Jurnalul de chirurgie este o revistă electronică, cu acces liber („Open
Access”), se adresează tuturor specialităţilor chirurgicale şi are drept obiective
asigurarea unui mijloc de informare eficient şi încurajarea tinerilor medici şi
cercetători de a-şi publica rezultatele activităţii clinice şi de cercetare.
Revistele electronice cu acces liber reprezintă platformele ideale pentru
publicarea rezultatelor cercetărilor întrucât articolele intră imediat într-un larg circuit
ştiinţific. Astfel, publicarea în Jurnalul de chirurgie asigură apariţia rapidă a
articolelor în format *.pdf şi indexarea acestora şi a rezumatului în IndexCopernicus,
DOAJ şi EBSCO Academic. Jurnalul de chirurgie publică următoarele tipuri de
articole: editoriale, articole de sinteză (review), articole originale, cazuri clinice, articole de tehnică şi anatomie
chirurgicală, articole multimedia şi de istorie a chirurgiei. Toate articolele sunt supuse unui proces de peer-
review. Membrii colectivului de redacţie asigură buna desfăşurare a procesului de recenzare în spiritul World
Association of Medical Editors (http://www.wame.org); autorii trebuie să respecte cerinţele International
Committee of Medical Journals Editors (http://www.icmje.org).
Începând cu 2012, Jurnalul de chirurgie apare într-un nou format, este patronat de Academia de Ştiinţe
Medicale şi, alături de revista Chirurgia, este agreat oficial de Societatea Română de Chirurgie şi utilizează
serviciile CrossRef (DOI) pentru indexarea articolelor.

STANDARD DE REDACTARE
Iniţializare pagină: Format A4, margini de 2,54 cm (1 inch).
Pagina de titlu:
Titlul: Times New Roman, 14, aldin (bold), centrat, la un rând; trebuie să fie cât mai scurt şi elocvent pentru
conţinutul articolului;
Autorii: Times New Roman, 12, normal, centrat, la un rând; vor fi notate: prenumele şi numele de familie,
gradul profesional. Trebuie precizate datele de contact ale primului autor sau ale autorului desemnat ca autor
corespondent: adresa de corespondenţă, telefon/fax şi o adresă de e-mail funcţională.
Apartenenţa autorilor: Numele instituţiei trebuie precizat în conformitate cu reglementările instituţionale.
Titlul prescurtat: titlu de 3-5 cuvinte, cât mai elocvent pentru articol.
Pagina rezumatului:
Rezumat în engleză: minim 200 cuvinte; Times New Roman, 10, la un rând, fără aliniate şi precedat de titlul
articolului scris în engleză, cu majuscule, urmat de cuvântul abstract (în paranteză, italic). Rezumatul trebuie să
fie structurat pe capitole: BACKGROUND, AIM, METHODS, RESULTS, CONCLUSIONS.
Cuvintele cheie (KEY WORDS) vor fi menţionate la sfârşitul rezumatului cu majuscule; de preferat acestea
trebuie alese din baza de date MESH (MEdical Subject Headings): www.nlm.nih.gov/mesh/MBrowser.html.
Textul propriu-zis al lucrării:
Textul: Times New Roman, 12, la un rând, structurat pe capitole: INTRODUCERE, MATERIAL ŞI METODA,
DISCUŢII, CONCLUZII.
Bibliografia: numerotată în ordinea apariţiei în text; Times New Roman, 10, la un rând, redactată după cerinţele
internaţionale (http://www.nlm.nih.gov/bsd/uniform_requirements.html). Referinţa bibliografică trebuie să
includă TOŢI autorii dacă sunt 6 sau mai puţini. Peste 7 autori vor fi notaţi doar primii 3 urmaţi de „et al.”
Numele revistei va fi notat în conformitate cu prescurtările PubMed, sau în întregime când acestea nu sunt
disponibile; redactarea acestuia se va face cu italice.

Tabelele vor fi inserate pe o pagină separată şi nu vor depăşi o pagină; titlul tabelului va fi numerotat cu cifre
romane: Times New Roman, 10, aldin, la un rând, deasupra tabelului. Formatul tabelului trebuie să fie cel
academic. Nu sunt acceptate tabelele salvate sub formă de imagini.
Figurile vor fi tipărite pe o pagină separată şi trimise în format *.jpg sau *.tiff. Nu sunt acceptate imaginile în
format *gif sau *png. Legenda figurilor va fi notată pe o pagină separată cu Times New Roman, 12, aldin, la un
rând şi vor fi numerotate cu cifre arabe.
Articolele multimedia:
Filmele şi prezentările Power Point vor fi însoţite de un rezumat consistent în engleză (de 300-500 cuvinte);
filmele vor fi în format *wmv, *.avi sau *.mpeg. Nu sunt acceptate filmele în format quick time. Fişierele
Microsoft Power Point (cu extensia .ppt) vor avea o dimensiune < 5Mb cu un număr de slide-uri <50.
Conflict de interese şi acknowledgements
Pe o pagină separată vor fi menţionate eventualele conflicte de interese şi „acknowledgements”.
Articolele vor fi adresate redacţiei în forma electronică (e-mail, CD, DVD) salvate în MS Word 1997-2003
(*.doc) şi eventual printate. Nu se acceptă articolele în format *pdf.
Articolele nu vor depăşi: lucrări originale - 15 pagini; referate generale - 15 pagini; cazuri clinice şi note de
tehnică - 8 pagini; recenzii si noutăţi - 2 pagini; comentarii/scrisori către redacţie - 1 pagină – maxim 10 titluri
bibliografice; articole multimedia: Power Point 50 Mb şi 50 slide-uri; fişiere video de maxim 5GB.

The glycocalix was first described in
1963 by Bennett as a polysaccharidic layer,
which covers the endothelial cells [1]. Long
time it was hard to evidence this anatomical
structure and it was mainly supposed by its
functional consequences. These difficulties
are related to the fragile nature of this layer
being rapidly destroyed by tissue preparation
and to the flow dependency, cultured
endothelial cells displaying poor glycocalix.
Modern in vitro or in vivo techniques
allowed glycocalix visualization and
research.
Today glycocalix is considered a
bulky, intravascular compartment, which
plays an important role in the homeostasis of
the vascular wall, blood flow and tissue
exchange regulation. It is the first barrier
between the intravascular space and cells.
Thus, the intravascular space consists of 3
compartments: cellular volume (mainly
erythrocytes), plasma volume and the
glycocalix volume.
The glycocalix structure is complex. It
may be described as a gel with a negative
electrical charge that “lubricates” the
internal side of vessels. At electron
microscopic view it looks like a “brush”
with a height of 60-110 nm lining the
vascular endothelium inside [1]. It is
composed of an array of macromolecules
anchored to the endothelial cells:
glycoproteins, proteoglycans and
glycosaminoglycans, but also contains
plasma proteins (albumin, fibrinogen),
enzymes (e.g., lipoprotein lipase, superoxide
dismutase) and growth factors (e.g. vascular
endothelial growth factor). This entire
structure is soaked with water. Glycocalix
and endothelial cells form together a
morpho-functional structure that is called
“endothelial glycocalix layer” [1].
The glycocalix physiology was
extensively studied in last decades and new
and complex functions are recently
described. The main functions described so
far are regulation of vascular permeability,
mediation of shear stress and prevention of
leukocytes and platelets adhesion [1].
The glycocalix pathology is uncovered
parallel to its physiology. Several injury
factors result in anatomical and/or functional
impaiment. Partial alteration (compaction,
fragmentation and shedding) consists of
selective enzymatic cleavage by heparinase
or hyaluronidase. This phenomenon is

accompanied by increased plasma level of
heparan, condroitin and other glycocalix
constituents [2,3]. Injury factors are
ischemia / reperfusion, hypoxia /
reoxygenation, inflammatory cytokines and
proteases and atrial natriuretic peptide [4].
Consequences of glycocalix injury are
increased capillary permeability (capillary
leak syndrome), tissue edema, enhanced
inflammation, hypercoagulability, loss of
vascular tone regulation [1].
The revised Starling theory According to the “classical” Starling
theory the capillary water exchange is
governed by the difference between
hydrostatic and colloid-osmotic pressure of
the intravascular and the interstitial space.
Water shifts out at the arteriolar and in at the
venular end of capillaries. A small part of
the interstitial fluid is drained to the
lymphatic circulation.
Anatomical and functional glycocalix
description resulted in the review of the
Starling theory [5]. The barrier to the
interstitial space is represented not only by
the endothelial cells, but by the morpho-
functional complex called “endothelial
glycocalix layer”. According to the revised
theory water exchange is governed by the
difference between hydrostatic and colloid-
osmotic pressure between the vascular
lumen and subglycocalix space devoided of
protein [1,5,6]. Colloid-osmotic pressure of
the interstitial space does not play a role in
the water movements. Thus, the rate of water
filtration at capillary level is lower than
predicted by the classical Starling theory and
the major route for water return into
intravascular space is lymph circulation [5].
Perioperative volume therapy and
glycocalix theory The intraoperative permissive
administration of volume solutions is based
on the concept of “third space” formation by
water shift from the intravascular to
interstitial space. But modern research
showed that “third space” does not exist.
Then the interstitial edema is the cause or
the consequence of “vigorous2 volume
administration?
According to glycocalix theory, there
are two types of water movements during
and after major surgery [6]. Type I, always
present, is physiological water and
electrolyte shift devoided of proteins from
the intravascular to the interstitial space by
an intact vascular barrier. Excessive
intraoperative administration of cristalloid
solutions will increase the volume of water
passing into interstitium. Type II,
inconstantly present, is pathological water,
electrolytes and macromolecules (proteins or
colloids) shift through an altered vascular
barrier. It depends on the location, extent
and duration of surgery. It is a result of
glycocalix degradation with two major
iatrogenic causes. On one hand, surgery
induces the glycocalix injury by mechanical
stress, endotoxin exposure, ischemia /
reperfusion. On the other hand, anesthesia is
commonly associated with excessive volume
administration. Hypervolemia stimulates the
secretion of atrial natriuretic peptide, which
causes glycocalix degradation with several
clinically significant consequences:
increased vascular permeability, tissue
edema, increased platelet aggregation [6].
This new theory of perioperative water
movements provides a logical basis for
volume therapy. Thus, minimizing type I
water shifts may be accomplished by
intraoperative cristalloid administration to
replace urinary and insensible losses and
colloid administration to replace blood
loss [6].
Minimizing type II shifts may result
from prevention of glycocalix degradation.
To accomplish these goals surgery should be
minimally invasive, with gentle visceral
manipulation, mechanical sutures,
atraumatic dissection and anesthesia may
include a neuraxial block and avoid
hypervolemia [6].
In conclusion, perioperative volume
therapy should not be governed by the
cristalloid / colloid or restrictive / liberal
controversy, but by the cristalloids and

colloids administration in optimal amounts
with optimal timing to minimize side effects
and to preserve the integrity of the
glycocalix.

THE IMPORTANCE OF CLINICAL TARGET VOLUME FOR RADIATION THERAPY IN
RECTAL CANCER (Abstract): Neoadjuvant chemoradiotherapy is the standard treatment in
locally advanced rectal cancer and it was demonstrated that radiation therapy associated with total
mesorectal excision reduces local recurrence rate. It is very important to precisely determine the
area that is subjected to radiation, in order to avoid side effects. The clinical target volume (CTV)
is dependent on tumor location, lymphatic drainage area and the degree of tumor invasion into
nearby organs. The CTV determination must take into account the internal organ motion during
the treatment. The radiotherapy simulating is possible due to specialized CT and/or MRI and
planning software. Using this method, it is possible to modulate the intensity of radiation on
tumor tissue, with minimal effects on normal tissues.

INTRODUCTION
Colorectal cancer rests the third most
common cancer in both sexes in Europe,
after prostate and breast cancers [1,2].
Approximately 30% of all colorectal cancers
are diagnosed in the rectum [1,2].
In the last three decades, cancer of the
rectum has seen many developments both in
terms of method of diagnosis and treatment:
mesorectal surgery (total mesorectal
excision, TME), postoperative chemotherapy
and preoperative chemoradiotherapy. At the
beginning of 2000s, preoperative
chemoradiotherapy followed by becomes the
new standard in the treatment of low-lying
locally advanced rectal cancer [3].
Currently, the management of rectal
cancer requires an individualized,
multidisciplinary approach. Multidisciplinary
team meetings significantly influence
decision for staging methods, neoadjuvant
treatment, sphincter saving surgery and
adjuvant treatment. This review discusses
the indications of the radiotherapy, different
treatment schedules and the important of
preoperative target volume in radiotherapy
in low rectal cancer.
RADIOTHERAPY’S INDICATIONS
Since 1975, controlled trials,
especially in United States, showed that
postoperative chemoradiotherapy decreased
rates of local recurrence and increased the
likelihood of survival compared with surgery
alone in patients with stages II and III rectal
cancer [4]. In the same period, in Europe
several studies were conducted on the
preoperative radiotherapy in patients with
resectable rectal cancer and without
metastasis [5,6].

