Annual Report Summary 2012 2012 Major Accomplishments Forest Grove: Received FEMA SAFER grant for Volunteer Coordinator position. This grant funds the position for four years, $426,000. Forest Grove: Received FEMA SAFER grant for Fire Prevention Captain’s position. This grant funds the position for four years, $263,000. Cornelius: Received FEMA AFG grant for Cornelius Type III engine. Award for $375,000, acquired and placed unit in service. With savings we achieved by purchasing off of an existing contract, we received approval for a grant amendment to build an additional Type VI engine (currently being built). Forest Grove: Completed construction on two Type VI wildland engines and placed in service. Joint: Renewed contract with City of Cornelius for Fire Chief services, updating contract to include automatic renewal. Forest Grove: Completed remodel of dorms at Station 4. Forest Grove: Completed remodel of Station 7, creating living/sleeping accommodations. Forest Grove: Completed purchase of FEMA funded fire training simulator and place in service. Joint: Completed project of acquiring AEDs for City Hall, Community Auditorium and Library. Also provided AEDs for Cornelius Police Department. Forest Grove: Completed hiring process for new firefighter and promotional process for new Lieutenant. Joint: Implemented new web-based records management system. Joint: Completed training and certification process for both the City of Forest Grove and the City of Cornelius to achieve national NIMS certification. Joint: Completed update of Emergency Operations Plan - plan adopted by both Cities’. Forest Grove: Completed contract with Pacific University for shared physician services. Joint: Established joint intern firefighter program with Cornelius Fire Department. Joint: Completed grant funded equipment acquisition of portable weather station, portable radios, and gas monitors. Forest Grove: Fire Marshal David Nemeyer received the "2012 Golden Sparky Award" from the Oregon State Fire Marshal. Joint: Implemented use of iPads in both department resulting in increased operational capability, flexibility, and decreased costs. Cornelius: Negotiated three-year union contract for Cornelius. Joint: Established communications patch with Yamhill County for all of Washington County fire departments. Forest Grove: Worked with Forest Grove Firefighters Association to bring back Fourth of July fireworks to Forest Grove. 2012 2012 Major Accomplishments

• Forest Grove: Received FEMA SAFER grant for Volunteer Coordinator position. This grant funds the position for four years, $426,000.

Fire Chief’s Report

Reporting Period: January 1-31, 2012

Notable Calls and Events

1. Forest Grove: Firefighter Bill Sauber passed away this month, memorial service schedule for February 3. Permission received from Governors office to fly flags at half-mast on this day in his honor.

2. Cornelius: Cornelius – House fire on Fremont Street. Heavy smoke damage to the house but the fire was quickly controlled and the belongings of the residents saved. This house was the former residence of Navy Hospital Corpsman Ryley Gallinger-Long, and current residence of his mother. Estimated damage was $50,000, cause is still under investigation.

3. Forest Grove – Flood water rescue. Boat 4 responded to a single car that had attempted to drive across Fern Hill road, which was flooded and closed. While launching the boat, a second car attempted the same thing with three more people. A total of four people were rescued from their disabled cars.

4. Forest Grove – Fatal car accident on Gales Creek Road. It is believed that the driver suffered a cardiac event prior to the rollover accident. This event required heavy extrication.

5. Joint: Overnight snow storm on January 18 led to multiple responses for downed trees and electrical lines, and resulted in several roads closed. Forest Grove/Cornelius EOC was set up and a pre-alert was sent out, but the expected following high winds did not occur and the EOC was not activated.

6. Cornelius: 2nd Alarm residential structure fire at 0200 on January 6, 2012 on Golf Course Road. Attic was heavily involved on arrival of first units, a quick attack limited the damage to the second floor and attic space. Units responded from Cornelius, Forest Grove and Hillsboro. Cause of the fire appears to be either faulty wiring or power overload of the electrical panel.

Training

1. Forest Grove: Chief O’Connor received an update on the Kidde Fire Training simulator build. Construction is on schedule.

2. Joint: After action review conducted for the Cornelius Fremont Street fire above. 3. Joint: Fire Chief taught first delivery of Advanced Fire Dynamics for Company

Officers at the above conference, a two day class on the advanced physics of compartment fire behavior

4. Forest Grove: Began orientation of two new volunteer firefighters. 5. Joint: Fire Chief completed first draft for training presentation on time and

project management.

6. Forest Grove: Division Chief O’Connor conducted hands-on training on foam operations for pumper operators at TVFR south facility.

7. Forest Grove: Division Chief O’ Connor coordinated February 25 class for Technical Report Writing for Fire Officer I.

Fire Prevention/Community Risk Reduction 1. Forest Grove: Fire Prevention staff conducted two tours for Pacific University

pre-school, with 40 children and 10 adults receiving fire prevention education. 2. Cornelius: Fire Prevention Technician Matt Johnston completed an inspection of

the horse barn on Flax Plant Road. Minor infractions noted. 3. Cornelius: Captain Steve Black and FPT Matt Johnston worked on identifying

high risk occupancies for updating and improving Cornelius pre-plan system. 4. Forest Grove: Inspected new business on Pacific Avenue – Little Stars Day

Care. 5. Forest Grove: Fire Chief submitted proposal to Forest Grove Firefighter

Association for community assistance program. 6. Cornelius: Fire Prevention Technician Matt Johnston completed investigation of

house fire on Golf Course Road. 7. Forest Grove: Chief Nemeyer attended Washington County Juvenile Fire Setter

Network Meeting at TVFR Aloha, Washington County Safe Kids Coalition Meeting in Hillsboro.

8. Forest Grove: Fire prevention has completed inspections of 19 of 34 restaurants, bars and other food establishments.

9. Joint: Met with Fire Prevention/Code Enforcement staff to brainstorm strategic goal objectives of improving/enhancing these programs.

10. Forest Grove: Division Chief Nemeyer completed inspections at Tom McCall West, Tom McCall East and after school program at Joseph Gale Elementary for state license requirements. Also working on annual report required for Oregon State Fire Marshal

Projects and Administration 1. Cornelius – Successful and fun Cornelius Crab Feed held Saturday, February 21,

with over 700 people attending. Firefighters and families from Forest Grove assisted their Cornelius brothers and sisters with this event.

2. Forest Grove: Station 4 remodel dorm room remodel is ongoing – walls are framed, sprinklers, HVAC and electrical is complete, drywall is installed.

