    In order to be nominated, the player must be able to travel to Columbus, Ohio; leaving Friday, June 14 and returningMonday, June 17.

    The IHSSBCA will pay for the tournament entry fee and the coaches costs, and will make travel arrangements. Willthe player, his/her family, or his/her school (or some combination thereof) be able to pay for the students travel costshotel costs, and food? Yes No

    On an additional sheet, please provide the following information. Keep total responses to no more than two single-sided sheets of letter-size paper or the equivalent (one double-sided sheet, etc.).

    Give the players strongest subjects in the context of Scholastic Bowl/quizbowl. List the AP/IB courses, courses taken at colleges, or other college-level coursework the player will have

    completed by the end of the current academic year. Give objective measures of the players Scholastic Bowl/quizbowl performance. This should include individua

    statistics from as many tournaments as possible, team performance at tournaments, conference results, andother relevant data directly obtained from Scholastic Bowl and/or quizbowl competitions.

    List any other significant awards, honors, or recognitions the player has received. Discuss the players ability to function as part of a team. (This can be written by the player himself/herself,

    his/her coach, his/her teammates, or anyone else with direct, relevant knowledge.) Provide a statement from the player discussing why he/she is qualified to be on Team Illinois. Optionally, the players coach (or any other coach) may provide a statement of endorsement.

    The Team Illinois selection committee will select four players to comprise the team it feels is best suited, as a whole, todo well at the National All-Star Academic Tournament. As such, highly qualified players may not make the team.

    Do not attach rsums, transcripts, or other additional documentation; any such material will be ignored.