Two treatment regimens have been
proposed: short schedule radiotherapy (25
Gy in five fractions), followed by surgery
within a week (the Swedish study) [3,7] and
long schedule radiotherapy (50,4 Gy divided
in 5 weeks) with surgery performed after 5-6
weeks (the Polish study) [8].
The advantages of preoperative
radiation include: reduction of local
recurrence; rectal tumor down staging;
possible sphincter preservation due to
reduction in tumor size; less irradiation of
small bowel and, therefore, less acute
toxicity; irradiation of well-oxygenated
tissues and theoretically greater eficacy;
avoidance of radiation of the “neorectum”.
1) Reduction of local recurrences
The first and primary indication for
neoadjuvant radio(chemo-)therapy in low-
lying locally advanced rectal cancer, in
addition to surgery is to prevent local
recurrences. The radiotherapy may prevent
the growth of cancer cells, which are not
excised during surgery.
The Dutch trial showed that short term
preoperative radiotherapy associated with
surgery with TME have a significant impact
in decreasing local recurrence, from 10.9%
to 5.6% compared to surgical treatment
alone [9]. The long term preoperative
radiotherapy (FFCD 9203 trial; EORTC
22921 trial) showed a significant difference
in local recurrence rate (5 years) which has
dropped to 8.1% when chemotherapy is
associated [10,11].
2) Downstaging
Neoadjuvant chemoradiation therapy
(CRT) for distal rectal cancer has resulted in
significant tumor downstaging and
downsizing, including complete tumor
regression. By switching the tumor in a
lower stage, it is possible to achieve
resection of a locally non resecable tumor.
Downstaging after preoperative radiotherapy
is dependent on the fraction size, total dose
applied and the time interval between the
first dose of radiotherapy and the date of
surgery. Evans et al. [12] demonstrated
increased rectal cancer downstaging when
the interval between preoperative radiation
and surgery had been more than 8 weeks.
3) Sphincter saving surgery
According to different trials [3,9,13-
17], rectal tumor shrinkage after neoadjuvant
therapy (radio-chemotherapy) increase the
possibility of applying a surgical technique
to preserve the anal sphincter (low anterior
resection rather than abdominoperineal
resection) and an important effect on quality
of life. The chance of sphincter saving
surgery was increased also by reassessment
of the rule of the 5 cm or even 2 cm for
distal margin [18,19].
The Lyon R90-01 randomized trial
comparing a short versus a long interval
preoperative radiotherapy, demonstrated an
increase in tumor response with long interval
(sterilized operative specimen 15% versus
5%) and a trend toward more sphincter
saving surgery in low rectal cancer (41%
versus 22%) [20].
FRACTION SIZE AND DOSE
The recommended dose for long
schedule radiotherapy in rectal cancer is 50
Gy, which is conventionally fractionated in
1.8-2.0 Gy. A mathematical model is used to
calculate the biological equivalent dose
(BED) of various treatment schemes [21]:
BED = nd (1+d / (α/β) ) – γ/α (T-Tk)
where n = number of fractions, d = dose per
fraction in Grays; T = duration of
radiotherapy in days, Tk = time until
repopulation starts.
The sensitivity of tissue is represented
by α/β and, for the rectal tumor cells, is
considered to be 10 Gy. It is supposed that
repopulation with tumor cells starts after 7
days and that subsequent loss of efficiency
(γ/α) is 0.6 Gy/day. These calculations
explained the using of the fractionation
schedule 5x5 Gy with the highest biological
equivalent dose of rectal tumor cells.
FOCUS SITES FOR RADIO-
THERAPY IN RECTAL CANCER
An important part of the radiation
treatment planning is to establish the region

of interest for the irradiation. In rectal
cancer, during the preoperative radiotherapy,
the small bowel is the most important organ
at risk, with adverse effects such as acute
radiation enteritis or late toxicity (e.g.
chronic diarrhea, bowel stricture, perforation
and hemorrhage) [22,23]. The development
of these reactions was highly correlated with
the dose and volume of small bowel
irradiation [24]. This risk can be reduced by
reducing the irradiated volume small bowel.
One of the methods for reduction in the
volume of small bowel irradiated is the
Intensity Modulated Radiation Therapy
(IMRT). According to this method, it is
possible to characterize the position and
motion of the small bowel, which is the
primary dose-limiting structure [25]. The
target volume can vary due to rectum and
bladder motion, depending on their state of
fullness.
Another important factor in defining
the area of interest for irradiation in rectal
cancer is the location of local recurrence.
Clinical investigations shows that the
frequently sites of loco-regional recurrence
can be classified into one of four categories:
1) low pelvis: below the top of the
acetabulum; 2) mid pelvis: from the top of
the acetabulum to the bottom of the
sacroiliac joint; 3) high pelvis: above the
bottom of the sacroiliac joint; 4) presacral:
within 2 cm anterior to the sacrum. [26]
The radiotherapy planning was
revolutionized by the ability to delineate
tumors and adjacent normal structures in
three dimensions using specialized CT and /
or MRI and planning software. Virtual
simulation, the most basic form of planning,
allows more accurate placement of radiation
beams than is possible using conventional
X-rays, where soft tissue structures are often
difficult to assess and normal tissues
difficult to protect.
The computed tomography (CT-
scans) is used to define a clinical target
volume (CTV), in order to be submitted to
external radiotherapy. This simulation of the
regions of interest is derived from the
Pinnacle planning system (Philips Radiation
Oncology Systems). This system allows the
user to visualize actual patient anatomy
based on the information provided by the 3D
data set. It also eliminates the need to
contour structures such as skin and bone to
visualize in 3D [27]. This planning system
performs the following:
- define the region of interest, which is
automatic isocenter placement;
- define beam sizes and orientations;
- shape the blocks with automatic
margins;
- dose grid resolution;
- treatment prescriptions;
- isodose normalization;
- dose volume histograms setup.
The images from the simulation help
the doctor to locate the tumor and better
identify the treatment area. This CTV is
performed in the prone position and vesical
repletion and contain the rectum and
perirectal tissues plus the regional
lymphatics (Fig. 1).

For middle and lower rectum tumor,
the CTV is accounted by the mesorectum.
The limits are represented beyond by the
sacro-recto-genito-pubic layers and inside by
the superior pelvirectal spaces. Backward,
the plan comes in contact with sacral
concavity and above with the anterior rectal
shell, which is represented by Denonvilliers
fascis in man and the recto-vagina fascis in

women. Upper, the limit of irradiation plan
is made by the S3 vertebrae. Below, the CTV
must stop at 1 cm under the anorectal
junction, corresponding to the anal sphincter.
For the tumors located in the upper
rectum, the irradiation area is defined as
follows: upper: the plane of 3 cm below the
inferior pole of the tumor; sideways: 3 cm
from the rectal wall anteriorly and on the
lateral sides; backward: 3 cm from the rectal
wall or in contact with sacral or vertebral
bone structures if this distance is less than 3
cm.
When the rectal tumor is locally
advanced, with regional invasion of
neighbor organs (sacrum, bladder, vagina,
uterus, prostate), the irradiated region should
include these areas and must exceed 5 mm
beyond the region.
The target irradiation volume should
include the corresponding lymphatic
drainage areas for the invaded organs: the
upper pelvirectal space, the ischio-rectals
fossa, the seminal vesicles and the prostate
for men and the posterior vagina wall for
women. For the lower rectal tumor, the
internal iliac lymph nodes should be
included in the CTV.
All these items are important in
determining a target volume to be submitted
to irradiation. Nuyttens et al. [25] showed
that CVT variations are unequal in all 3
planes. The authors recommend that the
target volume should be extended with 1 cm
above the tumor plane in order to create a
target volume that covers the anatomo-
clinical target volume. The acquisition of
detailed computerized tomography (CT)
imaging at the time of simulation, along with
three-dimensional (3D) treatment planning
software has been integrated with radiation
delivery hardware to create the modality
known as 3D conformal radiotherapy
(3DXRT).
CONCLUSION
Preoperative radiotherapy associated
with total mesorectal excision reduces the
risk of local recurrence in rectal cancer. The
radiotherapy planning was revolutionized by
the ability to delineate tumors and adjacent
normal structures in three dimensions using
specialized CT and / or MRI and planning
software. When the treatment volume
conforms to the shape of the tumor, the
relative toxicity of radiation on surrounding
normal tissues is reduced, allowing a higher
radiation dose to be delivered to the tumor.
The definition of the CTV which includes
both tumor and lymphatic drainage areas,
depending on tumor site, require a
simulation procedure that takes into account
multiple factors: tumor position, degree of
local invasion, movement of the rectum, the
fullness of neighbor organs, the position and
motion of the small bowel.

NON-MALIGNANT PORTAL VEIN THROMBOSIS IN LIVER CIRRHOSIS: DIAGNOSIS
AND ANTICOAGULANT TREATMENT (Abstract): It has been accepted that patients with
liver cirrhosis have a bleeding tendency related to the changes in the hemostatic system. However,
it has now been established that patients with cirrhosis are at risk for both bleeding and
thrombotic complications. These thrombotic complications include portal vein thrombosis, deep
vein thrombosis, coronary or cerebrovascular infarctions and pulmonary embolism. Portal vein
thrombosis (PVT) is frequently associated whith the advanced stages of liver cirrhosis. It has a
wide ranging clinical spectrum from being an asymptomatic state to a potentially life-threatening
situation. Development of PVT is often accompanied by complications, increased rate of
morbidity and mortality and may affect patient candidacy for liver transplant. With advances in
modern imaging techniques, PVT is being increasingly diagnosed. It is often difficult for the
clinicians to decide whether it is acute or chronic. No definitive evidence exists regarding the
treatment of acute portal vein thrombosis. Early anticoagulation with with low-molecular-weight
heparin results in a higher rate of recanalisation. Although anticoagulants seem to be the most
used treatment in the last few years, there is at present no consensus regarding the dose and
duration of treatment.

INTRODUCERE
Placa turnantă a tulburărilor
hemostatice în ciroza hepatică este ficatul -
locul de sinteză atât pentru factorii
coagulării cât şi pentru inhibitorii lor - cu
excepţia factorului von Willebrand.
Hemostaza în ciroza hepatică este într-un
echilibru extrem de instabil între procesele
protrombotice şi cele antitrombotice, direct
proporţional cu gradul insuficienţei hepatice,
reprezentând o provocare permanentă pentru
medicul practician.
În mod “clasic” pacienţii cirotici erau
consideraţi protejaţi de tromboze datorită
statusului lor „autoanticoagulant”; date
recente au demonstrat însă că tromboza
venoasă poate apare în ciroza hepatică chiar
în condiţiile profilaxiei medicamentoase [1].
ETIOPATOGENIE
Hipercoagulabilitatea din ciroza
hepatică poate fi sintetic exprimată prin
triada Virchoff:
1) flux scăzut;
2) vasculopatie asociată statusului cronic
inflamator;
3) scădere a proteinelor anticoagulante:
C, S, antitrombină.

Complicaţiile trombotice în ciroza
hepatică pot fi la nivel portal, mezenteric, pe
venele hepatice sau periferice, asociate sau
nu cu embolii pulmonare. Tromboza non-
neoplazică de venă portă (TVP) este o
afecţiune multifactorială complexă cu factori
favorizanţi multipli. Are o prevalentă între
0,6-26%, proporţional cu gradul severităţii
insuficienţei hepatice [2]. Astfel, dacă în
ciroza compensată prevalenţa este de 1%, în
cirozele pe lista de transplant variază între 8-
26% [3].
Pentru pacientul cirotic apariţia TVP
are semnificaţie negativă întrucât precipită:
hemoragia variceală, evoluţia mai severă a
cirozei şi, nu în ultimul rând, creşte riscul de
mortalitate posttransplant [4].
Factorii favorizanţi pentru TVP în
ciroza hepatică sunt [5-7]: sexul masculin,
chirurgia anterioară sau tratamentul
intervenţional al hipertensiunii portale,
sângerarea variceala anterioară, scăderea
numărului de trombocite, insuficienţa
hepatică severă. Mutaţia genei 20210GA a
protrombinei, a factorului V Leiden, creşte,
după ultimele studii, riscul de TVP dar
rezultatele sunt încă contradictorii [8].
Velocitatea fluxului portal venos este
invers proporţională cu scorul Child Plugh:
cu cât este mai mică cu atât insuficienţă
hepatică este mai severă. Astfel, dacă
velocitatea evaluată iniţial este sub 15 cm/s
incidenţa TVP este de 47,8%, comparativ cu
cei care au velocitate iniţială peste 15 cm/s,
care au incidenţa de TVP sub 2%. Mai mult,
velocitatea fluxului portal sub 10 cm/s este
considerată de unii cercetători ca factor de
risc independent de supravieţuire în ciroza
hepatică [2,7].
DIAGNOSTIC
Diagnosticul de tromboza recentă sau
„acută” non-neoplazică de venă portă este un
diagnostic dificil. Aprecierea vechimii
trombusului se poate susţine în două situaţii
[9]: 1) dacă pacientul a avut în urmă cu 60-
90 de zile explorare imagistica negativă
pentru diagnostic; 2) dacă pacientul cirotic
prezintă dureri în etajul abdominal superior,
febră, ascită instalată brusc, sângerare
variceală, deteriorare rapidă şi semnificativă
a rezervei funcţionale hepatice cuantificată
prin scorul Child Plugh.
Suspiciunea clinică şi echografică se
confirmă prin examinare ecografică şi Eco-
Doppler, angio-CT sau angio-RMN (Fig. 1,
2, 3, 4).

TRATAMENT
Tratamentul se face de obicei cu
heparină cu greutate moleculară mică
administrată în funcţie de greutatea
corporală: 0,5 mL x 2/zi la 50-60 kg, 0,6 mL
x 2/zi la 60-90 kg şi respectiv 0,9 mL x 2/zi
pe zi la peste 90 kg [10]. Se monitorizează
pacientul clinic, biochimic şi imagistic
(ecografie Doppler, angio CT, angio-RM).
Sunt puţine studii publicate în literatură cu
privire la frecvenţa, beneficiile şi efectele

adverse ale tratamentului anticoagulant în
TVP non-neoplazică.

În lipsa consensurilor şi a ghidurilor
aceste studii recomandă [9,14]:
- instituirea precoce a tratamentului,
ideal în primele 14 zile de la instalarea
TVP (în practica curentă este extrem
de dificil de stabilit vechimea
trombozei). Cu cât tratamentul se face
mai devreme, cu atât procentul de
recanalizare (completă sau parţială) a
sistemului port este mai crescut, rata
recanalizării fiind variabilă, între 33%
şi 45%. Aceste procente se pot
menţine şi chiar îmbunătăţi sub
tratament la 6 şi respectiv 12 luni.
- tratamentul odată instituit, cu răspuns
favorabil (recanalizare), necesită
menţinere indefinită în timp. Oprirea
anticoagularii aduce pacientul în
condiţiile fiziopatologice iniţiale care
au determinat tromboza. Studiul lui
Amitrano arată recurenţa trombozei, la
oprirea anticoagularii, într-un procent
de 27-38% [12].
- efectele adverse hepatice şi
extrahepatice ale anticoagularii sunt în
general minore, accentuate la cei la
care nu s-a obţinut recanalizarea. Un
factor predictiv cuantificat uşor este
numărul de trombocite, mai mic de
50.000 / mm3. Acesta este legat de
riscul de sângerare la nivelul tractului
digestiv inferior, extracţii dentare sau
sângerări genitale.
CONCLUZII
Se poate afirma că tratamentul
anticoagulant în TVP non-neoplazică din
ciroza hepatică este eficient, cu efecte
adverse minime. Recanalizarea se obţine în
aproximativ 50% din cazuri. Tratamentul se
menţine indefinit în timp datorită riscului
crescut de recurenţă.
Întrucât nu există ghiduri şi consensuri
internaţionale rămân în prezent extrem de
multe incertitudini:
- Stabilirea categoriilor de pacienţi cu
ciroză hepatică în afara celor de pe
lista de transplant care ar beneficia de
tratament anticoagulant;
- Profilaxia primară a TVP non-
neoplazică ar trebui făcută în toate
cirozele hepatice?
- Screeningul echografic pentru
diagnosticul precoce al TVP non-
neoplazice cu raport cost/beneficiu
favorabil la ce interval de timp ar fi
util (3 luni, 4 luni)?
- În cazul eşecului de obţinere a
recanalizarii se poate recomanda de
rutină şunt portosistemic transjugular
şi transplant?
Toate aceste incertitudini deschid noi
arii de cercetare pentru studii multicentrice

şi multinaţionale care să răspundă
întrebărilor privind corecţia tulburărilor de
coagulare, influenţei coagulopatiei şi
trombocitopeniei asupra evoluţiei şi
prognosticului cirozei hepatice. Sunt
necesare ghiduri şi consensuri care să
standardizeze tratamentul tulburărilor
hemostazei în ciroza hepatică.