3. Cornelius: Fire Chief participated in City Manager interviews for the City of Cornelius.

4. Forest Grove: Fire Chief attended annual Boards and Commissions dinner on Thursday, January 19.

5. Cornelius: Fire Chief updated Cornelius FEMA NIMSCAST report for 2011. 6. Forest Grove: Type VI engines are in Medford (January 30) for installation of

boxes. Project is ahead of schedule. 7. Joint: Fire Chief attended Oregon Fire Service Symposium and completed final

term as OFIA President. 8. Joint: Committee met to discuss annual banquet planning for Cornelius and

Forest Grove Fire Departments – this is our first joint banquet. Tentatively scheduled for April 27, more info to follow.

9. Joint: Chief O’Connor updated training request forms to all electronic system. 10. Cornelius: Meeting with Cornelius Rural Fire Board held. Discussion on

ownership percentages of fire apparatus. Researched and delivered document to board members that explains this.

11. Forest Grove: Public Safety Advisory Committee met and made recommendation to City Council regarding levy amount. Recommendation was for a minimum of $1.85 per $1000 and pursue an additional meter fee to enhance funding of public safety.

12. Joint: Washington County of Office of Consolidated Emergency Management sent out a request of those agencies interested in upcoming State Homeland Security Grant program. Fire Chief responded that both Cornelius and Forest Grove Fire are interested in participating again.

13. Joint: Fire Chief continued research into records management system. 14. New joint fire department organizational chart posted on the department internal

web site. 15. Joint: Received notification of grant award to develop a FEMA required

Continuity of Operations Plan (COOP) from Portland UASI for both Cornelius and Forest Grove. Grant will fund vendor that provides facilitation and software to develop this plan, estimated at $56,000.

16. Cornelius: Captain Reckmann completed revenue budget projections for budget process.

17. Cornelius: FF Jeff Lewis has completed 50% of annual Knox Box key inventory. 18. Cornelius: Engine 813 pump repair was completed by our outstanding

maintenance team of Wes Proffitt, Eric Hall and Rob Marcoff. Cost was under $13,000.

19. Joint: Fire Chief participated in disaster drill planning for April drills. 20. Joint: Fire Chief attended Washington County EMS Operations meeting – long

discussion on ambulance-only calls and fire department transport program. The latter was ended as a pilot program and is now approved.

21. Joint: Fire Chief met with WSC Insurance representative and received quotes for both Firefighter Associations.

22. Joint: Fire Chief attended Washington County Public Safety Executive Officers meeting where a long discussion on WCCCA funding and potential changes to the system.

23. Forest Grove: Fire Chief met with the CEO and CFO of WCCCA to discuss concerns regarding funding allocations and Forest Grove’s higher cost allocation.

24. Joint: Fire Department Annual Report draft completed and distributed for input. 25. Joint: OFIA Annual Report draft completed and distributed for input. 26. Joint: Washington County Fire Chiefs met on Thursday morning to discuss

possible joint and cooperative ventures for all Washington County fire departments. A presentation will be given at the Washington County Fire Districts annual meeting on February 7.

27. Joint: Fire Chief attended EMS Scientific Review Committee on Tuesday in Portland – this group (which we are a part of)is applying for a Federal grant to provide a possible pilot program for alternative call screening, transport options and advanced care for EMS calls. While still in the early stages, there is a short timeline for application.

28. Forest Grove: City Manager met with all three career shifts to discuss levy poll and lawsuit.

29. Forest Grove: Attended City stakeholders meeting for presentation on levy poll. 30. Forest Grove: Dwight Lanter has been cleared by his physician to return to duty. 31. Forest Grove: Fire Chief scheduled meeting with WCCCA director to discuss fees

for dispatch services and the “black box” method of determining those fees. Forest Grove Fire is facing a more than 12% increase.

32. Forest Grove: Division Chief O’Connor completed quarterly grant narratives for all Homeland Security Grants.

33. Forest Grove: Division Chief O’Connor received approval for grant amendment to use remaining grant funds for OPS 39 patch for Yamhill County communications; also requested grant extension for AFG mobile fire simulator.

Reporting Period: February 1-29, 2012

Notable Calls and Events

1. Cornelius - Responded to roll over MVA on NW Wren Road at NW Susbauer Road. 2 car t-bone style crash with significant impact enough that it rolled one car, trapping the driver. Station 8 and 4 crews stabilized the vehicle and extricated the driver of the rolled cross over type SUV out the rear hatch door. He was the only one injured and was transported from the scene using ground ambulance.

2. Cornelius - Responded to working house fire on N. 18th Avenue in Cornelius. Initially this came in as a task force for a dryer fire but was upgraded to full first alarm assignment. Station 8, 4, and HFD-6, responded to this incident. Of note at this incident was the use of solar panels on the home’s roof and the dynamics that were created as our crews had to find where to shut off the power from these units so they could safely overhaul the fire. Total damage estimated at $36,000. During the first 30 minutes of this call Forest Grove and Cornelius received 6 unrelated calls.

3. Forest Grove – Responded with Boat 4 to Newberg to assist with water rescue. Driver was extricated and transported by Life Flight. Boat 4 assisted with removal of vehicle from the pond.

4. Joint: Personnel from Forest Grove and Cornelius attended memorial service for retired firefighter Bill Sauber, who died on January 29. Flags flown at half mast in both City’s with Governor Kitzhaber’s permission.

5. Forest Grove: Investigators responded to a youth-set fire in bathroom at Neil Armstrong Elementary.

Training

1. Forest Grove: Forest Grove – Chief O’Connor evaluated a possible acquired structure for live fire training on Ritchey Road.

2. Joint – three Forest Grove interns, 5 Forest Grove volunteers and 4 Cornelius volunteer recruits began their fire academy this last month.

3. Joint: Chief Ian O’Connor coordinated a lieutenant/officer/AIC training process for Cornelius and Forest Grove volunteer and career staff. 11 people participated in the process, which consisted of tactical simulations, in-box scenarios, employee counseling scenarios, and presentations.

4. Joint: Dr. Daya provided monthly physician case reviews. 5. Cornelius: Lt. Robards placed Blitz fire nozzle/monitor in service. 6. Joint: Division Chief O'Connor received $1500 grant for officer development

classes from OFIA.

7. Joint: Division Chief Nemeyer taught NFPA Firefighter II academy classes for Fire Prevention/Pub Ed., Pre-Incident Surveys, and Fire Origin and Cause Saturday, Feb 18.

Fire Prevention/Community Risk Reduction

1. Forest Grove: Chief Dave Nemeyer participated in the Plan Review Board meeting to discuss the Primetime Sports Bar plans, performed two driveway inspections, and completed a list of pre-fire plan priorities.

