Completing Private Lessons Student Attendance Form To Private Lessons Instructors: Please read and follow this resource when completing your attendance forms. This is required by our Business Office for accurate accounting purposes and detailed facility use. 1. Please make sure you enter the beginning time of the private lesson and the ending time of the lesson. This will facilitate a more accurate billing. 2. Please print the student’s name clearly on the attendance sheet. 3. Have the student sign the attendance sheet. If there is a student listed, and there is no signature or the P/E/U is not circled, the instructor will be invoiced for that lesson. 4. Circle the appropriate letter in the box (P/E/U), which means present, excused, or unexcused. (To determine this status, refer to you instructor guidelines.) 5. The “Notes” section of the form is for your use. Many instructors use this to record notes of student’s progress for the director and for their own records. 6. Complete the instructor information at the top of the attendance sheet. Please print clearly. 7. Teacher signature is required on this form, before it is turned in to the director. 8. Turn in attendance sheets the same day you are at the campus. Use a new attendance sheet for each day/and each campus. There should be an appropriate place to put them so the director on duty can sign off on them. Please confirm this location with the campus director.