TRANSTHORACIC VERSUS TRANSHIATAL ESOPHAGECTOMY: COMPARATIVE
STUDY REGARDING SURGICAL APPROACH IN ESOPHAGEAL CANCER (Abstract):
BACKGROUND: Surgical resection can offer the best curative treatment for oesophageal cancer
but is associated with high postoperative morbidity rates. Most common surgical approaches are
transthoracic (TT) and transhiatal (TH) techniques. Transhiatal approach has the advantage of
reducing the pulmonary morbidity in patients with impaired pulmonary function. AIM: The aim
of this study is to compare the TT and TH approach, in terms of preoperative assessment and
short term outcomes. MATERIALS AND METHODS: We performed an observational study on a
prospective collected database which included all the patients diagnosed with oesophageal cancer
in which surgery was performed. A detailed assessment of comorbidities was performed using
several scales: Charlson and age adjusted Charlson score, physiological score of POSSUM.
Postoperative complications were graded according to the Dindo-Clavien classification.
RESULTS: During a 9 years period surgery was performed in 50 cases, 33 by TT approach and
17 by TH approach respectively. The mean age was 58.7 ± 2.21 years old (95% CI 56.3-61.2).
Patients in the TH group had a higher Charlson score (3 vs 2, P = 0.01), age adjusted Charlson
score (5 vs 4, P = 0.03) and physiological score (17 vs 15, P = 0.04). TT techniques were mainly
used for middle oesophageal cancers (69.7%) and TH for lower oesophageal tumors (82.4%). The
overall operative morbidity was 60% with no difference between the two groups even for minor
and major complications. Pulmonary complications occurred in 23 cases (46%), cardiac
complications in 5 cases (10%), anastomotic leakage in 6 cases (12%) and recurrent nerve
paralysis in 6 cases (12%). Multivariate analysis showed that age adjusted Charlson score
(OR = 2.77; 95%CI 1.114-6.9236) and physiological score (OR = 1.7601; 95% CI 1.2067-2.5674)
were predictors for complications. CONCLUSION: In our study mortality and morbidity showed
no statistical difference in relation to the surgical approach. An accurate preoperative assessment
and tailoring an adequate surgical approach can limit the percentage of postoperative
complications.

BACKGROUND
Esophageal cancer is the seventh
cancer related deaths with 482,300 new
cases per year [1] and usually occurs as
either squamous cell carcinoma usually in
the middle and upper part of the
mediastinum or adenocarcinoma in the lower
third and the gastroesophageal junction.

There has been a shift in the incidence of the
histological subtypes of oesophageal cancer
with the incidence of adenocarcinoma rising
in the past 3 decades. The greatest recent
changes in the management of oesophageal
cancer are related to the treatment and
surveillance of Barrett esophagitis and
management of early stages oesophageal
cancers. For locally advanced tumors
surgery alone provides poor prognosis [2]
and in these cases induction therapy is often
used with the aim of increasing the R0
resections rate, treatment of micrometastasis
and improvement of survival. Surgical
resection provides the best survival
advantage for early stages and in tumors
with good response after neoadjuvant
treatment. The type of surgical approach
varies depending on surgeon and
institutional preference; the two most
utilised techniques are the transthoracic with
cervical or mediastinal anastomosis and
transhiatal techniques. The reference and
most cited largest randomised trial
comparing the two surgical approaches
included 220 patients with adenocarcinoma
who were assigned to either transhiatal
resection or Ivor Lewis technique [3,4]. The
transhiatal group had shorter hospitalisation
(19 vs 15 days), less blood loss and less
overall morbidity (57 vs 27%, P < 0.001). In
the other group more lymph nodes were
resected (31 vs 16, P < 0.001) with no
difference in terms of R0 resection rate,
postoperative mortality (4 vs 2%, P = 0.4)
and survival. The overall morbidity rate after
esophagectomy ranges between 50 to 60%
[5,6], the most common being the
cardiopulmonary complications.
The aim of our study was to compare
the two major approaches for
oesophagectomy in a group of patients
operated for oesophageal cancer in terms of
preoperative assessment and early surgical
outcome.
MATERIAL AND METHOD
We performed an observational study
analysing all the patients who underwent
esophageal resections for oesophageal
cancer, from January 2004 until March
2013. Tumors of the cervical oesophagus as
well as subcardial tumors (Siewert III type)
were excluded from this analysis.
Preoperative workout consisted in
medical history and examination, functional
evaluation, barium contrast esophagogram,
endoscopy with biopsy, thorax and
abdominal CT scan, abdominal ultrasound.
Functional status was assessed by means of
routine blood tests, electrocardiogram and
pulmonary function tests. Second line
investigations such as: cardiac ultrasound
and cardiopulmonary stress tests were
performed when necessary.
After initial diagnosis and staging,
patients with locally advanced tumors were
submitted to a neoadjuvant protocol
according to the functional status.
Comorbidities have been quantified
accordingly to the Charlson comorbidity
scale [7] and age adjusted Charlson
comorbidity scale [8] and included:
hypertension (systolic blood pressure higher
than 140 and/or diastolic pressure higher
than 90); cardiac dysfunction (history of
ischemic heart disease, unstable angina,
cardiac failure, atrial fibrillation), pulmonary
dysfunction (FEV1 < 70% of predicted
values and / or vital capacity < 80%),
vascular dysfunction (history of abdominal
aortic aneurism, vascular bypass, carotid
stenosis), neurological dysfunction (history
of cerebrovascular accident or transitory
ischemic attack, epilepsy or Parkinson), liver
cirrhosis, chronic renal failure, diabetes
mellitus on treatment. In order to have a
more accurate information regarding
functional status of the patients we
calculated the physiological score of the
POSSUM (Physiological and Operative
Severity Score for the enUmeration of
Mortality and morbidity) based on the
original description and codification of each
component [9].
The POSSUM score was developed to
allow risk-adjusted prediction of
postoperative mortality and morbidity in
surgery according to the physiological
condition of the patients, severity of surgery,

age and general health and consists of a
physiological part (12 factors with 4 grades
score) and an operative part (6 factors with 4
grades).
Surgical procedures:
Transhiatal approach
The patient was placed in the supine
position with the head turned toward the
right and the neck extended. After
laparotomy, the stomach was mobilised by
dividing the gastrocolic ligament while
preserving the right gastroepiploic vessels.
The short gastric vessels were divided.
The esophagogastric junction was freed from
the hiatus; the lesser omentum and the
gastrohepatic ligament were divided. The
left gastric artery was ligated and divided
with celiac lymph nodes dissected. The
oesophagus was dissected under direct
vision through the widened hiatus of the
diaphragm up to the inferior pulmonary vein.
A left cervical incision is made along
the anterior border of the left sterno-cleido-
mastoid muscle and dissection is carried out
along the prevertebral space. When the
cervical oesophagus was found, blunt
dissection was performed down to the level
of the thoracic outlet; blunt dissection was
carried out along the oesophagus until the
entire organ was mobilized. Several
centimetres of oesophagus were gently
pulled up into the cervical region and then
transected.
The distal stump was carefully closed
and the stomach and oesophagus were then
removed through the diaphragmatic hiatus.
After Kocher’s manoeuvre and pyloroplasty,
the oesophagus, cardia, and an area along the
lesser curvature were resected and the stump
was closed using a linear stapler or a running
suture.
The stomach graft was delivered to the
neck wound through the posterior
mediastinal route by gentle traction using a
silicone drain attached to the proximal part
of the gastric tube and a hand sewn
anastomosis was performed.
Transthoracic approach
Based on a surgeon’s preference, when
a triple approach was chosen, a right
thoracotomy is performed as an initial step
of the procedure and included removal of the
azygous arch, low paratracheal, subcarinal,
paraesophageal and parahiatal nodes in
continuity with the resected esophagus. For
tumors of the middle part of the thoracic
oesophagus, the right reccurential lymph
nodes were removed. The pleural cavity was
drained and the chest wall was closed.
Afterwards, the patient was placed in a
supine position, in the same manner as for
the transhiatal approach. The cervical and
abdominal part of the intervention was
similar to the transhiatal technique described
above. This represents a modified McKeown
technique; we prefer to perform thoracotomy
as the first step to allow a better assessment
of resectability in order to avoid potential
unresectable locally advanced tumors. The
stomach graft is pulled to the neck incision
in similar fashion as in the transhiatal
technique.
When the Ivor-Lewis technique is
performed, the initial step was a median
laparotomy with gastric mobilisation with
preservation of the right gastric and
gastroepiploic vessels and resection of the
upper third of the stomach. After the
abdominal stage, a right posterolateral
thoracotomy is done allowing resection of
the oesophagus and mediastinal
lymphadenectomy. The intrathoracic
anastomosis is performed manually.
In all patients a pyloroplasty and a
jejunostomy feeding tube inserted at the end
of surgery in order to provide accurate
nutrition in the early postoperative period
were performed. In patients with previous
gastric surgery a colic interposition was
preferred.
Pathologic examination was recorded
in all patients and tumors were classified
using the UICC TNM 7th classification.
Postoperatively, patients were
transferred to the intensive care unit and
extubated when they were hemodinamically

and respiratory stable; jejunostomy feeding
was administered in the first postoperative
day, swallow X-ray studies were performed
on the 7th postoperative day to assess the
anastomosis and gastric tube passage and a
liquid diet was initiated when there were no
signs of leakage. Postoperative
complications in any of the patients
undergoing either of the two approaches
were noted. Postoperative morbidity and
mortality were defined as any complication
or death which occurred within 30 days after
surgery.
Complications were graded according
to the Dindo Clavien classification [10]
based on the therapeutic consequences.
Grade I and II were considered minor
complications and grade III and IV as major
complications, grade V complications was
considered any major complications leading
to the death of the patient.
Based on these considerations, patients
were divided in two groups: patients who
underwent transhiatal approach (TH group)
and respectively, patients who underwent
transthoracic approach (TT group).
Statistical analysis
Statistical analysis was performed
using MedCalc v 9.2.0.1. Medians and
interquartile ranges at the 25th and 75th
percentile were calculated and subsequently
depicted when relevant. Mann-Whiney U
tests and Chi-square testes were used when
appropriate. Significance was set at
P < 0.05.
RESULTS
Between January 2004 and March
2013, fifty consecutive patients diagnosed
with oesophageal cancer underwent surgical
resections.

Patients’ characteristics are
summarised in Table I. There were no
significant differences between the groups in
terms of age. All of the patients were
considered on preoperative assessment by
the anaesthesiologist of being ASA 2 or 3.
All scores which included a thoroughly
assessment of the physiological status or of
the co-existing comorbidities were
statistically significant between the two
subgroups of patients. Patients in which
thoracotomy was performed had a better

Charlson score (P = 0.01), age adjusted
Charlson score (P = 0.03) and a better
physiological score of POSSUM (P = 0.04).
In terms of pre-existing conditions patients
in which thoracotomy was performed had a
lower incidence of cardiac, pulmonary, liver
diseases and diabetes.
There was no difference in terms of
neoadjuvant treatment, although there was a
tendency for performing the induction
therapy on the patients from the last 5 years
included in the study.
The pathological characteristics of all
the patients are summarised in Table II. In
the vast majority of cases the tumor was
localised in the middle and lower part of the
esophagus, tumors considered Siewert I and
II were encountered in 14% of all cases
(n = 7). There were differences in terms of
surgical approach based on tumor site: the
transthoracic approach was more frequent
used for tumors located in the middle third
of the oesophagus and the transthoracic
approach, for tumors located in the lower
part of the oesophagus. The predominant
histology (72%, n = 36) was squamous cell
carcinoma. As a rule, tubulised stomach was
used for reconstruction, in 6 cases it was
used colon interposition graft due to the
previous gastric surgery. In 44 cases
anastomosis was performed in the neck, the
remaining 6 cases being intrathoracic
anastomosis, all of the cases were hand sewn
anastomosis. Final pathological staging
showed a predominance of the locally
advanced tumors (stage III, 52%, n = 26),
with no difference in terms of surgical
approach. The patients were usually
transferred intubated in the Intensive Care
Unit and were extubated the next day.
Postoperative morbidity occurred in
60% of the cases (n = 30) and was mostly
represented by cardiac and pulmonary
complications (Table III). Eighteen patients
developed minor complications according to
Dindo-Clavien classification and 15
developed major complications. There were
no differences in terms of overall, minor or
major complications between the two
subgroups of patients. Postoperative
mortality rate was 10%, with no difference
based on surgical approach. In these cases
death was due to the: sepsis secondary to
anastomotic leakage, bilateral broncho-
pneumonia, massive myocardial infarction,
stroke and severe pancreatitis. Mean hospital
stay was 14±1.06 days and ICU stay 6±0.55
days with no difference between surgical
approaches. Multivariate analysis as seen in
Table IV showed that age adjusted Charlson
score and physiologic score of POSSUM
were independent prognostic factors
affecting postoperative complications.

DISCUSSION
Despite the recent shift in management
of oesophageal cancer to perform
neoadjuvant chemo-radiotherapy, surgical
resection remains the most important part of
multimodal treatment for early stages and
locally advanced disease [11]. The best
surgical approach, in terms of outcomes and
quality of lymphadenectomy is still a subject
of debate. Although the minimally invasive
techniques recently developed are used, the
traditional surgical approaches are still the
most used in surgical practice.
Several variables as comorbidities,
tumoral stage, surgical technique (especially
the type of approach) and surgeon and
hospital experience should be considered
when an audit of operative outcomes is
performed.
A multicentre study regarding the
preferred surgical techniques in oesophageal
cancer [12] showed that the most common
approaches are the transhiatal (35-44%) and
Ivor Lewis (36-50%) with variability
regarding the site of the tumor. In a small
number of cases “en bloc” resection (3.9-
6.5%) and the three fields lymphadenectomy
(13% for proximal tumors and 1.3% for
gastroesophageal junction tumors) were
preferred.