2. Forest Grove: Division Chief Dave Nemeyer is working with Pacific University public safety to establish occupant load limits in rooms/buildings on campus, and worked with FG School District on a number of issues related to FG high school site and code requirements.

3. Forest Grove: Chief Dave Nemeyer completed annual report to State Fire Marshal Office. State Fire Marshals Office approved our application to continue our exempt status.

4. Cornelius: FPT Matt Johnston Attended a meeting with city officials, construction and building officials from the Virginia Garcia project. It was determined that the alarm system and sprinkler system be fully functioning as well as a clear means of egress for the occupants. Also advised them to make up an emergency operations plan for both clinic and construction crews in time of emergency, and offered assistance in creating this.

5. Cornelius: Matt Johnston completed design of Cornelius business fire inspection program; prioritized and broke up inspections into groups by priority and frequency of inspection. Divided up into 4 groups (annual, bi-annual, tri-annual and non-inspectable) and those were divided into years (bi-and tri-annual occurrences).

6. Joint: Chief Dave Nemeyer completed design of an “After the Fire” brochure. 7. Cornelius: Fire Prevention Technician Matt Johnston completed inspection of

horse barn facilities of Flax Plan Road – six violations were found. 8. Forest Grove: Fire prevention met with Eric Cannon of the FG United Church of

Christ for a building tour and review of emergency/exit lighting needs. Identified a number of deficient areas where changes would be encouraged.

9. Forest Grove: FPT Matt Johnston worked to find a company that cleans kitchen hood systems that also speaks Spanish so that the La Estrella restaurant could improve the safety of their kitchen.

Projects and Administration

1. Joint: Completed annual review and update of Emergency Operations Plan. Updated plan to serve as joint plan for City of Forest Grove and City of Cornelius.

2. Cornelius: Completed Cornelius Fire Department budget with Captain Reckmann.

3. Cornelius: Received notification that Cornelius Fire Department has received award for a Type III engine through American Fire Grant, grant award is $375,000 with a match of $18,750.

4. Cornelius: Fire Chief presented resolution to adopt Emergency Operations Plan to Cornelius City Council. Council approved resolution.

5. Forest Grove: Firefighter Bob Haney announced his retirement after 41 years with Forest Grove Fire and Rescue.

6. Joint: Implemented 2 tablet computers for Cornelius duty officer and volunteer coordinator (grant funded) and one for Forest Grove duty officer. These tablets replace the air cards in the MDT’s (a significant cost savings) and are used to enhance communications, training, response information, and provide field reference of critical information. Phase 2 of this project will involve the possibility of electronic forms.

7. Joint: Met with records management consultant to review our current Firehouse utilization and get suggestions for improvement.

8. Joint: Updated joint fire department web site and fixed problem with officer development program chart and organizational chart (https://sites.google.com/site/westernwashingtoncountyfire/home). Also enhanced apparatus maintenance form and drill registration.

9. Forest Grove: Successfully completed 2 SAFER grant applications. Both of these grants are 100% funded and have no matching requirements.

10. Joint: Fire Chief attended EMS Scientific Review committee meeting in Portland, where the grant that was delivered to the Federal government last week by the OHSU scientist was discussed and explained. This was a $30 million dollar grant that could drastically alter the way emergency medical services is provided in Washington County.

11. Joint: Fire Chief attended disaster exercise planning meeting at TVFR. Tabletop planned for April 5, 0830-1230. Full scale exercise is planned for April 24, and the primary purpose will be to test our damage assessment procedures.

12. Joint: Fire Chief contacted TVFR for assistance in preparing for ISO. 13. Joint: Fire Chief attended joint Fire Board meeting of Washington County Fire

Chiefs. Presentation was provided by Chief Dustin Morrow on possible consolidation, fire authority models that could work within Washington County.

14. Joint: Conducted Forest Grove/Cornelius senior officer meeting on February 2. 15. Cornelius: Fire Chief completed Walmart grant application for AEDs for

Cornelius Police Department.

16. Forest Grove: Two Type VI engines returned from Medford with tanks and pumps installed. Final phase is installation of cabinets. Forest Grove: Type VI engine cabinet installation on schedule and should be completed by end of month.

17. Forest Grove - Station 7 remodel is underway. As of this report, the new walls had been dry walled. Contractor estimates that it will take 3 weeks to complete this job, putting us done with the project on or around March 16th.

18. Joint - Fire Chief attended statewide meeting at DPSST on modern building construction, future changes, code issues, and developing a training program for firefighters on how to approach these.

19. Forest Grove - Robert Webb continues to make an impact on our pre-fire plan program and this week he continued to tour facilities and draw plans. Right now he is working his way through the plans that were started but not completed previously.

20. Forest Grove: Station 4 remodel is complete. Dorm room furniture is being purchased.

21. Forest Grove: Captain Dwight Lanter returned to full duty. 22. Joint: Completed quarterly review and update of SOG and protocols, and

distributed. 23. Forest Grove: Began research on regional RFP for upcoming tender replacement. 24. Cornelius: Stair climb participants held a fundraiser at Human Bean, raising

$800. 25. Cornelius – received formal notification form TVFR that they are donating a used

Notable Calls and Events

1. Forest Grove - Severe motor vehicle accident on Highway 6 on March 23, Volkswagen Bus versus semi-truck. Patient was extricated and transported but died at the hospital.

2. Forest Grove – Large gas leak in the 900 block of Goff Road after workers accidentally cut a 2” line with a backhoe. Crews monitored neighboring houses for LEL of natural gas. Area was blocked off until Northwest Natural could repair the leak. No evacuations required.

3. Forest Grove – On March 17, C Shift attended the annual fund raiser dinner at the Forest Grove Senior Center.

4. Forest Grove - Structure fire in manufactured home on Wilson River Highway, believed to have started in attic.

5. Cornelius – Motor home fire next to house on South Nectarine. Motor home was 3’ from a house but fire was confined to the motor home, a 1958 converted 30’ Greyhound bus. The motor home was a complete loss.

6. Forest Grove: Responded to weather related crash resulting from snow/ice build up on the Wilson River Highway summit. This crash involved 20 to 30 vehicles, including 3 semi trucks and a tour type bus. No injuries.

7. Cornelius: Lieutenant Sheila Jackson announced she will retire from the Cornelius Fire Department on May 1, 2012 after 20 years of service.

8. Cornelius: Kurt Albee received Cornelius City Council recognition for his 35 years of dedicated service to the Cornelius Fire Department. The event was attended by over 40 firefighters from both Cornelius and Forest Grove.

