2013 ANNUAL REPORT

The year 2013 was a remarkable one at ReDiscover. In order to respond to the rapidly increasing needs of the community, we hired 86 additional staff and expanded our footprint in the community. We purchased a complex in Lee’s Summit to house our headquarters and certain services. A partnership was formed with the Linwood Area Ministry Place (LAMP) to renovate the historic Linwood Presbyterian Church which will provide a new home and bigger presence in midtown Kansas City for our Women’s Substance Abuse program.

We also saw a marked increase in the number of people in our services. Up from 600 to nearly 800 served each day.

With growth comes the challenge to stay connected to the people and community we serve. We have to be smarter in our execution, stretching the precious dollars we have while maintaining the quality of our services and our strong commitment to improving the lives of our clients.

As you review this annual report, you will see evidence of programs developed and expanded that are part of our long-term strategic plan. We made a commitment to the people we serve to improve access to physical health care, advocate for safe, affordable housing and reduce barriers to access our services.

Housing is a great concern for our clients. We know that a stable living environment is essential to maintain good mental health. Many clients are homeless, at risk for homelessness or are residing in unnecessary dependent living situations. Our new Housing Program provides several tiers of

residential settings with differing levels of support depending on the client’s needs. With a safe, affordable housing situation, people can begin to rebuild their lives.

Sixty-eight percent of Missourians with mental illness have one or more chronic physical conditions. For many, their physical health needs have gone unmet until now. With the expansion of our already successful Disease Management and Health Care Home programs we have been able to deliver positive health outcomes to 750 persons at ReDiscover by coordinating their health care. These programs have provided a cost savings to Medicaid of 45.4 million statewide but more importantly, have saved lives.

Last year we became acutely aware of the number of people that called our crisis line but were unable to get into treatment. In order to provide more timely care and greater access to services we launched Open Access. This service allows for walk-ins, and triages each caller to determine immediate needs. Additional psychiatrists have been hired to shorten the wait from intake to first appointment. This will continue to be an area that will have a heightened focus in 2014.

Our employees continue to be our biggest advantage. Their energy, passion and commitment to the people we serve sets us apart and produces extraordinary results. We will continue to re-invest in our staff through various trainings, such as Trauma Informed Care so that we work to our fullest ability.

As we move ahead into 2014, I wish to thank our staff for their role in helping us achieve our goals while maintaining a commitment to our core values. DISEASE MANAGEMENT Improving the quality of health care

The mortality of those with severe and persistent mental illness is 25 years earlier than the average American. More than 80 percent of those premature deaths are due to treatable medical conditions caused by preventable risk factors such as smoking, obesity, substance abuse and inadequate access to medical care.

The Disease Management project is working to change that statistic by identifying high cost Medicaid clients, who have a serious mental illness coupled with a chronic medical condition and connecting them with mental health services.

This year ReDiscover‘s Disease Management Team has connected with 88 persons, improving their health outcomes and saving the State approximately one million dollars in Medicaid funds.

SAFE HOUSING More than a place to sleep

Safe, affordable, and appropriate housing helps maintain good mental health. Stable housing opportunities facilitate independence, ease transition out of hospitalization and reduces costs.

Realizing that inadequate housing resources exist for the persons we serve, ReDiscover identified housing as one of our top three advocacy issues in 2013. A Housing Coordinator was hired and we partnered with SAVE, Inc. to help manage certain housing aspects. By the close of the year, over 106 persons were receiving housing support.

Housing options vary depending on the needs of the client and can range from respite to independent apartment living.

ReDiscover believes that by living where they choose, clients are more likely to have a higher quality of life, maintain independence and rebuild confidence and social networks.

HEALTH CARE HOME Treating the whole person

Health Care Home continues to deliver quality care coordination for low-income persons with chronic health conditions, serious mental illnesses and substance use disorders.

ReDiscover was able to improve the health of the persons we serve and save State Medicaid $1.4 million by reducing inpatient and emergency room costs. Currently nearly 700 persons are enrolled in the program at ReDiscover.

FOUNDATION FOR GROWTH

ReDiscover purchased Savannah Square, a complex of three buildings in Lee’s Summit that will provide 40,544 feet of additional office space. Administration, Adult and Family Services and Access will move into the new offices in 2014 with more planned as space is available.


CARF ACCREDITATION Center of excellence

ReDiscover received a three year CARF (Commission on Accreditation of Rehabilitation Facilities) accreditation in July. CARF accreditation signals quality with a focus on the satisfaction of persons served and is an assurance to consumers that our programs and facilities are of the highest quality. Payers also recognize CARF accreditation as a demonstration of superior performance for their clients.

Gina was emotionally and physically abused by her father from the time she was three years old. Her childhood was a blur, spent moving from place to place, attending nine different schools in as many years. At the age of eighteen Gina found the courage to leave home and cut off all contact with her father. She attended college and became a kindergarten teacher.

Gina had buried her past but being around children brought back terrible memories of abuse. It became more than she could bear. Gina took a handful of pills and ended up in a psychiatric hospital.

She was diagnosed with PTSD, major depression and schizoaffective disorder. Gina began therapy and was referred to ReDiscover.

“I would be institutionalized or dead if I hadn’t received help. I made serious attempts at killing myself,“ says Gina.

Immersing herself in therapy, Gina has come out on the other side. She works at ReDiscover mentoring others and is grateful for the opportunity to make a difference. Gina credits her Team Leader for having faith in her and convincing her that she could provide hope for those struggling with mental illness.

Gina is back doing what she started years ago… teaching. But now she is whole.