Te prognostic scores based on general
evaluation of the patient (ASA score) and on
the overall comorbidities (POSSUM,
Charlson and age adjusted Charlson scores)
were significantly different in the two
groups of patients (Table I). This is probably
explained by the selection of patients
suitable for a transthoracic resection;

basically, the thoracotomy was not
performed in a patient with an impaired
cardiac and pulmonary function and this was
directly reflected in the scores for functional
assessment.
Age did not differ in the two groups,
result which is concurent with literature data
[13]. Comorbidities and overall functional
capacity are more likely to affect
perioperative morbidity more than age alone
[14]. Patients with poor pulmonary status
(forced vital capacity of less than 80% and
forced expiratory volume in one second less
than 70% of predicted value) have been
associated with higher risk for pulmonary
complications [15].
In our series patients suffering from an
impaired pulmonary function were not
considerate suitable for thoracotomy, and a
transhiatal approach was used.
Other organ dysfunction can increase
the risk of postoperative morbidity and
mortality.
Liver cirrhosis can be a
contraindication for surgery in some centres
although Child A cirrhotic patient can
undergo esophagectomy with acceptable
mortality and morbidity [16]. All the patients
in our series with a preoperative impaired
liver function developped a major
complication and death occurred in two
cases, no matter the type of surgical
approach.
Based on the results of a meta-
analysis, the mortality rate after
esophagectomy varies from 0 to 22%
depending on the selection of the patients,
experience of surgical team and the level of
postoperative care [17]. Low volume centres
(less than 5 cases per year) had a median of
18% mortality and high-volume centres
(more than 20 cases per year) a median 4.9%
mortality. We had a 10% mortality rate by
performing 5 to 6 oesophagectomies per
year.
Pulmonary complications are the most
frequent complication after esophagectomy
and have been the leading cause of two-
thirds of postoperative mortality [18]. In a
10 year review of 1,777 oesophagectomies it
was reported a 49.5% rate of complications
of which pneumonia represented 21.4%,
respiratory failure which required
reintubation in 16.2% [5]. The choice of
approach may also have a direct impact on
respiratory complications. In a randomised
study of 220 patients to a transthoracic
versus a transhiatal approach for mid to
distal oesophageal cancer, the comparison
revealed a significantly higher rate of
pulmonary complications in the
transthoracic group (57% vs 27%,
P < 0.001), but with the same 5-year
survival [3].
In our study we noted similar rates of
pulomary complications. Given the study
type (observational) we cannot conclude that
both surgical approaches are similar in term
of pulmonary morbidity rate. However we
can conclude that a tailoring surgical
approach can decrease the pulomonary
complications.
Cardiovascular complications after
esophagectomy includes a large group of
events, in which atrial fibrillation remains
the most common, with reported rates from
11.5% until 22% [6,19]. It is very important
to prevent this type of complication because
of the concern that conduit perfusion can be
altered in the periods of relative hypotension
precipitated by atrial fibrillation. In our
series we encountered atrial fibrillation as a
direct effect of another major complication,
such as sepsis following anastomotic leak.
Recurrent nerve injury occurs during
dissection of the cervical oesophagus and it
is associated with the site of the
anastomosis. The results of a meta-analysis
[20] showed that the cervical anastomosis
increases this risk (OR = 7.14; 95%CI: 1.75-
29.14; P = 0.006). In our series we found to
have significantly higher rates of recurrent
nerve injury following the transhiatal
approach, although the major part of the
anastomosis was performed in the cervical
region.
One of the most challenging steps of
the esophagectomy is the preservation of an
accurate perfusion at the site of the
anastomosis; compromised conduit

perfusion can lead to a reduced blood flow
with an impaired healing and a high risk for
fistula [21]. This is important when
performing a cervical anastomosis that
requires a longer gastric conduit with a high
risk of diminished blood flow. The
anastomotic leakage rates are reported in the
literature from 10.9% to 16% [3,6,22,23]. A
large study [24] reported a 2% rate of gastric
tip necrosis that required reintervention. The
endoscopic evaluation of oesophageal
replacement revealed gastric conduit
ischemia in 9.2% of patients [23]. The
thoracic anastomosis requires a shorter
conduit length and less tension which are the
key elements for a lower leak rate. The
disadvantages of this site of the anastomosis
are the opening of thoracic cavity and the
possible consequences of an intrathoracic
fistula. Although in our series there were no
differences in terms of surgical approach we
encountered only one fistula by performing
an intrathoracic anastomosis, all the others
were cervical site fistulas after either
transhiatal or McKeown techniques. So, the
choice of anastomotic location remains
surgeon dependent based on personal
experience and awareness of outcomes.
Our study has several drawbacks. The
first one is that represents a small number of
patients and this can limit the accuracy of
the results. This is the direct result of
relatively low percentage of patients suitable
for surgery in Romania; many of the newly
diagnosed cases are locally advanced or
metastatic lesions in a patient with severe
comorbid conditions. The second major
drawback is the heterogeneous cohort of
patients, we had a relatively long inclusion
period and this induced a bias in the
appliance of multimodal treatment,
perioperative management in terms of early
extubation, fluid restriction management,
pain management and respiratory
rehabilitation. Also, the management of
postoperative complications was relatively
different during the inclusion period, with a
higher clinical suspicion toward the
occurrence of a complication after even a
minor change in the postoperative course of
a patient.
CONCLUSIONS
Surgery is still the standard of care and
the best way to cure or palliate patients with
oesophageal cancer. Significant
improvement in oncologic staging and
functional preoperative evaluation has led to
better patient selection and better outcomes.
Despite these improvements, esophagectomy
is still associated with high morbidity and
mortality risks. In order to minimalize the
surgical trauma to the patient a various
surgical approaches are used to treat
patients. The surgeon should put in balance
the oncologic principles of surgery
(complete removal of the tumor with
negative longitudinal and circumferential
margins and adequate lymph node removal)
and the risks of a specific surgical approach.
The best way to reduce the surgical impact is
to personalise the surgical approach to
patient and tumor characteristics.

LIFE THREATENING BLEEDING FROM A GASTROINTESTINAL STROMAL TUMOUR:
A CASE REPORT (Abstract): Gastrointestinal stromal tumours (GIST) are rare tumours of the
gastrointestinal tract. They can cause massive gastrointestinal haemorrhage as illustrated in our
case. Our patient was admitted to the emergency in shock due to massive haematochezia. The
patient was revived with multiple transfusions and crystalloids. 99m-Tc labeled Red Blood Cells
(RBC) scan showed bleeding from right upper quadrant of abdomen. Laparotomy revealed a
tumour in jejunum which was resected and proved to be GIST by immunohistochemistry. The
patient improved dramatically after surgery and his haemoglobin level stabilized at 10g/dL.
Repeat RBC scan revealed that there was no bleeding. The postoperative recovery was
uneventful. CONCLUSION: GISTs are rare but clinical important tumours. As in our case they
can cause life threatening bleeding. Intensive care to stabilize the patient and modern diagnostic
methods like RBC scan has proved invaluable. Segmental enterectomy has been the definitive
treatment the tumour and the gastrointestinal haemorrhage.

INTRODUCTION
Gastrointestinal stromal tumours (GIST) are
rare tumours of the gastrointestinal tract.
They originate from gastrointestinal
pacemaker cells and are characterized by
over-expression of the tyrosinekinase
receptor cKIT. [1,2]

A 42 year male presented to the
emergency department with complaints of
massive bleeding per rectum after he was
operated by a local quack for suspected
haemorrhoids two days back. The patient
was in shock and very pale. At admission his
blood pressure was 60/40 mmHg, pulse
120/min and was having acute respiratory
distress.
Large volumes of crystalloids were
infused in an attempt to revive him, but he
was having severe respiratory distress and
ongoing bleeding per rectum. He was
intubated and shifted to the intensive care
unit. Routine investigations revealed

haemoglobin 2.6 g/dL and platelet count
40,000/mm3. Multiple transfusions of
packed RBC and platelets were used to
stabilize him. Examination under anaesthesia
was done in the operation theatre in
suspicion of bleeding haemorrhoids, but the
source was not found in the anorectum. It
was inferred that the bleeding was from
more proximal part.
After stabilising a RBC scan was
performed on the patient. The source of
bleeding was reported to be in the right
lumbar region of the abdomen (Fig 1).

A provisional diagnosis of
angiodysplasia of right colon was made. The
patient was further transfused packed RBC
and platelets and taken up for emergency
exploratory laparotomy. On laparotomy a
firm growth of about 4 cm diameter was
found in the antimesenteric border of
jejunum about one and a half feet distal to
the duodenojejunal flexure (Fig. 2).

The vessels supplying the growth
appeared dilated and thin walled. No
regional lymph node was enlarged. No
peritoneal deposits could be found. The
growth was resected with the segment of
jejunum containing it with adequate free
margin of 5 cm and end-to-end anastomosis
of jejunum was done (Fig. 3). The patient
improved dramatically after surgery and his
haemoglobin level stabilized at 10 g/dL.
Repeat RBC scan revealed that there was no
bleeding. (Fig. 4) The postoperative
recovery was uneventful.
Histopathologic study revealed a
spindle cell tumour arising from the smooth
muscle layer of jejunum. Immuno-
histochemistry for KIT receptor tyrosine
kinase (KIT, CD 117) was positive and the
diagnosis of GIST was confirmed. The
patient has thrived well and with no
sequelae.
DISCUSSION
The mesenchymal tumours have been
classified over the years into leiomyomas,
schwannomas, leiomyoblastomas or
leiomyosarcomas. But in recent years there
has been more insight into the origin of these
tumours. Mazur and Clark in 1984 reported
that many supposed smooth muscle tumors
lacked immunohistochemical or electron
microscopic evidence of smooth muscle or
neural characteristics, and they suggested
that the term ”gastric stromal tumor” would

be more appropriate [1]. Kindblom proposed
that these tumours arise from the interstitial
cells of Cajal which are pacemaker cells of
the gut [2]. They originate from
gastrointestinal pacemaker cells and are
characterized by over-expression of the
tyrosine kinase receptor cKIT.
Though rare, gastrointestinal stromal
tumours (GIST) are the most common
mesenchymal tumours of the gut. GISTs are
rare tumours but the most common
mesenchymal tumours of the gastrointestinal
tract. Studies from Finland show the annual
incidence to be about 10-20 / million [3].

Most GISTs arise in the stomach
(about 60%) and small intestine (30%), and
less commonly from the duodenum, the
colon and rectum. GIST of oesophagus is
extremely rare [4-6]. No significant
difference in incidence between males and
females has been noted. Median age of
presentation is 69 [7].
The symptoms depend upon the size of
the tumour. Small GISTs are usually
asymptomatic and are incidental findings in
laparotomies for other conditions. Larger
tumours may present as bleed, abdominal
masses, cause obstruction or rarely perforate.
Bleeding is the most common presentation
of GIST (about 50%) [8]. Bleeding can be
chronic causing unexplained anaemia or may
be acute. Acute bleeding can be life
threatening necessitating emergency
management [9]. About 10% cases at
presentation have metastatic disease. The
most common site of secondaries is liver.
Peritoneal metastases are uncommon and
lymphatic spread is rare [5].
There are no recognized specific
radiological examinations for GIST
diagnosis. Endoscopy can reveal submucosal
masses and ulcerated mucosa. But often the
mucosa appears normal. Contrast enhanced
CT is mainstay of diagnosis. It can assess the
size and the location of the tumour. It can
also diagnose hepatic metastasis. [10]
Obscure gastrointestinal bleeding is
very difficult to diagnose. Newer diagnostic
techniques like video capsule endoscopy
(VCE), contrast enhanced CT, CT
angiography and 99m-Tc labelled Red Blood
Cells (RBC) scan can localize the source of
bleeding [11]. Orellana reported a high rate
of correct localization of active GI bleeding
up to 93% by RBC scans [12]. Positron
emission tomography with 18-FDG is
particularly useful for detection of secondary
localisation of GIST, but has been chiefly
used for patient follow-up [13, 14].
The primary treatment of GIST is
complete excision with surrounding tissue.
The survival correlates directly with the
completeness of the removal. Resectable
GIST with low risk factors should be
observed only and the high risk cases should
be considered for imatinib. The unresectable
GIST should be considered for imatinib or
complementary resection. The GIST not
responding to imatinib may be managed by
dose escalation or trial of experimental
agents [15].
Synchronous hepatic metastases must
be resected if surgically feasible. The
unresectable tumours may be treated with

neoadjuvant Imatinib and subsequent
complementary resection. Even if a tumour
is unresectable, resection is palliative for
mass effect and bleeding [16]. Lymph node
dissection is usually not needed because like
sarcomas, GIST metastasizes through blood
and lymphatic spread is very late.
Imatinib is a tyrosine kinase inhibitor
and offers very specific antitumour effect
against GISTs. Its role in inoperable and
metastatic disease has been proved [17, 18].
CONCLUSION
GISTs are rare but clinical important
tumours. As in our case they can cause life
threatening bleeding. Intensive care to
stabilize the patient and modern diagnostic
methods like RBC scan has proved
invaluable. Complete resection of the
tumour has been the definitive treatment of
the tumour and the gastrointestinal
haemorrhage.

MULTIPLE COLONIC AND GASTRIC METASTASIS OF ENDOMETRIAL
ADENOCARCINOMA – CASE REPORT (Abstract): INTRODUCTION: In the gynecological
malignancies endometrial adenocarcinoma is the second most common, giving early
hematogenous and lymphatic metastasis to the lungs, brain and bones. CASE REPORT: We
present the case of a 71 year old patient who underwent surgery two years ago for endometrial
adenocarcinoma and followed postoperatively multimodal oncological treatment. The patient was
admitted in January 2013 in the First Surgery Clinic of the Emergency Clinical County Hospital
Târgu Mureş, Romania, for chronic anemia, consumptive disease signs and subocclusive
syndrome. The preoperative exams (upper and lower gastrointestinal endoscopy, thoraco-
abdominal computed tomography and ultrasound) found a tumor invading right, transverse and
sigmoid colon as well as gastric antrum and spleen. A subtotal colectomy with ileo-rectal
anastomosis was then performed associated with distal gastrectomy and splenectomy. The
histopathological exams revealed gastric and colonic metastasis from the endometrial serous
cancer operated two years ago. The postoperative evolution was uneventful. CONCLUSIONS:
Endometrial cancer can metastasize to the colon and stomach even without the presence of
endometriosis. Multivisceral resections and preoperative exams of the digestive tract are justified
in terms of subjective complaints of an occlusive simptomatology.

INTRODUCTION
Among gynecologic malignancies,
endometrial adenocarcinoma is the second
most common, with early lymphatic and
hematogenous metastasis [1,2].
Hematogenous dissemination is frequently
noted in lungs, brain and
bones [3]. Metastasis of endometrial
adenocarcinoma in the colon are cited in the
literature, but in most cases are associated
with endometriosis [4,5]. Isolated splenic
metastasis are reported in literature [6]. Our
observation shows the possibility,
exceptionally rare, of multiple metastasis of
endometrial adenocarcinoma to the colon
(ascending, transverse and descending
colon) and to the stomach (gastric antrum)
invading the lower pole of the spleen.
CASE REPORT
We present the case of a 71 year old
woman who underwent in 2011 a radical

lymphadeno-hystero-colpectomy (Wertheim
operation) for endometrial serous
adenocarcinoma noted as stage IIIb
(pT3bN1M0) according to the 2009 FIGO
(International Federation of Gynecology and
Obstetrics) staging [7]. The patient received
postoperatively radio- and chemotherapy.
The initially favorable evolution (no signs of
local recurrence and distant determinations)
was complicated two years postoperatively
with impaired general condition,
subocclusive syndrome (intermittent
abdominal pain, constipation alternating
with diarrhea), weight loss (6 kg in 3-4
months) and chronic anemia.
The reevaluation in the oncology
service revealed at the abdominal
ultrasound, multiple tumors situated on the
right flank and left upper quadrant of the
abdomen, adjacent to the intestinal loops and
multiple lymph nodes up to 14 mm in
diameter in the proximity of the superior
mesenteric artery. The tumor specific
antigens also markedly increased: CA-125:
40.6 u/mL, CA-19.9: 83.2 UI/mL, CEA: 12
ng/mL.