Training

1. Joint: Division Chief O’Connor met with TVFR Training Captains to discuss joint compartment fire behavior training.

2. Forest Grove: Division Chief O’Connor assisted with teaching components of NFPA Instructor III class.

3. Joint: Dr. Daya provided monthly medical training to both departments on March 19.

4. Forest Grove: FF Ted Penge completed NFPA Instructor II class, Lt. Chris Woodford completed all classes for NFPA Fire Officer II.

5. Joint: Fire Chief taught ICS 100 to Spring recruit firefighter academy, Division Chief Nemeyer taught Hazmat Awareness and Operations to same group.

6. Forest Grove: Division Chief O’Connor successfully completed re-accreditation of the department’s fire training program with DPSST.

7. Cornelius: FF Kevin Ritcheson was certified to operate Engine 813 and FF Andrea Pumarejo has been certified to operate Engine 814 and 815.

8. Forest Grove: FF Will Murphy received training on use of Washington County’s new mobile EMS training simulator and is in the process for reserving it for future training dates.

9. Forest Grove: Received updated on mobile fire training unit – exterior work nearly complete, will begin soon on interior components. Expecting delivery in the 2nd or 3rd week of May.

10. Forest Grove: Captain Joe Smith attended Type II Incident Management training at the TEEX facility in College Station, Texas.

Fire Prevention/Community Risk Reduction

1. Forest Grove: Division Chief Nemeyer provided a fire safety presentation to over 100 seniors at the Homestead Community Center.

2. Forest Grove: Fire inspections conducted of Forest Grove Community Charter school, Hilda Higgbe Home, Half-Moon Sports Bar, Diamond Place, FG Sushi, Mandarin Chinese Restaurant. Re-inspections of Grendels, Connies Corner Café, Sweet Pea Treasurers, Izgara Restaurant.

3. Joint: Division Chief Nemeyer attended school district safety committee meeting to discuss juvenile fire intervention programs – in the last most we have had 3 separate fires set in our schools.

4. Forest Grove: B Shift conducted a “red card party” at Jennings McCall – “red cards” are special cards that senior citizens keep on their refrigerators with medication and medical history listed for fire department paramedics to refer to in an emergency.

5. Forest Grove: Fire Marshal Nemeyer participated in the reading event at the kindergarten and first grade classes at Dilley Elementary School as part of a celebration for Dr. Seuss’ birthday.

6. Joint: Fire Prevention Officers from both departments attended 3 day seminar on fire codes related to hazardous materials and flammable/combustible liquids in Hillsboro.

Projects and Administration

1. Forest Grove: March Public Safety Advisory Committee meeting held. Updates provided on levy.

2. Forest Grove: Submitted fire department proposed budget to finance. 3. Forest Grove: Gales Creek remodel is complete. Forest Grove remodel has some

pending electrical issues to be resolved but should be complete soon. Furniture has been ordered.

4. Cornelius: Lt. Duncan scheduled NFPA medical exams for firefighters that are due.

5. Joint: Continued research into records management systems for both departments.

6. Forest Grove: Responded to FEMA questions regarding details of SAFER application.

7. Cornelius: Lt. Duncan researched volunteer firefighter eligibility for EAP programs.

8. Cornelius: Received donated Type VI engine from TVFR, began process for placing in service.

9. Joint: Fire Chief attended Fire Defense Board – discussions on patch to YCOMM (they supported assisting with the funding for this), and progress on the TVID hydrant program.

10. Cornelius: Captain Marc Reckmann received notification from FEMA that the department was eligible for reimbursement of expenses that occurred for the intern program after December 2010 – applied for $15,100 in funds.

11. Joint: Fire Chief attended Washington County EMS Operations meeting, major discussion on response criteria to medical/rehab/care facilities with recommendations made.

12. Forest Grove: Proposal given to City Department Heads for achieving NIMS level certification for employees.

13. Joint: Division Chief Nemeyer completed a draft policy for social media use. 14. Joint: Fire Chief had teleconference with Fire Chief’s from Western Fire Chiefs

Association and Oregon Fire Chiefs Association to discuss preparation for ISO rating – recommendation that we stall ISO to do our rating until they have adopted their new standard and corrected some of the turmoil currently going on with ISO.

15. Joint: Withdrew the grant request for continuity of operations planning for this year until we can get both Cities NIMS qualified.

16. Joint: Division Chief O’Connor continued administrative work on YCOM/WCCCA patch.

Notable Calls and Events 1. Forest Grove - Apartment fire on Oak Street at 0400 on April 29, started by a

stove burner left on. One male victim was found unconscious in the bedroom and rescued, treated on scene and revived. Transported to Tuality Hillsboro and later discharged after full recovery. There were no working smoke alarms in the apartment. Estimated $40,000 in damage.

2. Forest Grove – Life Flight transport of 3 year-old child hit in head with rock on April 13. Life Flight was landed at Tom McCall.

3. Forest Grove – Crews treated a victim of a self-inflicted gunshot wound (shot gun) to the chest on April 19. Life Flight was not available.

4. Joint – Successfully held first annual Cornelius/Forest Grove Firefighter Banquet with over 150 people attending on April 27. Event was coordinated by DC Dave Nemeyer and Captain Steve Black.

5. Joint - Successfully conducted/participated in countywide disaster drill on April 24, 2012. Joint EOC established with City of Forest Grove/City of Cornelius, testing windshield damage assessment during a mock earthquake and EOC procedures.

6. Forest Grove – two car accident on Highway 47 and Fernhill on April 11 requiring extrication – units from Forest Grove, Cornelius and Metro West responded.

7. Forest Grove – Metro West ambulance had an explosion/flash fire in their oxygen filling station, seriously injuring one employee. While Metro West was dealing with this event, Forest Grove assisted by providing ambulance coverage for the west end and continues to provide oxygen filling for their agency until the investigation can be completed and their system restored.

8. Forest Grove – Fire/explosion on Willamina drive, caused when a welder was working on a drum he thought was empty. One patient was transported to a regional trauma facility. PIO completed 7 media interviews for this event.

Training 1. Joint – April 28, successfully conducted live fire training exercise at acquired

structure on Pacific Avenue, providing live fire training to over 30 firefighters from both agencies.

2. Cornelius – Firefighter JC Butler was promoted to Lieutenant after successfully completing the requirements for the position.

3. Cornelius – Lieutenant George Robards continues training on new Type VI – anticipated in-service date May 15.

4. Joint – Four new Cornelius volunteers and four new Forest Grove volunteer firefighters successfully completed the regional training academy and graduated April 29.

