Is there a foreign odour? 1.2 Neck Distended neck veins Use of accessory muscles Trachea deviation Subcutaneous emphysema Check for track marks Check for medicalert necklace Administer Oxygen 1.3 Chest Auscultate the chest Is there equal bilateral chest rise and air entry (6 sections of chest) Is there wheezing / or other irregular breathing sounds Muffled Sounds Check for scars Check for pacemaker Check for bruising 1.4 Abdomen Palpate the abdomen Check for rigidness (4 quadrants) Check for guarding Check for oedema / swelling Distension Tenderness Check for scars Check for bruising 1.5 Pelvis Check for track marks Is the pt. incontinent? 1.6 Lower extremities and feet Legs Check for track markers on legs (if legs exposed) Feet Check for track marks Check for pedal oedema Check pedal pulse Check capillary refill Babinski 1.7 Upper extremities and hands Arms Check for track marks Wrist and hand Check radial pulse Check for medicalert bracelet Check capillary refill 1.8 Turn pt. lateral (towards yourself) Check for sacral oedema

1 MEDICAL PT. ‐ HEAD TO TOE 

1.1 Head 

Temperature Skin – signs of cyanosis/ pale, cold and clammy/ signs of hypoxia Face – is the face symmetrical  Eyes – pearl (use pupil torch) Mouth – are there any foreign bodies? Is there a foreign odour? 

1.2 Neck 

Distended neck veins Use of accessory muscles Trachea deviation Subcutaneous emphysema Check for track marks Check for medic‐alert necklace Administer Oxygen 

1.3 Chest

  Check capillary refill 

  Babinski  

1.7 Upper extremities and hands 

Arms  Check for track marks

Wrist and hand  Check radial pulse 

  Check for medic‐alert bracelet

  Check capillary refill 

1.8 Turn pt. lateral (towards yourself) 

Check for sacral oedema