The lower gastrointestinal endoscopy
revealed in the ascending colon the presence
of an infiltrative hemi circumferential tumor
with friable mucosa and bleeding on touch,
which can’t be exceeded by the endoscope
(Fig. 1). A deep ulcer of 1.5 cm in diameter
is discovered in the loop of the sigmoid
colon, covered with fibrin as well as rectal
angiectasia probably related to radiation
proctitis.
The histopathological findings of the
biopsies have revealed the diagnosis of
serous adenocarcinoma metastasis, with a
microscopic appearance being similar to the
endometrial adenocarcinoma for which the
patient underwent surgery in the past.
The patient is admitted in First Surgery
Clinic of the Emergency Clinical County
Hospital Târgu Mureş for malaise,
abdominal pain, flatulence, weakness and
fatigue. The physical exam noted a distended
abdomen, tenderness and a hard, painful and
poorly delimited tumor in the upper
abdominal quadrant. Laboratory tests noted
severe anemia (Hb: 6.75 g/dL, Ht: 22%,
serum Iron: 2.90 µmol/dL) and
hypoproteinemia (serum proteins: 5.70
g/dL). After a brief preoperative preparation,
the patient underwent surgery under general
anesthesia with endotracheal intubation.
Operative exploration revealed
multiple tumors located in different organs:
abscessed tumor with abdominal wall
invasion on the right colon, tumor of
transverse colon with transverse mesocolon
invasion, tumor in the stomach with spleen
invasion, sigmoid tumor and multiple
peritoneal metastasis in the vicinity of
sigmoid tumor. We performed a subtotal
colectomy with ileo-rectal end-to-end
anastomosis, a 2/3 distal gastric resection
with gastro-duodenal end-to-end
anastomosis, splenectomy (Fig. 2, 3), and
citoreductive pelvic peritonectomy for the
peritoneal macrosopic metastasis. The
surgical procedures were performed using
electrothermal bipolar vessel sealing
devices.
The postoperative course was
uneventful. The patient was discharged in
the 24th postoperative day.
The histopathological exam noted
massive infiltration of tumor cells in the
colon and gastric wall with the presence of
serous adenocarcinoma (Fig. 4) and
immunohistochemical positivity to
cytokeratin 7 (CK7) (Fig. 5) and negativity
for cytokeratin 20 (CK20). Similar aspects

were found in lymph nodes and peritoneal
metastasis (Fig. 6, 7).

DISCUSSION
Incidence of uterine papillary serous
endometrial cancer is 5-10% from whole
endometrial hystological types [7]. Colon
metastasis of endometrial adenocarcinoma
are very rare, and most commonly are the
result of direct contact or peritoneal seeding
than hematogenous or lymphatic
dissemination. These are discovered usually
in the form of serous membrane implants
than of intraluminal tumors [8]. In our
observation the tumor had a macroscopic
appearance suggestive for a primary
synchronous carcinoma of the colon (three
different locations) with gastric invasion in
the antrum and corpus. Only the
colonoscopic biopsies suggested the
relationship between serous endometrial
adenocarcinoma and the present. Data from
the literature regarding the spread of
endometrial neoplasms suggests that
secondary determinations to the colon are
very rare, and often associated with
endometriosis or peritoneal dissemination,
consequence of an advanced endometrial
cancer [9,10]. The present case shows no
signs of endometriosis (clinically and
histologically), and hystopathological
protocols excluded a peritoneal
carcinomatosis in the first presentation
which shows once again its particularity.
Sohaib and Houghton, in a study
conducted on a total of 86 patients with
relapsed endometrial adenocarcinoma,
revealed no cases of metastasis to the colon,
the main target organs were the lymph nodes
(48%), vagina (42%), peritoneum (27%),
lungs (27%) and rectum (3%) [1]. Yantiss
and Clement shows 14 cases of malignant
tumors in the colon with an endometrial
starting point but with coexisting
endometriosis [9].

Lapetino proved that colorectal and
endometrial adenocarcinoma have a number
of morphological similarities, but the
immunohistochemical profiles are different:
the endometrial adenocarcinoma is CK7
positive, ER (estrogen receptor) positive,
CK20 negative, and CDX-2 negative,

whereas colorectal adeno-carcinoma is
always positive for CDX-2, mostly CK20
positive and negative for CK7 and ER, these
features being also confirmed in our
observation. [8]

Recurrence of endometrial cancer
involved in this case three colonic segments
(ascendent, transverse and sigmoid), the
distal part of the stomach and pelvic
peritoneum. The surgical procedure was
performed with electrothermal vessel sealing
devices, wich reduced operation time,
intraoperative bleeding and postoperative
lymphoragia, and allow “en bloc” resection
with minimal septicity.
CONCLUSIONS
The particularity of the presented case
regards an infrequent progression of an
endometrial adenocarcinoma correctly
treated (oncologic resection followed by
adjuvant therapy): synchronous gastric and
multiple colic metastasis developed 4 years
after the primary tumor diagnosis and
treatment. The clinical appearance initially
pleaded for primary synchronous cancers of
gastrointestinal tract. Multivisceral
resections were performed and the
histopathologic exam confirmed the
endometrial adenocarcinoma metastasis. The
pathogenic mechanism of colic and gastric
metastasis remains obscure in this case
because the patient didn’t have
endometriosis or peritoneal carcinosis.

RELAPSED OLFACTORY NEUROBLASTOMA WITH INTRACRANIAL INVASION- CASE
REPORT (Abstract): INTRODUCTION: Olfactory neuroblastoma, also call as
esthesioneuroblastoma, is a rare neuroectodermal malignant tumor arising from the olfactory
epithelium, located in superior part of the nasal cavity and characterized by frequent local
recurrences. The treatment of tumor is surgical (cranio-facial resection) follow by radiotherapy
and chemotherapy. CASE REPORT: The authors present a particular case of olfactory
neuroblastoma, operated twice by nasal approach (2008-R0 surgery, 2011-R1 surgery) and re
relapsed after one year, showing anterior cranial fossa extension. In 2012, the pacient was re
operated by paralateronasal approach (maxillary resection) and after one month, his death
occurred following a cerebral hemorrhage and cerebral perilesional edema. CONCLUSIONS: An
earlier combined treatment can improve the survival the quality of life.

INTRODUCERE
Neuroblastomul olfactiv, menţionat ca
esthesioneuroblastom, a fost descris pentru
prima dată de Berger şi Luc în 1924 [1].
Acesta este considerat a fi o malignitate
neobişnuită a cavităţii nazale. Tumora se
dezvoltă din celulele epiteliale senzoriale
olfactive, situate în mod normal în partea
superioară a cavităţii nazale, inclusiv
cornetul nazal superior, acoperişul nasului şi
lama cribriformă [2].
PREZENTARE DE CAZ
Prezentăm cazul unui pacient, în vârstă
de 52 de ani, nefumător, provenit din mediul
urban, care se prezintă în clinica noastră
pentru obstrucţie nazală cronică dreaptă,
anosmie, epistaxis ocazional drept, cefalee
frontală dreaptă şi nevralgii faciale,
simptome cu debut insidios în urmă cu
aproximativ 6 luni.
Din istoricul bolii se detaşează două
intervenţii chirurgicale prin abord intranazal
realizate în urmă cu 4 ani (2008) şi respectiv
un an (2011) pentru neuroblastom olfactiv
stadiul A Kadish, în 2008 şi stadiul B
Kadish, în 2011. De notat că, prima
intervenţie a fost considerată R0, iar ultima,
R1. Având în vedere rezecţia oncologică şi
stadiul, pacientul nu a urmat tratament
adjuvant (radioterapie). În schimb, după cea
de-a doua intervenţie chirurgicală, pacientul
a refuzat radioterapia propusă.
Examenul endoscopic nazal actual
evidenţiază o masă tumorală exofitică, alb-
cenuşie, cu zone hemoragice, localizată la
nivelul fosei nazale drepte cu obstrucţie
cvasitotală a acesteia, perforaţia septului

nazal prin care formaţiunea tumorală
pătrunde şi la nivelul tavanului fosei nazale
stângi. Consultul oftalmologic a evidenţiat
acuitate vizuală normală la ambii ochi cu
mişcări oculare fără restricţii.

Examenul computer tomografic (CT)
cu substanţă de contrast evidenţiază o masa
tisulară proliferativă iodofilă cu diametru de
4,6 x 3 cm situată la nivelul etmoidului,
masă care se extinde în sinusul frontal drept
şi la nivelul lobului frontal de aceeaşi parte
în regiunea olfactivă, sub forma unei
formaţiuni tisulare de 2,5 x 1,6 cm (Fig. 1),
stadiul C Kadish.
Examenul neurochirurgical efectuat
contraindică intervenţia pe zona cerebrală
afectată.
Se decide o intervenţie de reducţie
tumorală. Sub anestezie generală cu
intubaţie orotraheală, se practică ablaţia
formaţiunii tumorale prin abord para-latero-
nazal drept (re-zecţie parţială de maxilar
drept cu îndepărtarea peretelui lateral nazal
drept, a etmoidului anterior şi posterior până
la lamina papyracea, a peretelui anterior şi
inferior al sinusului frontal drept împreună
cu formaţiunea tumorală). Rezecţia tumorii a
fost R2, tumora de la nivelul etmoidului
neputând fi îndepărtată în totalitate datorită
prelungirii sale în lobul frontal de aceeaşi
parte. Examenul anatomo-patologic al piesei
operatorii a evidenţiat un aspect caracteristic
pentru neuroblastom olfactiv (celule mici,
rotunde, albastre) (Fig. 2) confirmat prin
imunohistochimie care evidenţiază celule
tumorale SYN pozitive, S100 pozitive, NFT
neconcludent, EMA pozitive în rozetele de
tip Flexner, GFAP pozitiv în celulele de
susţinere; indexul Ki-67 a fost de 25%.
Postoperator, evoluţia pacientului a
fost iniţial favorabilă şi s-a recomandat
efectuarea tratamentului adjuvant, pacientul
fiind programat pentru începerea
radioterapiei la o lună de zile de la
intervenţia chirurgicală.
În momentul prezentării pentru
tratamentul radioterapic, pacientul a
prezentat rinoragie bilaterală, motiv pentru
care s-a practicat tamponament nazal
anterior bilateral şi se decide internarea în
clinica O.R.L.
Examenul clinic O.R.L. evidenţiază:
cicatrice postoperatorie para-latero-nazală
dreaptă, exoftalmie dreaptă cu limitarea
mişcărilor ochiului drept. Endoscopia nazală
prezintă o formaţiune tumorală exofitică ce
obstruează total ambele fose nazale,
formaţiune care se extinde la nivelul
coanelor, precum şi sânge proaspăt, cheaguri
sangvine. În ziua următoare, starea
pacientului s-a deteriorat, prezentând

tulburări de comportament, somnolenţă şi
dezorientare temporo-spaţială. Se solicită
efectuarea de urgenţă a unui CT cerebral.

CT-ul cerebral relevă recidivă
tumorală în fosa nazală de 6,2 cm cu liza
septului nazal, a celulelor etmoidale,
peretelui intern a sinusurilor frontale,
peretelui medial a orbitei drepte cu extensie
spre lobii frontali şi spaţiul coanal medial
drept, cu efect de masă pe structurile liniei
mediane pe care le deplasează cu 1,5 cm.
Prezintă edem perilezional important, care se
extinde în lobii frontali şi parietali, cât şi
ştergerea giraţiei la nivelul scoarţei cerebrale
(Fig. 3). După efectuarea CT-ului cerebral,
pacientul este transferat în secţia de
Neurochirurgie a Institutului de Boli
Cerebrovasculare Bagdasar-Arseni. După 2
zile, pacientul decedează prin inundaţie
hemoragică cerebrală.
DISCUŢII
Neuroblastomul olfactiv este o tumoră
malignă cu evoluţie către extensie locală şi
care invadează direct baza craniului, orbita şi
ţesutul moale adiacent lamei cribriforme.
Metastazele la distanţă şi regionale sunt
prezente în 14-38 % din cazuri la momentul
diagnosticului şi cele mai frecvente sunt la
nivelul ganglionilor limfatici cervicali,
plămâni şi oase [4].
Boala apare, în general, între decada a
5-a şi a 6-a de viaţă. Cu toate acestea, unii
autori au sprijinit ideea că, în marea
majoritate a cazurilor raportate există o
distribuţie bimodală (între decada a 2-a şi a
6-a). Cazuri sporadice au fost, de asemenea,
raportate la copiii mai mici de 10 de ani.
Neuroblastomul olfactiv reprezintă circa 2-
6% din tumorile sinusurilor paranazale şi
tumorile cavităţii nazale, şi numai 0,3%
dintre tumorile maligne ale tractului digestiv
superior [2,3].
Nu există un sistem de stadializare
universal acceptat pentru neuroblastomul
olfactiv. Kadish şi colaboratorii au fost
primii care au propus un sistem
de stadializare bazat pe o serie de 17
pacienţi [5].
Cei doi factori importanţi care
influenţează prognosticul la pacienţii cu
neuroblastom olfactiv sunt extinderea bolii şi
grading-ul histologic.
Supravieţuirea este corelată cu
expresia proteinei S-100 şi un indice Ki-67
de sub 10% [7].
Recomandările de tratament variază de
la abordările endoscopice minim invazive
până la modalităţi de tratament agresiv:
abord paralateronazal, inclusiv rezecţie
cranio-facială combinată cu chimio-
radioterapie [4]. Nu există studii
randomizate care să permită realizarea
unui ghid de tratament la pacienţii cu

neuroblastom olfactiv. Combinaţia chirurgie
şi radioterapie adjuvantă este tratamentul
utilizat pe scară largă la pacienţii cu boala
local avansată.
În cazul prezentat de noi, abordul
iniţial endoscopic nazal (în 2008) a fost
posibil datorită localizării strict endonazale a
tumorii (stadiul A Kadish); în schimb
tratamentul primei recidive din 2011 a fost
inadecvat atât din punct de vedere al
tratamentului chirurgical (rezecţie R1) cât şi
a lipsei tratamentului adjuvant.
Referitor la atitudinea terapeutică
prezentată, având în vedere stadiul Kadish
C, chemoradioterapia neoadjuvantă ar fi fost
poate o alternativă chirurgiei iniţiale putând
ameliora rezultatele [8].
Pentru diagnosticul diferenţial cu alte
tumori maligne de la baza craniului, sunt
utile CT-ul şi RMN-ul. Tumorile specifice
includ papiloamele inversate, carcinomul cu
celule scuamoase, adenocarcinom,
carcinomul nediferenţiat naso-sinusal,
hemangiomul şi carcinomul metastatic.
Având în vedere prezenţa de „celule
mici, rotunde, albastre” pe lamele de
histopatologie, diagnosticul diferenţial este
destul de larg, incluzând melanomul,
rabdomiosarcomul, carcinomul nediferenţiat
naso-sinusal, limfomul, sarcomul Ewing,
adenomul hipofizar, plasmacitomul,
paragangliomul, şi tumora primitivă
neuroectodermală. [6,9]
Istoria naturală a bolii, a cărui
progresie lentă, către recidivă locală,
agresivă şi metastaze la distanţă, a
determinat recomandări diferite de
tratament. Cu toate acestea, un consens
general sprijină recomandarea actuală de
tratament: rezecţie cranio-facială urmată de
radioterapie pentru leziuni moderate, cu
completarea tratamentului cu chimioterapie
pentru pacienţii cu recidive şi metastaze.
CONCLUZII
Lucrarea prezintă un neuroblastom cu
o evoluţie extrem de agresivă care, pe fondul
unui tratament inadecvat în antecedente
(rezecţie R1 fără tratament adjuvant),
dezvoltă o recidivă ce a invadat baza
craniului, dura mater şi o parte semnificativă
a lobilor frontali pătrunzând în cavitatea
ventriculară. Această extensie tumorală a
neuroblastomului este neobişnuită. Singura
posibilitate terapeutică, în aceste cazuri local
avansate, esteo abordare multidisciplinară
(neurochirurg, chirurg O.R.L., oftalmolog şi
radioterapeut), combinând rezecţia cranio-
facială cu radioterapia.