5. Joint – Both departments completed annual physical ability training, conducted by Lt. Phil Duncan and FF/PM Keith Baas.

6. Joint – 5th Monday training for all Forest Grove and Cornelius Firefighters conducted April 30, with presentations from Tom Belusko on insurance coverage and from Karlie Moore, nutrition and fitness expert, who gave presentation on firefighter health, nutrition and exercise programs.

7. Joint – Captain Mike Lanter provided the final portion of the NFPA Aerial Operator on April 28.

8. Joint – Dr. Daya and FF/PM Murphy provided EMS training utilizing the Washington County mobile EMS training simulator and the high fidelity training manikins.

9. Joint – Coordinated with Chief Sam Phillips to provide Incident Safety Officer Class here on November 17.

10. Joint - Representatives from Cornelius and Forest Grove police and fire participated in OCEM tabletop disaster drill at Beaverton Library – focus was on working with local Multi-Agency Coordination Centers and County EOC for resource request in a simulated severe winter weather event. Very successful training.

Fire Prevention/Community Risk Reduction

1. Forest Grove – Conducted monthly child safety seat inspection program for community on April 25. 10 cars/seats inspected. FF Kevin Ritcheson is researching possibility of providing a child safety seat inspection program in Cornelius, with tentative schedule for late May.

2. Forest Grove – Inspections – DMH, a large cabinet manufacturing facility on 24 and conducted crew tour, Strain Science Center (Pacific U), Rainbow Lanes Bowling Alley, El Torero, Jennings McCall Center, Masonic Grand Lodge Office, F&C Machine Shop/Cabinets.

3. Cornelius – Inspections – Sears Hometown Store, Mazatlan Mexican Restaurant, Little Italy, Club Saludable, Cristal’s Decore, Libreria Cristiana Nueva Vida, Zapateria La Muneca, Cornelius 9 Cinemas, Betty’s Dog Grooming, Flax Plant Horse Stable, Tabbs Sports Bar, Coastal Farm and Ranch, Cedar Street Apartments, Parsonage Apartments, Pacific University Scott Hall, Jade Green Palace and La Estrella. and Cornelius Police Department.

4. Forest Grove – Fire Marshal Dave Nemeyer was interviewed by the Hillsboro Argus about juvenile fire setter intervention programs.

5. Joint - Completed distribution of “This School Had a Fire” brochures to public and private schools in Forest Grove. This is a publication created by OSFM for schools to understand juvenile fire setting and inform them of local resources to provide intervention if needed.

6. Cornelius – Started research into code requirements for bio-diesel manufacturing plant at Summit Foods and historical agreements regarding foam trailer.

Projects and Administration

1. Cornelius – Fire Chief provided presentation to joint meeting of Cornelius City Council and Cornelius Rural Fire Protection District on April 30. Presentation was on volunteer firefighter’s recruitment, retention and training.

2. Forest Grove – provided presentation to joint Forest Grove City Council/Forest Grove Rural Fire District on the volunteer firefighter program on April 4. FF/PM Will Murphy and C Shift provided a demonstration of the Washington County Mobile EMS Training Simulator.

3. Joint – Fire Chief attended Washington County Fire Defense Board meeting on April 19, discussions on emergency tones and use with withdraw, abandon and mayday.

4. Joint – Both departments are setting up for performing annual hydrant flushing. 5. Cornelius – Lieutenant Phil Duncan continues working on development of

volunteer recruitment and marketing plan. 6. Cornelius – Working with Firefighters Association to provide BBQ at upcoming

Cornelius Clean-up Day. 7. Cornelius – BC Rob Marcoff and Lieutenant George Robards replaced the motor

on the station compressor, repairing the unit at significant cost savings. 8. Joint – Fire Chief completed 3rd Executive Fire Officer applied research project

and submitted to National Fire Academy – scheduled to attend final Executive Fire Officer class May 20-June 2, 2012.

9. Forest Grove – Firefighter Brad Shinpaugh is finally returning to full-duty with a healed knee (returns May 5).

10. Forest Grove – Division Chief David Nemeyer is coordinating our Memorial Day event, and has submitted request forms for Air Force F-15 fly over conducted at the time of the event.

11. Cornelius – Fire department crews participated in Cornelius Children’s Parade on April 28, 2012.

12. Forest Grove – Reached agreement with the FG School District to conduct a Fourth of July Fireworks event in Forest Grove in coordination with the Forest Grove Firefighters Association.

13. Forest Grove – Fire Marshal Dave Nemeyer worked out a partnership with Pacific University where they will pay for our annual maintenance on our fire extinguisher training prop in return for access to it for their training.

14. Forest Grove – Chad Toomey and I are in the final stages in the development of shared supervising physician agreement with Pacific University.

15. Cornelius – Laura Christy completed the budget book for the Cornelius Rural Fire Protection District.

16. Forest Grove – Fire Marshal Dave Nemeyer continues research into fire alarm monitoring system for Station 7.

17. Cornelius – Thank you letter created for TVFR donation of Type VI engine for Cornelius City Council.

18. Joint – Fire Chief continued work on replacement records management system. The hold up now is the data link between WCCCA and the vendor. We are trying to have dispatch data automatically dumped into our RMS, but we have a situation where people are not playing well together.

19. Forest Grove – Fire Chief completed first round on proposed budget with Finance and City Manager.

20. Forest Grove – Forest Grove Firefighters Association held their quarterly meeting/dinner at Station 4, and planned upcoming fundraising community events.

21. Forest Grove – Division Chief Nemeyer is working on state required documentation for Cadet Program. This includes teacher license documents for Damian Peters, as well as information to help ensure that our students can receive college credit from PCC. This is an ongoing project that we are receiving support from FGHS and PCC as we complete the documents.

22. Forest Grove – Fire Chief attended Washington County EMS Ops meeting. Metro West has agreed to restock EZ IO needles.

23. Joint – Posted wall charts of current strategic plan process at Station 4 and Station 7 for computer-phobic personnel.

Notable Calls and Events 1. Forest Grove – Condo fire at Laurel West Condominiums with successful rescue

of occupant and pets. 2. Forest Grove – Grass field fire on Anderson Road on May 23, limited to about 2

acres. Fires field fire of the season. 3. Joint – Assisted Banks Fire with approximately 20 acre brush/wildland fire. 4. Forest Grove – May 10 barn fire on Spreadborough started by a burn pile where

embers blew into the roof eaves. Units from Forest Grove, Banks and Cornelius stopped the fire with little loss.

5. Forest Grove – May 11 early morning fire at 58166 NW Wilson River highway, a manufactured home that was completely consumed on arrival. Crews from Forest Grove, Cornelius and Banks responded. The cause is under investigation.

6. Forest Grove – Fire Marshal responded to a cold fire at Joseph Gale Elementary School on May 7. This was found by school staff when they came in that morning and consisted of a 2 feet long by 6 inches wide area of combustible liquid and at least one dozen wooden matches. No structural damage, but light damage was noted on the concrete.

7. Cornelius – burning wood shed on SW Plumlee Road – approximately $1000 in damage to adjacent home.

8. Cornelius – Lightning caused tree fire on SW Firdale road, fire at the top of a 100’ fir tree.

9. Forest Grove – Forest Grove Firefighters Association hosted a pancake breakfast raising over $1000.

10. Joint – Forest Grove and Cornelius Fire conducted a joint Memorial Day ceremony at the Community Flag Pole – included flyover by Air Force F-15. Coordinated by Chief Nemeyer.

11. Forest Grove – Responded to two separate natural gas leaks from relief valve on Oak Street, requiring some neighborhood evacuations.

12. Forest Grove – Forest Grove – provided medical standby at Pacific University during graduation ceremonies on May 19.

Training 1. Joint – Chief O’Connor travelled to New York and performed final inspection of

the Mobile Fire Training Simulator. (Received the Mobile Fire Training Simulator June 4).

2. Joint – Began planning for training at old Joseph Gale Elementary School prior to demolition.

3. Joint – Fire Chief completed final class of Executive Fire Officer Program (Executive Leadership) at National Fire Academy.

2. Forest Grove – Fire Prevention posted blog about wildfire awareness week that detailed fire breaks, clean roofs and adequate access.

3. Forest Grove – DC Nemeyer finished the “completeness review” for the new Primetime Sports Bar with no comments or inadequacies in regards to the plan

4. Forest Grove – Division Chief Nemeyer gave presentation of Forest Grove Fire and Rescue to Kiwanis.

5. Forest Grove- Fire Prevention began working with FGPD on National Night Out Event.

6. Forest Grove – Firefighter/AO Jeff Bade is working on preplans for Pacific University.

Projects and Administration

1. Joint – Presentations at Cornelius City Council and Forest Grove City Council for National EMS Week, with unit citations given for CFD and FGR personnel that performed code saves in 2011.

2. Forest Grove - Chief Nemeyer is re-evaluating our high school cadet program – we may not be able to continue this program.

3. Joint – Fire records management system research has hit a wall – problems with linking into the WCCCA CAD system. Re-evaluating options.

4. Forest Grove – DC Dave Nemeyer completed research into fire alarm system for Gales Creek Station and received authorization to proceed.

5. Cornelius – Firefighter John Phelan was promoted to Lieutenant after successfully completing the requirements for the position.

6. Forest Grove – Volunteers Dave Doornink, Bill Shults, and Cody Brown represented FG&R and the Safe Kids Day at the Oregon Zoo event on May 5. This brought in over 5,000 kids to hear various safety messages which our portion was the traditional home fire escape lesson with the TVFR Safety House

7. Cornelius – Captain Marc Reckmann organized fire department participation in upcoming Cornelius Clean Up Day (May 19)

8. Forest Grove – Division Chief Ian O’Connor completed grant amendment and received approval to utilize remaining grant funds for generator for EOC trailer.

transported by Life Flight to OHSU. 2. Cornelius – Lightning caused tree fire on SW Firdale road, fire at the top of a

100’ fir tree. 3. Forest Grove – Retirement party held for Captain Bob Haney, who completed 41

years with Forest Grove Fire and Rescue. 4. Cornelius: Commercial structure fire in nursery greenhouse on Golf Course Road

on June 22, 2012. Fire is still under investigation. 5. Cornelius: Residential fire on Baseline on June 9. Garage fire, one person was

transported for burns. Cause is undetermined at this time, approximately $40,000 in damage.

6. Forest Grove: Motor vehicle accident at Highway 47 and Maple - Life Flight was used to transport one patient.

7. Cornelius: Lieutenant Phil Duncan received City Council proclamation for upcoming Fill-the-Boot campaign in Cornelius.

8. Forest Grove - Fire Department hosted annual FG Employee Breakfast on June 7. 9. Cornelius: Commercial fire dispatched to Virginia Garcia Building on 11th Street

on Wednesday. Smoke discovered in building, cause was determined to be a malfunctioning furnace.

Training 1. Forest Grove – Began planning for field fire training in Verboort in August. 2. Joint – Starting process for volunteer firefighter hiring for fall academy. 3. Joint: Chief O'Connor conducted extensive training at old Joseph Gale School.

Firefighters from across the county utilized the buildings for truck training, roof ventilation, rescue, and wall breaching exercises.

4. Joint: FF/PM Will Murphy conducted monthly EMS training (Dr. Daya on vacation) and simulations, with simulated gunshot victim in parking lot of fire station (this was not an actual call, despite the rumors).

5. Joint: Scheduled NIMS training class for City Employees (ICS 100 class) in mid-July.

6. Forest Grove: Completed in-service training on new mobile fire training simulator and began using in fire department training activities.

Fire Prevention/Community Risk Reduction

1. Forest Grove – Chief Nemeyer wrote guest article on wildfire preparedness published in home and garden section of June 6 FG News Times.

2. Forest Grove – Division Chief Nemeyer gave presentation on Forest Grove Fire and Rescue to Kiwanis.

3. Forest Grove – Fire Prevention met with Clean Water Services to review Fernhill Wetlands Enhancement Project. Preplan tours are scheduled for this summer.

4. Forest Grove – Firefighter/AO Jeff Bade is working on preplans for Pacific University.

5. Forest Grove – DC Nemeyer finished the “completeness review” for the new Primetime Sports Bar with no comments or inadequacies in regards to the plan

6. Joint: Forest Grove and Cornelius crews completed tour of new Joseph Gales School and assisted Fire Marshal with alarm checks.

7. Forest Grove: Fire Marshal David Nemeyer worked with Forest Grove School District officials to identify cause of multiple false alarms at high school. A faulty device was identified and the situation has been corrected.

8. Forest Grove: Reviewed proposed plans for church on Pacific Avenue for occupancy and exit requirements; participated in Plan Review Board meeting for PrimeTime restaurant (fire department requirements have been met).

9. Forest Grove: The May 30 child car seat inspection conducted by FF Geoff McFarland inspected 11 seats in 2 hours, and found 9 of those seats installed incorrectly (82%). All were corrected. Next event is scheduled for June 27.