GALLBLADDER VOLVULUS FROM THE OPEN TO LAPAROSCOPIC APPROACH
(Abstract): Gallbladder volvulus (GV) or gallbladder twist, is a rare surgical emergency. The
demographic tendency is septua- and octogenarians of the female sex and its overall incidence is
increasing in laparoscopic era. GV is frequently undiagnosed before surgical procedure. However,
clinical presentation and associated imagistic findings can lead to an accurate diagnosis, when
there is a index of suspicion for gallbladder volvulus. When the diagnosis has been established
preoperatively laparoscopic cholecystectomy can be performed safely. We discuss herein two
cases of elderly, fragile women presenting to the emergency department complaining of sudden
onset right upper quadrant abdominal pain with suspicion acute cholecystitis. The last case is
diagnosed and treated by laparoscopic approach.

INTRODUCERE
Volvulusul de vezică biliară (VVB)
sau torsiunea veziculară este o urgenţă
chirurgicală, rareori diagnosticată
preoperator. Este mai frecventă la femei, în
decada şapte-opt de viaţă, care se internează
în urgenţă cu suspiciunea de colecistită
acută. De la prima observaţie, relatată de
Wendel în 1898, au fost publicate în
literatură peste 500 de cazuri, majoritatea
operate clasic [1]. Experienţa a 2 cazuri
întâlnite în practică la o distanţă de 24 ani
ne-a determinat să revizuim literatura. Şi
astăzi este necesar să se precizeze cât mai
rapid diagnosticul preoperator şi să se
intervină de urgenţă clasic sau laparoscopic
ca în orice sindrom de torsiune de organ.
PREZENTAREA CAZURILOR
Observaţia 1 Bolnava T.S., de 92 ani,
se internează în urgenţă pe 19.02.1982 în
Clinica I Chirurgie Iaşi pentru dureri
violente, apărare musculară şi hiperestezie
cutanată în hemiabdomenul drept,
hipotensiune, paloare, greţuri, oprirea
tranzitului pentru gaze, cu debut în urmă cu
24 ore.
Examenul clinic la internare relevă o
bolnavă caşectică (IMC = 16,2 kg/m2), cu
cifoscolioză, subfebrilă, cu stare generală
influenţată, pliu cutanat persistent şi edeme
perimaleolare.
Abdomenul excavat, cu suprafaţa
micşorată, nu urmează mişcările respiratorii.
Palparea obiectivează dureri şi apărare în
fosa iliacă dreaptă şi în hipocondrul drept,
matitate la percuţie sub rebordul costal, fără
zgomote la ascultaţie.
Din examenele paraclinice, reţinem:
anemie (Ht = 27%; Hb = 8,3 g/dL),
(leucocitoza 10000 / mm3); hematurie şi
leucociturie, hipoproteinemie. Radioscopia

abdominală simplă arată aerocolie, fără
nivele hidroaerice.
După două ore de tratament antibiotic,
antispastic şi antalgic, tabloul clinic nu se
modifică şi, cu diagnosticul prezumtiv de
colecistită acută, se intervine, prin
laparotomie transrectală dreaptă cu anestezie
generală prin IOT.
La deschiderea cavităţii abdominale se
constată lichid gălbui în cantitate medie,
ficat mic, cu lob stâng proeminent; marele
epiploon acoperă o formaţiune subhepatică
ovoidală de 10 / 6 cm, de culoarea frunzei
moarte, cu zone necrotice punctiforme,
legată de ficat printr-un mezou volvulat în
sens orar cu 270º.
Se devolvulează vezicula biliară, care
este infarctizată, sub tensiune, cu cistic
tirbuşonat; coledocul care era ridicat de
vezicula torsionată este normal. Se practică
colecistectomie anterogradă, toaletă
peritoneală şi drenaj subhepatic.
Vezicula extirpată conţine bilă de
stază, iar peretele său este infarctizat.
Evoluţia postoperatorie este simplă,
bolnava externându-se după 12 zile,
vindecată.
Examenul microscopic arată colecistită
ulcero-necrotică, cu edemaţierea şi necroza
peretelui.
Controlată după 6, 12, 24 de luni,
bolnava este fără acuze.

Observaţia 2. Bolnava G.E., de 86 ani,
hipostenică, cu insuficienţă cardiacă,
cataractă senilă, ateroscleroză, guşă
nodulară, cifoscolioză se internează în
urgenţă pe 28.04.2006, cu dureri în
hipocondrul drept, greţuri, vărsături,
anorexie, tahicardie. Boala debutează acut în
urmă cu 6 ore. Examenul fizic evidenţiază
apărare în hemiabdomenul drept, semn
Murphy pozitiv.
Examenele de laborator sunt
neconcludente; nu are leucocitoză.
Ecografia abdominală (Fig. 1) arată o
veziculă biliară volvulată median, ocupată
de un calcul de 50 mm şi proximal de 2
polipi colesterolotici de 8 mm. Peretele
ambelor compartimente este gros,
multistratificat, hipoecogen, fără semnal
Doppler.
Se suspectează un volvulus de veziculă
biliară pe litiază.
Laparoscopia exploratorie evidenţiază
ficat cu lob stâng ptozat, colecist volvulat la
180º în sens orar, cu perete cianotic (Fig. 2).
Se devolvulează vezicula biliară care făcea
un cort ce ridica pediculul biliar infiltrat
edematos; se indenfică canalul cistic, de
aspect sinuos, care se clipează şi
secţionează. Se clipează vasele cistice
juxtavezicular şi se practică colecistectomie
retrogradă laparoscopică.
Vezicula, cu diametrul longitudinal de
20 cm, are perete edematos cu zone de
necroză şi conţine un calcul mixt de 6 cm.
Evoluţia postoperatorie este simplă. Se
externează a 3-a zi postoperator. Controlată
după 1, 2 şi 6 ani bolnava nu prezintă
simptomatologie digestivă.
DISCUŢII
Volvulusul veziculei biliare este o
urgenţă chirurgicală rară, fiind semnalată o
dată la peste 350.000 internări [2].
Raportările publicate în literatură în perioada
laparoscopică şi în era prelaparoscopică
rareori depăşesc două cazuri [3-10].
Boonstra EA şi colab. [11] a arătat că
volvulusul vezicular apare numai în cazul
existenţei unui mezocolecist evident (Fig. 3),
cu lipsa de fixare a veziculei biliare la ficat,
în caz de visceroptoză frecvent întâlnită la
persoanele în vârstă. Ateroscleroza arterei
cistice şi un canal cistic sinuos pot servi ca

puncte de sprijin pentru torsiunea veziculei
biliare. [11]
Nakao analizând 245 de cazuri în
literatura japoneză, arată ca VVB tinde să
apară mai ales la femeile în vârstă [12],
deşi au fost publicate şi cazuri pediatrice
(mezoul veziculei biliare poate exista
congenital) [13].

Raportul bărbaţi / femei este de 3 / 1,
iar incidenţa cea mai mare este consemnată
în decadele şapte şi opt de viaţă [14].
Cercetând factori de risc ce predispun la
apariţia VVB putem menţiona: sex feminin,
vârsta peste 70 ani, scădere în greutate, ptoza
ficatului, cifoscolioza, ateroscleroza,
mezocolecist alungit, pierderea grăsimii
viscerale [1,15].
Volvulusul veziculei biliare se poate
face în sens orar sau antiorar, cu 180-360º
sau chiar mai mult, ceea ce duce la ischemie
cu necroză şi eventual perforaţie. Gradul de
torsiune poate fi folosit pentru a diferenţia
torsiunea completă (> 180º) de cea parţială
(< 180º), ultimele fiind confundate cu o
colică biliară. Torsiunea în sens orar a fost
atribuită peristaltismul gastric, în timp ce
torsiunea în sens antiorar a fost legată de
peristaltica colonului [1]. Histologic se
constată un infarct, mai mult sau mai puţin
complet al peretelui, cu flebotromboză în
submucoasă.
Colelitiaza nu favorizează torsiunea
mesoaxială, doar un sfert dintre bolnavi
prezentând şi litiază [12].
Starea generală se alterează rapid si
apare o veritabilă stare de şoc generată de
durere şi fenomenele de iritaţie peritoneală.
Greţurile, vărsăturile se instalează precoce.
Simptomatologia este dominată de
durerea intensă cu sediul în hipocondrul
drept şi epigastru, instalată brusc la un
bolnav fără antecedente biliare, la care se
adaugă precoce fenomene de iritaţie
peritoneală. Lau descrie o triadă clinică în
volvulusul de veziculă biliară (Tabel I) [16].
Cele două cazuri prezentate au avut toate
cele 9 semne; la acestea putem adăuga
ineficienţa tratamentului medical, care
impune intervenţia chirurgicală în urgenţă.

Ecografia evidenţiază o veziculă cu
perete îngroşat şi lichid pericolecistic
asemănător colecistitei acute. Examenul
poate suspecta torsiunea, atunci când
vezicula biliară se află în fosa iliacă dreaptă,
inferior de ficat, prezentând o structură
hipoecogenică, fără semnal Doppler parietal
[17,18].

Computer tomografia şi imagistica
prin rezonanţă magnetică pot aduce date
utile pentru diagnostic [19-22].
Diagnosticul este rareori precizat
preoperator, în literatură fiind menţionate
doar 4 cazuri [19]. Afecţiunea este de obicei
confundată cu colecistită acută [1,15,19,20],
care poate fi temporizată (dar în volvulus
intervenţia chirurgicală se impune în
urgenţă), apendicita acută [23], cu ulcerul
perforat, pancreatita acută sau torsiunea unui
chist de ovar.
Indicaţia operatorie este formală, ea
fiind susţinută de existenţa apărării sau
contracturii şi lipsei de răspuns la
tratamentul cu antibiotice şi antispastice.
Colecistectomia poate fi efectuată prin
abord clasic sau laparoscopic [13]. Prima
colecistectomie laparoscopică pentru VVB a
fost efectuată de Schroder şi Cusumano, în
1994 [25]. Abordul laparoscopic permite,
înainte de toate, precizarea diagnosticul [26].
Colecistectomia este facilă după
devolvulare, dar impune o atenţie deosebită,
deoarece mobilitatea neobişnuită a mezoului
poate antrena formarea unui „cort
coledocian”, a cărui ignorare poate duce la
lezarea CBP. Poate fi necesară
decompresiunea prin detorsiune pentru a
clarifica anatomia pedicului biliar; disecţia
meticuloasă este esenţială, deoarece
coledocul poate fi la marginea anterioară a
ficatului atunci când este răsucit,
predispunând la leziuni iatrogene.
Colangiografia peroperatorie este utilă în
cazurile cu anatomie „incertă”. Uneori se
impune conversia.
Prognosticul este favorabil în cazul în
care colecistectomia este efectuată precoce.
Dacă tratamentul este întârziat, în condiţiile
bolnavului vârstnic, infarctul şi perforaţia
duc, prin peritonită biliară, la creşterea ratei
mortalităţii.
Mortalitatea postoperatorie, în general,
mică (5%) fiind dependentă de comorbidităţi
şi de precocitatea intervenţiei [1,26,27].
CONCLUZII
VVB este o urgenţă chirurgicală rar
întâlnită; tabloul clinic obişnuit este
reprezentat de apariţia bruscă a durerilor în
hipocondrul drept asociate cu apărare şi
tulburări dispeptice la o femeie vârstnică, cu
visceroptoză şi status hipoponderal.
Cu toate progresele diagnosticului
imagistic, diagnosticul preoperator este rar
precizat. În absenţa răspunsului la
tratamentul medical, se impune explorarea
laparoscopică care va preciza diagnosticul şi
realiza colecistectomia după devolvulare.
Recunoaşterea intraoperatorie a anatomiei
arborelui biliar este esenţială pentru a evita
leziunile de cale biliară principală.

TECHNICAL IMPROVEMENTS IN DOMINO LIVER TRANSPLANTATION (Abstract): A
new modality of liver transplantation was introduced in 1997: the sequential or domino live
transplantation. In this kind of transplantation, the FAP (familial amyloidotic polyneuropathy)
patients’ livers are used as grafts especially for recipients older than 55—60 years. The obvious
inconvenience is that the FAP liver can continue to provide transthyretin, with the probability of
later development of neuropathy in the new recipient.

BACKGROUND
In patients who underwent
transplantation for familial amyloidotic
polyneuropathy (FAP) type I, the explanted
liver is normal [1,2], except for the
production of transthyretin. The original
technique used for transplantation in FAP
patients, has been changed to promote
sequential transplantation [3].
During the hepatectomy of FAP
patients, the inferior vena cava (IVC) is not
preserved, and in many cases the veno -
venous bypass is employed [2,4]. Some
surgeons even report that the pericardium
may be sectioned in order to lengthen the
vena cava stumps [2-4].
The big problem and challenge in
domino liver transplantation remains the
IVC length for both patients, the FAP patient
and the domino recipient.
In this case, a new technical
improvement may be used which allows the
FAP hepatectomy to be performed with IVC
preservation, in which it is not necessary to
use veno - venous bypass nor to open the
pericardium sac.
TECHNICAL NOTE
We report the case of a 41 years old
woman who accepted to be the domino
donor. A cadaveric liver graft became
available and was harvested using the
standard technique. The IVC bellow the
renal vein, with both common iliac veins
were taken to be used as vascular grafts. The
FAP patient hepatectomy was performed
with IVC preservation, and veno - venous
bypass was not necessary. The cadaveric
graft was implanted with latero-lateral veno-
caval anastomosis. The pedicle element

anastomosis was performed by the standard
technique. The FAP patient did not required
blood transfusion. The postoperative period
was completely uneventful, and the patient
was allowed to leave the hospital on the 8th
postoperative day.
The FAP liver as a domino graft was
harvested without vena cava, only with right,
middle and left hepatic veins stumps and
was perfused on the back table with Belzer
solution. The middle and left hepatic veins
were joined together, and the venous graft of
both iliac veins with the common cuff of the
IVC was anastomosed with right hepatic
vein and the new common trunk of the
middle and left hepatic veins, using 5-0
Pprolene® running suture.
A 63 years old man with end stage
liver disease secondary to C hepatitis, agreed
to accept the FAP liver. The recipient’s liver
explantation was either performed with
preservation of the IVC, and the liver was
implanted in the standard piggy back
procedure, using the caval stump of the
venous graft as the outflow from FAP liver.
The postoperative course was also
uneventful, and the patient was discharged
on the 14th postoperative day. The Doppler
hepatic vein waveform always showed
triphasic waves, consistent with a good
outflow from the FAP liver.
DISCUSSION AND CONCLUSION
This new technique for domino liver
transplantation was first described by Pena
[1]. The vascular outflow anastomosis in the
domino recipient is performed with an
iliac / caval vein graft from the cadaveric
donor. For this reason, some technical
difficulties such as short vena cava stump,
pericardial effusion, and bad outflow in both
patients are avoided. This technique also
avoids the required veno - venous bypass or
the hemodynamic changes after caval
clamping in FAP patients.
In summary, this new technical
improvement may be considered as a
possibility to avoid the complications
described in the domino procedure,
especially in the FAP patients, and are in
accordance with domino transplant
philosophy.