Projects and Administration

1. Forest Grove – FF Rick Ilg completed respiratory FIT testing for A, B and C Shift.

2. Cornelius – Firefighter Robert Conrath was promoted to Lieutenant. 3. Cornelius – Toured a group of 60 children through station and provided fire safety

lectures. 4. Cornelius – Hydrant flushing is complete – beginning hydrant testing with goal of

July 30 completion. 5. Cornelius – Matt Johnston completed research into foam trailer agreement with

Summit Foods. 6. Forest Grove – Chief Nemeyer met with representatives from the Friends of

Historic Forest Grove to discuss fire protection for A.T. Smith House on Elm Street.

9. Cornelius: At Cornelius City Council meeting provided retirement presentation to Lt. Sheila Jackson, promoted three new lieutenants (JC Butler, Robert Conrath, John Phelan), and presented achievement award to Captain Steve Black.

10. Joint: In support of National Firefighter Safety and Health Week, Lt. Phil Duncan and Lt. JC Butler conducted self-rescue and rapid intervention team training with firefighters from both departments.

11. Cornelius: Fire Chief participated in IT planning meeting, discussing computer system planning, smart phone, iPad use, and technological needs for the City.

12. Forest Grove: Captain Mike Lanter completed review of tender specs developed by TVFR.

13. Cornelius: Type III engine was ordered from HME. Delivered expected in early July.

14. Joint: Proposal for updating our joint intern program is being reviewed by Lieutenants Buckingham and Duncan.

15. Joint: Wildland training is in full swing, with FF/PM Tony Carter conducting training on new Type VI engines for both departments.

20. Cornelius: Successfully sold surplus Type VI truck. 21. Cornelius: Lieutenant George Robards was promoted to Captain effective July 1. 22. Cornelius: Received notification that Type III engine will be delivered in mid-

July. Submitted grant amendment for Type VI, received initial denial but a follow-up email from FEMA recommended an alternative that is being pursued.

23. Cornelius: Received offer for sale of Squirt 8 which was approved by the City Manager.

24. Joint: Successfully completed training house burn in one of the house on Sunset. House was utilized for several days by fire crews for advanced training practice.

25. Joint: Division Chief O’Connor conducted several days of advanced firefighter tactical and skills training at the old Joseph Gale School prior to its demolition.

26. Joint: Successfully completed natural gas leak drill with Northwest Natural Gas representatives in other house on Sunset. The Fire Department also offered this

house for law enforcement drills, which was taken advantage of by WCSO special tactics team. This house is scheduled for live fire training on this coming Monday.

27. Forest Grove: ICS class scheduled for July 24 for city employees is almost full. 28. Forest Grove: Successfully ordered and received three AEDs for the City

Auditorium, Library and City Hall. Joint order with Cornelius resulted in significant cost savings.

29. Cornelius: Successfully ordered and received two AEDs for Cornelius Police Department one for Cornelius City Hall/Library. Joint order with Forest Grove resulted in significant cost savings.

30. Cornelius: Successfully reached agreement in contract negotiations with Cornelius Firefighters.

31. Joint: Submitted conflagration list of qualified personnel to Fire Defense Board for upcoming fire season.

32. Joint: Completed research into grant funded weather station for EOC 4 and prepared for purchase after July 1.

33. Cornelius: Cornelius Rural Fire Protection District meeting was held, with discussions on proposed language changes to City-District agreement. Captain Reckmann completed the requested equipment inventory for the fire board with was presented at this meeting.

34. Forest Grove – Chief Nemeyer is re-evaluating our high school cadet program – we may not be able to continue this program.

Notable Calls and Events

1. Cornelius: Responded to a fire in the kitchen of a triplex located at 945 Davis on July 24. Fire extended into the attic and caused an estimated $30,000 in damage. Occupants were fortunate to have escaped, as they had deactivated their smoke alarm. Crews responded from Cornelius, Forest Grove and Hillsboro. Simultaneously FG units responded to a car fire on Gales Creek Road.

2. Forest Grove: Motor vehicle accident at Highway 47 and Sunset that required Life Flight transport.

3. Forest Grove: Forest Grove Paramedic/Firefighter Ted Penge and Firefighter Brad Shinpaugh successfully delivered a baby at a private residence on Ballad Lane.

4. Forest Grove: Firefighter Association held a pancake breakfast fundraiser on the day of the Concourse, successfully coordinated by Andrew Lanter.

5. Joint: Patrick Wineman, a BC with Marion County, will be joining the Forest Grove and Cornelius Fire Departments as a Volunteer Battalion Chief.

6. Forest Grove: The Forest Grove Firefighters Association received a certificate of appreciation from the Rotary Foundation for their community impact and positive action within the City of Forest Grove. Award was received by Division Chief Rob Hutchens.

7. Joint: Successfully (for the most part) conducted Fourth of July Fireworks display for the community. Unfortunately donations did not match expenditures.

Training

1. Joint: Live fire training successfully conducted at house in Forest Grove on

Sunset Drive on July 2. 2. Forest Grove: Completed training for 45 city employees in basic ICS and

emergency operations plan on July 24. Next class is being scheduled. 3. Joint: Division Chief O’Connor is coordinating a Fall pumper/operator academy. 4. Forest Grove: FF Ivan Bratchuk completed creation of new street and map

training program.

Fire Prevention/Community Risk Reduction

1. Joint – Chief Nemeyer and FF Kevin Ritcheson began planning for October Open House and our fire prevention and education efforts in the schools.

2. Joint – ongoing multiple plans reviews and site inspections for Forest Grove High School, new Wilco building in Cornelius, planned new subdivision in FG, dentist office and multiple new homes in both rural districts.

3. Cornelius – ongoing discussion with Summit Foods regarding sprinkler systems and Cornelius Foam Unit 8.

4. Successfully conducted monthly child safety seat training. 5. Duty crews continue to participate in public education and awareness at

downtown Wednesday Farmer’s Market. 6. Forest Grove: Jeff Bade completed preplans for all Pacific University buildings.

Projects and Administration

1. Forest Grove: Truck 4 did not pass its annual aerial test – some cracks in the outrigger needed welds and some other problems. Repairs are just about complete. Will hopefully be back in-service by July 31.

2. Joint: Department participating in this year’s conflag mobilization include Forest Grove, Cornelius, Hillsboro, Banks and TVFR.

3. Joint: Currently reviewing the service provider of firefighter medical exams. All other Washington County departments have stopped utilizing Concentra. Researching other providers.