EXTRACAPSULAR TOTAL THYROIDECTOMY IN THE BENIGN THYROID DISEASE
(Abstract): The extracapsular total thyroidectomy is the surgical technique for the monobloc
resection of the thyroid parenchyma, between the Charpy fascia and the thyroid gland’s own
capsule with the preservation of the recurrent nerve, parathyroids, and the exterior branch of the
superior laryngeal nerve. Even though this technique represents the treatment of choice in the case
of malignant thyroid lesions (in association with the lateral or central compartment
lymphadenectomy or not), its indication has been extended in the presence of a benign cytology
(FNAB – Bethesda II, III) associated with lesions disseminated in the entire thyroid parenchyma.
The intracapsular total thyroidectomy, the subtotal thyroidectomy and the Dunhill operation are
surgical techniques also indicated in the case of benign multinodular thyroid lesions disseminated
in the entire parenchyma. The aim of this video documentary is to illustrate the extracapsular total
thyroidectomy in the benign thyroid disease. Different procedures key points are highlighted:
operating room set up, the regional bilateral superficial cervical plexus anaesthesia with the aim of
reducing post-operatory pain, the left lateral surgical approach of the left thyroid lobe, and the
superior polar approach of the right thyroid lobe respectively, the extracapsular monobloc
resection of the thyroid parenchyma spearing the recurrent nerve and the parathyroids, the
dissection of the James-Berry ligament, drainage and closure of the musculofascial plans and the
wound. Even the indication of extracapsular total thyroidectomy in the case of benign thyroid
lesions is still controversial, this technique reduces the risk of recurrent and parathyroid
complications and, the risk to develop tumors on the remnant thyroid parenchyma.

Atât Facultatea de Medicină cât și
Epitropia Casei Spitalelor „Sf. Spiridon” din
Iași s-au preocupat de modernizarea
învățământului clinic chirurgical [1-3].
Deposedată de terenuri agricole și
păduri, posibilitățile financiare ale Epitropiei
erau reduse după primul război mondial.
Astfel că, după repetate intervenții la
Ministerul de Instrucțiune Publică, s-a ajuns
la încheierea unui contract între acest
minister, reprezentat de Prof. Nicolae
Costăchescu și Epitropia Casei Spitalelor
„Sf. Spiridon”, în baza căruia se punea la
dispoziția Spiridoniei suma de 35.000.000
lei, în scopul construirii unui palat al
clinicilor chirurgicale (aprobarea regelui -
I.D.R. 1202/1927). [1,2]
Acordarea subvenției era condiționată
de respectarea următoarelor clauze [1,2]:
- suma acordată va fi folosită exclusiv
pentru construirea Palatului Clinicilor
Chirurgicale;
- executarea lucrărilor se va face prin
licitație;
- construcția se va realiza pe terenul
Epitropiei, în spațiul cuprins între
strada Muzelor și strada Universității,
pe aproximativ 5000 m2, după
planurile și propunerile Epitropiei și va
servi pentru totdeauna Clinicilor
Chirurgicale ale Universității;
- dacă Epitropia va da clădirii altă
destinație, Ministerul de Instrucțiune
Publică va avea dreptul să revendice
suma;
- planul de construcție va fi supus
aprobării Ministerului de Instrucțiune
Publică (Dosar 1197/1929).
Epitropia acceptă clauzele menționate
mai sus și se întocmesc planurile și devizele de
construcție, care sunt executate de arhitectul
I. Pompilian din București, pentru suma de 4%
din valoarea totală a construcției. Odată
planurile și devizele aprobate de către
Minister, se trece la organizarea licitației de
construcție, la care participă 11 firme; se alege
oferta cea mai avantajoasă, cea înaintată de
inginerul Pietro Moccia, care oferă un rabat de
31,50% din valoarea construcției. Consiliul de
Miniștri, prin Jurnalul său din 27 august 1929,
aprobă încheierea contractului cu această firmă
și pune la dispoziția Epitropiei, pentru anul
1929, suma de 5.000.000 lei. În executarea
contractului se convine ca minim 70% dintre
lucrători să fie români. [1,2]
Lucrările de construcție încep în
septembrie 1929. Au existat unele probleme
în executarea fundațiilor din cauza calității

terenului, acesta fiind teren de umplutură și
teren vegetal de tip cuaternar nămolos, de
slabă consistență. Perimetrul ales pentru
construcție avea o înclinare pronunțată și
infiltrații de apă. Din această cauză,
fundațiile au necesitat o coborâre mai mare
decât cea prevăzută inițial. În toamna anului
1929 s-a lucrat într-un ritm alert, astfel că la
începutul lunii noiembrie se turnau deja
fundațiile. S-a convenit, de asemenea, ca
materialele întrebuințate să fie de cea mai
bună calitate, Epitropia rezervându-și dreptul
de a controla în permanență calitatea
materialelor și a lucrărilor, și cu prioritate
calitatea betonului armat. [1,2]

Deși au intervenit dificultăți financiare
provocate de criza economică mondială,
lucrările nu au fost întrerupte, astfel că într-un
interval de 2 ani (1 septembrie 1929 – 18
septembrie 1931) au fost acordate avansuri în
17 tranșe, care s-au ridicat la suma de
34.441.673 lei. Suma inițială de 35.000.000 lei
va fi cu mult depășită, în acești ani intervenind
o puternică inflație. Au intervenit și mari
întârzieri în plata facturilor din cauza lipsei
banilor primiți de la guvern. La un moment dat
exista chiar pericolul ca lucrările să fie sistate,
antrepriza înregistrând pierderi și amenințând
cu renunțarea la rabatul de 31,5% stabilit
inițial prin licitație. Epitropia „Sf. Spiridon”,
prin Prof. Ion Tănăsescu (Fig. 1), intervine
energic la Guvern, care deblochează fonduri și
lucrările de construcție pot fi continuate.

În ianuarie 1932, în urma unor anchete
întreprinse din partea Ministerului de
Instrucție, a Ministerului Sănătății și a
Ministerului de Interne, se constată unele
neconcordanțe în realizarea proiectului de
construcție. Ca de obicei, presa intră în alertă
și unul dintre ziarele locale titrează:
„Alarmantă stare de lucruri descoperită
recent la temelia noilor construcții de la
Sf. Spiridon” (vezi ziarul Lumea din 29
ianuarie 1932). În materialul său, reporterul
vorbea despre schimbarea totală a planului
de construcție și de abateri grave din partea
antreprenorului, care ar fi realizat o
construcție „cu totul diferită de cum e
prevăzută în proiect”. Afirmația era absolut
hazardantă. Din motive obiective,
constructorul a fost obligat, pentru siguranța
clădirii, să modifice sistemul de fundație.
Față de planul inițial, care prevedea o
fundație din beton simplu și cu pereți din
zidărie de cărămidă cu învelitoare metalică
și planșeuri din beton armat, compoziția
terenului a impus antreprenorului P. Moccia
să schimbe sistemul de fundație executând
un radier general de beton armat, iar la
solicitarea beneficiarului modifică acoperișul
transformându-l în terasă. Este adevărat că
devizul inițial de 35.000.000 lei a fost cu
mult depășit, dar aceasta se datora însă
inflației galopante din anii 1930-1932,
precum și unor îmbunătățiri de soluții
constructive aduse pe parcursul lucrărilor de
execuție. [1,2]
Deși lucrările de realizare a Palatului
s-au desfășurat cu multe întreruperi din
cauza întârzierilor provenite din rambursarea
sumelor cuvenite constructorului, se aprecia,
în septembrie 1932, că una din clinici, cea a
profesorului Ion Tănăsescu, „era aproape
terminată” (Lumea nr.4292/21 septembrie
1932). Aceasta se datora faptului că
renumitul chirurg supraveghea îndeaproape
lucrările și, pe de altă parte, făcea repetate

demersuri la Primăria municipiului pentru a
se efectua racordul cu rețeaua de apă și
canalizare a orașului. [1,2,4]
La numai 9 luni de la articolul extrem
de critic publicat în ianuarie 1932, același
ziar titra: „Cel mai modern și mai frumos
spital din Europa ridicat la Iași” (Lumea nr.
4293/23 septembrie 1932). De data aceasta,
criticile se transformă în laude și nu se mai
vorbește nimic despre tragerea la răspundere
a antreprenorului și a epitropilor vinovați de
deturnarea finanțelor publice. Deoarece
documentele arhivistice nu fac o descriere a
acestei construcții, preluăm din acest ziar
prezentarea făcută ieșenilor, în septembrie
1932: „Clădire monumentală numai în
beton armat, înaltă de 4 etaje (Fig.2).
Clădirea ocupă un spațiu mare, fiind
împărțită în 4 corpuri mai mici. Intrarea
principală foarte impozantă va fi prin curte.
O scară monumentală dă într-un hol mai
mic. De ambele părți laterale câte o odaie
pentru portari. Se instalează câte o cabină
de telefon la fiecare clinică. [1,2]

Urmează câte un vestiar somptuos,
după care dăm într-un alt hol mare, care va
fi amenajat ca sală de cursuri pentru
studenți. Frumoasă, ornată pe dinăuntru și
afară cu stucaturi în ghips, ce contrastează
cu simplitatea restului clădirii. Acest hol are
un balcon de beton armat suspendat. Felul
cum plutește în aer a uimit și pe specialiști,
care s-au îndoit de trăinicia lui. În dreapta
holului sunt scări monumentale în beton
armat cu balustradă de fier forjat. Aceste
scări în număr de 4 (patru) urcă până la
acoperiș, două dintre ele sunt exclusive
pentru studenți, iar alte două la dispoziția
personalului medical. În afară de scări mai
sunt și câteva ascensorii, unul foarte mare în
care vor urca tărgile sau cărucioarele cu
bolnavi, un alt ascensor pentru personal și 4
ascensoare mai mici pentru rufe.
Ascensoarele și scările urcă până la
acoperiș. Aici o inovație: în loc de acoperiș
s-au făcut terase pentru cură de soare, cu
balustradă de beton armat pentru a opri
curenții. Terasele ocupă o suprafață de 2500
m2. Balustradele vor purta 300 de becuri,
care vor lumina feeric terasele. Acestea vor
fi puse la dispoziția tuturor bolnavilor și a
copiilor, iar cei contagioși vor fi puși la o
parte”. Această terasă a fost construită la
insistențele profesorului Ion Tănăsescu, care
se adresa primului epitrop, profesorul Vasile
Rășcanu: „Când se cheltuiesc zeci de
milioane de lei pentru o clinică, poate să se
mai cheltuiască încă ceva pentru o terasă,
care va fi o binefacere pentru bolnavi”. [1-4]
Realizată cu un an înainte de intrarea
în funcțiune a Clinicii Chirurgicale a
Profesorului Tănăsescu, descrierea Palatului
este parțială și va fi completată în septembrie
1933, când ni se prezintă interiorul clinicilor.
La etajele clădirii se aflau câte trei rezerve
de clasa I, cu baie și closet separate și mai
multe saloane de bolnavi cu săli de baie,
toate în mozaic și teracotă. Saloanele erau
dotate cu ventilatoare, toate camerele erau
mari și luminoase. [1,2,4]
Clinicile dispuneau de instalație de
calorifer. „La fiecare etaj sunt camere
pentru laborator, odăi de pregătire a
bolnavilor, unde sunt injectați și adormiți”.

Cu mult entuziasm sunt descrise sălile
de operație: „Minunea Palatului o formează
sălile de operație (Fig. 3). Numai în Elveția
există ceva identic. Într-o sală mare cu
terasă în formă de amfiteatru și cu balcon
iarăși în terasă s-a amenajat o sală specială
în cristal, hermetic închisă. În această sală
nu intră decât profesorul. Orice zgomot din
afară nu pătrunde în interior. Bolnavul vine
adormit cu căruciorul din sala de pregătire.
Aici el este operat în văzul studenților care
stau pe băncile din amfiteatru din jurul sălii
de cristal. Sunt 6 astfel de săli, dar nu s-a
amenajat decât una singură”. [1,2]
Parterul Palatului era destinat corpului
medical, fiind dotat cu săli de consultație,
cabinet de radiologie, baie, cancelarie, două
laboratoare, bibliotecă și muzeu. Din lipsa
fondurilor, terminarea lucrărilor a fost
amânată, dar antreprenorul a luat decizia de
a executa în devans scara principală de la
intrare.[1,2]

O inaugurare neoficială a acestui
așezământ spitalicesc s-a făcut la data de 2
septembrie 1933 și la aceasta au participat
epitropii Spiridoniei, profesori de la
Facultatea de Medicină, medici de la Spitalul
Central. Nu lipsește slujba de sfințire a
localului efectuată de părintele Salomei. În
octombrie se preconiza o inaugurare oficială
la care să participe și Regele Carol al II-lea,
dar acesta ar fi declarat, după primirea
invitației, că nu poate lua parte la
„inaugurarea unor clădiri neterminate”.
Și cu această ocazie presa locală se
exprimă în cuvinte elogioase la adresa
edificiului: „e desigur cel mai frumos din
țară și rivalizează cu clinicile similare din
străinătate”; despre sălile de operație se
afirmă că sunt în număr de patru,
„amenajate după ultimele cerințe ale
medicinei și sunt specializate pe cazuri”.
La construcția acestora s-au avut în
vedere sistemele europene în materie, astfel
încât „una din săli este amenajată după
sistemul francez, iar alta după modelul
german”, ambele fiind capitonate cu pereți
de sticlă, în spatele cărora studenții vor
putea urmări operațiile fără a stingheri pe
clinicieni” (Fig. 3). [1,2]
Reporterul era entuziasmat și de
capacitatea sterilizatorului „care în câteva
ore va steriliza mii de pansamente”. [1,2]
În toamna anului 1933, Profesorul Ion
Tănăsescu avea, în calitate de colaboratori,
pe doctorii Barbillian, Scutaru și Sonea, toți
foști studenți ai săi. În total, clinica dată în
funcțiune avea 6 saloane mari și 17 rezerve,

care dețineau 80 de paturi. Ulterior, numărul
de paturi a crescut la 130. [4,5]
Instalația de baie era apreciată ca un
unicat „care nu se mai găsește în Iași”. [1]
Printre participanții la ceremonia de
preinaugurare se afla și profesorul Nicolae
Costăchescu, de la Facultatea de Științe a
Universității (profilul chimie), care era la
acea dată Președinte al Senatului României.

Cei care pătrund astăzi în Palatul
Clinicilor Chirurgicale pot citi în holul
principal următoarea inscripție fixată pe
peretele de la intrarea în amfiteatru: „Acest
Palat al Clinicilor Chirurgicale, a cărui
construcție a început în 1929 și s-a terminat
în luna noiembrie 1932, din inițiativa și prin
nețărmuritul sprijin al d-lui Nicolae
Costăchescu, Ministerul Instrucțiunii
Publice, 1929-1931, și prin stăruința
neobosită a d-lui profesor dr. Ion Tănăsescu
și profesor dr. Vasile Râșcanu, epitropi ai
Casei Sf. Spiridon, 1928-1931.
S-a inaugurat în luna noiembrie 1933,
punându-se această placă pentru a fi
generațiilor prezente și viitoare ale Iașiului
drept pildă de muncă și devotament în
serviciul binelui obștesc”, Arhitect, prof.
Pompilian, Antrepenor, ing. P. Moccia
(Fig. 4). [2]

La inaugurarea din noiembrie 1933, în
clădire a fost instalat numai Institutul
Chirurgical condus de profesorul Ion
Tănăsescu (Fig. 5), cu o capacitate de 130 de
paturi, care funcționa „în cele mai
remarcabile și moderne condițiuni”. [1-5]
Clinica I Chirurgicală a rămas
neterminată. După unele amenajări sumare,
în această aripă a fost instalat Serviciul
General de Spital. În anii 1941-1942 se
efectuează lucrări importante finalizându-se
și amenajarea părții destinate Clinicii I
Chirurgicale. [1]
Această incursiune în trecut reprezintă
o lecție pentru generațiile următoare pentru
că vor învăța să-și prețuiască înaintașii și
dacă ei pot vedea mai departe este rezultatul
faptului că au urcat și s-au sprijinit pe umerii
lor.
Mai bine spus, trecutul este cel care ne
clădește prezentul, totodată viitorul nu poate
fi coerent și legitim fără să cunoști trecutul
sau așa cum spune un citat celebru, mai
actual astăzi ca niciodată, când, pasivitatea
nu mai naște nici măcar reactivitate: ”Cine
nu-și cunoaște trecutul, nu poate înțelege
prezentul și nu va putea să vadă în viitor”.

Nous présentons la malade H. P. âgée
de trente-six ans, qui a été opérée à l’Institut
de Chirurgie « Le Roi Charles II » pour un
néoplasme du col utérin.
Le jour de l’hospitalisation de la
malade, on constate la présence d’un
épithélioma en forme de choux-fleur, avec
les paramètres un peu infiltrés, le droit
surtout, où la lésion semble s’étendre vers
l’insertion du vagin. L’utérus est
parfaitement mobile, la paroi recto-vaginale
n’est pas infiltrée. Nous apprécions que ce
cas se prête assez facilement à une opération
du type Wertheim.
Nous intervenons et, après l’inventaire
des lésions, nous trouvons le cas encore plus
facile que nous ne l’avions pas prévu.
Ce n’est qu’au moment de la
dissection du paramètre droit que nous nous
sommes rendus compte que nous nous étions
trompés: à cet endroit se trouvaient une série
de ganglions, les uns situés sur la paroi
même du bassin, les autres le long des
vaisseaux iliaques externes, dont un
ganglion aussi grand qu’une noix et
impossible à isoler de la paroi veineux.
Connaissant la publication de Brocq
qui, dans un cas similaire, a réséqué les deux
veines iliaques externes, nous avons eu
recours à la même technique. Nous avons
réséqué la veine iliaque externe avec le
ganglion adhèrent - entre deux ligatures - du
côté droit seulement car, du côté gauche, le
paramètre a été relativement facile à
disséquer. Nous avons complètement évidé
le tissue cellulo-ganglionnaire, et nous avons
terminé l’hystérectomie par un drainage
vaginal avec péritonisation correcte au-
dessus.
La plaie abdominale a guéri per
primam, et la brèche vaginale était presque
complètement fermée le 14-ème jour.
Cependant, du point de vue
circulatoire, l’évolution a été arrêtée par
l’apparition d’un œdème dur, volumineux,
survenu le 4-5-ème jour après l’opération.
Cet œdème s’accentuait dès que la malade
quittait son lit, et surtout si elle restait plus
longtemps debout. La différence de volume
entre les deux membres était visible, et la
malade ressentait certaines douleurs et de la
gêne en marchant.
Revue après deux mois, la malade se
trouvait dans l’état suivant:
- examen génital: les culs-de-sac
indolores, souples, les paramètres
libres;
- le membre inférieur droit: augmenté de
volume, présente un œdème mou,
indolore, plus accentué à la jambe et
dans le dos du pied; il présente les
diamètres suivants:
gauche droit
le coup de pied 22 cm 25 cm
la jambe ½ moyenne 32 cm 39 cm
la cuisse 1/3 inf. 40 cm 43 cm
la cuisse 1/3 sup. 45 cm 51 cm
La circulation collatérale est évidente
dans la ½ inférieure de l’hémi-abdomen
droit.
En revoyant la malade après trois
mois, on constate que l’œdème cède de plus
en plus, la différence de diamètre entre les
deux membres étant à peine perceptible. La
malade fait facilement 4 km à pied, elle peut
rester debout, vaquer à ses occupations et
elle n’accuse plus aucune gêne.

A l’examen local, on ne constate pas
trace de récidive. Cependant, en ce qui
concerne la possibilité d’une récidive, nous
ne pouvons en tirer aucune conclusion,
l’opération étant trop récente.
Nous n’avons pas présenté ce cas pour
ré ouvrir la discussion au-sujet du traitement
du cancer du col ou de la technique de
l’hystérectomie abdominale type Wertheim.
De même, nous n’avons pas l’intention de
préconiser la résection de la veine iliaque
externe comme une technique élargie,
applicable dans tous les cas.
Nous croyons cependant que, dans
certains cas où la dissection ganglionnaire
n’est pas possible sans léser la veine, ou sans
mettre en péril la paroi veineuse, nous
pouvons recourir à la résection de la veine,
sans craindre des troubles circulatoires trop
graves (même ceux qui existent n’étant pas
définitifs).
Dans le cas publié, Brocq montre que
les troubles ont été minimes, et que la
malade n’a présenté qu’un œdème
malléolaire passager.
Notre cas a été moins heureux:
l’œdème a été accentué, dur et il a duré plus
longtemps.
Nous tenons à attirer votre attention
sur le fait que l’examen anatomo-
pathologique a montré qu’il s’agissait d’une
forme d’épithélioma cylindrique. Dans le
ganglion extirpé, on a trouvé des métastases
néoplasiques, qui ne pouvaient pas être
mises en doute.
On sait à quel point la radio-sensibilité
de cette forme d’épithéliome est faible, de
sorte que la malade a bénéficié d’autant plus
de l’exérèse élargie de la lésion. On connaît
aussi le tropisme ganglionnaire accentué,
que présentent ces épithéliomes; c’est
probablement la raison pour laquelle
l’adénopathie était si volumineuse.
Dans la littérature que nous avons pu
consulter, nous n’avons rencontré la
résection des veines iliaques externes dans
l’hystérectomie Wertheim que dans le cas
publié par Brocq en 1935.
Si les ligatures veineuses sont
effectuées assez fréquemment dans les cas
où il existe des troubles circulatoires
antérieurs (artérites, thrombo-phlébites),
elles sont, par contre, moins connues- chez
l’homme, du moins- lorsque la circulation
est normale.

Pentru o lungă perioadă de timp
includerea unor structuri vasculare
importante într-un bloc tumoral a fost
considerată drept o contraindicaţie formală
pentru exereza tumorii canceroase. Această
„dogmă” a fost însă infirmată în ultimele
decenii, odată cu dezvoltarea tehnicilor de
chirurgie vasculară reconstructivă.
Articolul pe care îl comentez după 75
de ani este de o actualitate surprinzătoare.
Soluția aleasă de cei doi tineri chirurgi, care
s-au dovedit ulterior chirurgi de mare clasă -
atât profesorul Chipail la Iaşi, cât şi doctorul
Silviu Carp la spitalul de Urgență din
București, este și astăzi conformă cu
principiul extirpării radicale a tumorii
primare uterine și a limfonodulilor de drenaj
pelvin. Inspirați din experiența relatată de P.
Brocq, autorii nu au pregetat să extirpe un
segment din vena iliacă externă împreună cu
un limfonodul iliac extern, care făcea corp
comun cu peretele venei iliace externe
drepte. Deci interesul radicalității oncologice
instituit de W. Halsted a prevalat în fața
posibilelor sechele funcționale ale circulației
venoase profunde a membrului inferior
drept. Pe vremea aceea tehnicile de chirurgie
vasculară erau în etapa de pionierat. Iată însă
că chiar și în zilele noastre, în literatura
consultată părerile autorilor oscilează în
legătură cu necesitatea restabilirii
continuității venei iliace externe. Astfel,
rezecția unui segment de venă iliacă externă
fără restabilirea fluxului venos se practică
astăzi în cazul recoltării de transplante
venoase autologe folosite în reconstrucțiile
vasculare după rezecțiile majore hepato-
bilio-pancreatice pentru cancer [1,2]. Alți
autori preferă refacerea continuității venei
iliace externe folosind proteză PTFA [3],
reconstrucția cu ajutorul transpoziției venei
iliace interne ipsilaterale [4] sau folosirea
venei femurale superficiale [5].
Un studiu clinico-funcțional privind
circulația venoasă în membrul inferior drept
la un lot de 66 de pacienți cărora li se
recoltase un segment de venă iliacă externă
dreaptă pentru restabilirea unor continuități
vasculare după rezecții tumorale majore în
etajul supramezocolic [1] a arătat prezența la
o mică parte din pacienți a durerilor în
membrul inferior stâng, însoțite de prurit
după ortostatismul prelungit.

Neamț, la care au participat peste 200 de
chirurgi din toată țara și din Republica
Moldova.
Nivelul științific ridicat a fost atins
prin tematica bine aleasă (iatrogenie, trauma,
chirurgie de urgență, chirurgie oncologică,
chirurgie minim invazivă, varia) și, mai ales,
prin participarea unor chirurgi de prestigiu
(M. Beuran, N.M. Constantinescu,
C. Copotoiu, V. Sârbu, M.R. Diaconescu,
N. Dănilă, Șt. Georgescu, N. Iordache,
D. Sabău, O. Unc, Cr. Lupașcu,
G. Rojnoveanu, R. Scerbina).
Deschiderea oficială a reuniunii a
cuprins, pe lângă mesajele de bun venit din
partea autorităților locale și două conferințe
Actualitatea afirmației făcute de Rabelais în
urma cu 500 de ani: „Știința fără conștiință,
duce la ruina sufletului” susținută de
Nicolae M. Constantinescu și „35” istoria și
cronica unor întâlniri de V. Răzeșu.
În secțiunea de trauma au fost
susținute 4 conferințe privind Hemo-
peritoneul experimental (E. Aneste),
Management of liver trauma in a level I
trauma hospital (M. Beuran), Tendințe
actuale în chirurgia de urgență și traumă
(E. Nicolau), Pregătirea în chirurgia de
traumă (E. Târcoveanu) și 5 lucrări
referitoare la plăgi abdominale, rolul
chirurgiei plastice în rezolvarea
traumatismelor de membru inferior, poli-
traumatisme cu predominanță neuro-
chirurgicală, nutriția la pacienții chirurgicali.
În secțiunea Iatrogenie coordonată de
Prof Șt. Georgescu s-au prezentat 2
conferințe magistrale și 10 lucrări, care au
dezbătut acest capitol actual, delicat și foarte
important.
Momentul emoționant al consfătuirii a
fost sărbătorirea a 100 ani de la nașterea
marelui profesor I. Juvara, formator de școală
chirurgicală care a coordonat chirurgia
românească în ultimele decenii. Au fost
lansate cu această ocazie volumele domnului
dr. N. Botezatu Ion Juvara, Omul, Chirurgul,
Dascălul, personalitate emblematică a
medicinii românești și 35 de V. Răzeșu, care
au fost distribuite gratuit participanților. N.
Botezatu a realizat cu acest prilej o medalie
remarcabilă CENTENAR Prof Dr. Ion Juvara
– chirurgia românească recunoscătoare. În
prezența fiicei Profesorului, distinsa Doamnă
Alexandra Chiliman Juvara au depănat
frumoase amintiri V. Răzeșu, N. Botezatu, M.
Beuran, N.M. Constantinescu,
Tr. Nicola, C. Copotoiu, E. Nicolau,
D. Sabău, V. Sârbu, N. Iordache, M. Ghiur,
E. Târcoveanu.
O altă sesiune interesată a vizat
chirurgia minim invazivă și a cuprins 3
conferințe (Locul chirurgiei în tratamentul
obezității în 2013 – N. Iordache, Abordul
laparoscopic al leziunilor hepatice –
Șt. Georgescu, Avantajele abordului
laparoscopic in afecțiunile chirurgicale ale
splinei – E. Târcoveanu) și 12 lucrări privind
abordul laparoscopic al chistului hidatic
hepatic (D. Sabău), apendicita acută
(V. Palade), herniile și eventrațiile
(O. Unc), suprarenalectomia laparoscopică
(E. Târcoveanu) și note de tehnică
(C. Bradea).

Secțiunea VARIA cu trei sesiuni a
cuprins 20 de lucrări pe diverse teme
chirurgicale importante în practică
(pancreatită acută, eventrații, anastomoze
colonice, diverticul gastric, cancer gastric,
afecțiuni chirurgicale ale tiroidei, hemoroizi,
tumori stromale, cancer de sân, tehnici de
chirurgie oncologică, cancere de rect), a
oferit participanților posibilitatea de a face
schimburi de idei în domeniile cele mai
diverse ale patologiei chirurgicale.
De un real interes s-a bucurat sesiunea
de prezentare de cazuri clinice de către
rezidenți, (11 lucrări), sesiune care a
demonstrat vigoarea tinerilor care vor
continua tradiția acestor reuniuni.
Sesiunea de postere a cuprins 14
lucrări, din care cinci au fost premiate.
Impresionantă a fost participarea
colectivului chirurgical tânăr de la
Tg. Mureș, condus de Prof. Dr. C. Copotoiu,
care a susținut un regal de 16 lucrări. Merită
subliniată si contribuția științifică a
delegației de la Chișinău, condusă de Prof.
Rojnoveanu și Conf. R. Scerbina.
Discuțiile academice după fiecare
lucrare, participarea tuturor centrelor
universitare, (de la Constanța la Timișoara,
de la București la Chișinău) organizarea
impecabilă, ambianța plăcută a
evenimentelor sociale au garantat reușita
acestei manifestări devenită tradițională pe
scena evenimentelor științifice din ţara
noastră.
La plecare, toți participanții au promis
că vor reveni cu plăcere la cea de a XXXVI-
a Reuniune a Chirurgilor din Moldova.