4. Forest Grove: Completed installation of fire and burglar alarm at Gales Creek Station.

5. Forest Grove: Received notification from FEMA that we have received a SAFER grant to fund a Lieutenants position responsible for volunteer recruitment and retention. They did not fund the full amount, and this is not meant to be a matching grant. SAFER finally returned calls on July 23 and they are investigating – they agree there appears to be a mistake. Awaiting a return call from them to determine next actions.

6. Cornelius: Received the new HME Type III and began placing in service. Anticipate having it in-service by August 15. Will submit proposed amendment for purchase of Type VI after the Type III is in service.

7. Joint: Fire Chief attended Metro Fire Chiefs meeting, with presentations on property tax system and future predictions by Chief Mike Duyck, and discussion on ISO rating system and how to respond to a request to rate our department (stall).

8. Joint: Re-designed intern program to create joint fire intern program – awaiting approval of FEMA of amendment.

9. Joint: Senior Officer meeting held on July 24. SOG on Social Media was distributed to officers for discussion and input.

10. Forest Grove: FF/PM Rick Ilg began research for purchase of SCBA communications system.

11. Forest Grove: Stryker stairchair ordered for Rescue 4. 12. Forest Grove: Reviewed bids TVFR received for new tenders – bids were too

high, currently reconsidering options. 13. Joint: Yamhill patch update is complete, waiting on YCOM to complete training

on this to implement.

14. Cornelius: Completed AFG application for air/rehab trailer and two thermal imager cameras.

15. Cornelius: Successfully sold SQ8 for $37,000 to a fire department in Idaho. 16. Joint: Received AEDs for Forest Grove City Auditorium, City Hall, Library and

Cornelius City Hall. These are being installed July 25. 17. Joint: Forest Grove and Cornelius conducted a joint physical ability test for new

volunteer firefighter candidates. Seventeen were tested, 16 passed. 18. Joint: Began purchasing process for four new gas monitors (2 for FG and 2 for

Cornelius). 19. Forest Grove: Ordered portable weather station (grant funded) for EOC trailer. 20. Joint: Washington County Fire Chiefs met and discussed increased sharing of

joint purchasing to include uniforms, turnouts, apparatus and fuel. Departments agreed to joint hose testing contracts to reduce costs. Discussed shared volunteer firefighter programs.

21. Forest Grove: New Type VI engines and Type III engines are complete and in-service.

Notable Calls and Events

1. Cornelius: Cornelius firefighters conducted a successful "Fill-the-Boot" campaign on August 24 and 25, raising over $11,000 for the Muscular Dystrophy Association.

2. Forest Grove: On August 15 C shift responded to assist a victim of domestic abuse. After treating her injuries, the crew utilized funds provided by the Forest Grove Firefighters Association (and with help from the hotel staff) to get the victim out of the house (the suspect was still at large) and temporarily housed at McMenamins Hotel.

3. Joint: Up-staffed both departments during the high temperature "red flag" heat wave due to the high fire danger.

4. Forest Grove: Field fire on Martin road, confined to 1 acre. 5. Forest Grove: Medic 4 provided two transports to area hospitals in the last 30

days due to system overload calls. 6. Forest Grove: August 9 Group home residential fire in the 2600 block of Main

Street, started by a cigarette on a mattress. 7 people were displaced by this fire, Captain Joe Smith injured his leg trying to prevent an occupant from re-entering the burning building.

7. Cornelius: August 5 field fire on Dober road – crews were able to stop a 6’ foot flame front from spreading to a adjoining wheat field.

8. Forest Grove: August 1 wildland fire (1-2 acres) off on Clapshaw Hill road, started by a unattended burn pile.

9. Forest Grove: August 1 cardiac arrest event on Fernhill Road. 10. Forest Grove: Responded to mutual aid request for house fire on July 30. 11. Cornelius: July 27 house fire on South Cherry street – fire was confined to the

attic, caused by a recessed light that had malfunctioned. Estimated damage of $10,000.

12. Cornelius: July 27 tri-plex fire on N. Davis Street, started by a cooking fire – estimated $30,000 in damage to the structure. No working smoke alarm in the structure.

13. Cornelius: August 2 - fatality motorcycle accident near Fred Meyers. 14. Forest Grove: August 2 – Motorcycle accident on Pacific Blvd with two critically

injured patients, one transported by Life Flight. 15. Cornelius: Bicycle accident on Adair – victim was unconscious, transported to a

regional trauma center. 16. Cornelius: July 31 FF Andrea Pumajro and crew successfully delivered a baby. 17. Forest Grove: The weekend of August 4-5 Forest Grove Fire responded to 32

emergency incidents, including a propane explosion in a converted van in the Gales Creek area.

Training

1. Joint: FF/PM Ted Penge conducted a series of successful high-rise drills at

Pacific University campus. 2. Joint: Fitness expert Karlie Moore provided another class on firefighter health

and nutrition at the Community Auditorium for Washington County firefighters. 3. Forest Grove: Provided emergency plan and incident command training to over

40 city employees – next class (joint with Cornelius) scheduled for September. 4. Joint: Completed physical ability test and interview process for new volunteers –

Cornelius has hired two new recruits and Forest Grove four new recruits. 5. Forest Grove: Wildland fire training drill conducted on Thatcher Road – 27 acres

of wheat field stubble was donated by local farmer for the training. Crews from Forest Grove, Cornelius, Banks, and ODF participated in the four hour field burn drill.

6. Joint: Dr. Daya provided monthly EMS training to the crews on August 21.

Fire Prevention/Community Risk Reduction

1. Forest Grove: FF Ivan Bratchuk provided CPR training to 9 city employees on August 7 and August 10 (project is to train all City employees).

2. Joint: Cornelius and Forest Grove Fire personnel participated in National Night Out Events in Cornelius, Forest Grove, and Gales Creek.

3. Began planning for October Open House and Fire Prevention Week. Projects and Administration

1. Forest Grove: Received formal notification from FEMA that our grant award has been increased and fully funded to $420,000 for a four year Lieutenant position. Supplemental budget request scheduled to go to City Council September 24. If approved Lt. Tad Buckingham will fill this position.

2. Cornelius: Agreement with Cornelius Professional Firefighters was taken before Cornelius City Council on August 6 and approved, and contract was ratified by union.

3. Forest Grove: Submitted 2012 SAFER grant to fund Captains position lost in 2009.

4. Cornelius: Submitted 2012 SAFER grant to continue funding of volunteer coordinator position funded since 2008.

5. Cornelius: Amendment to change reimbursement for interns was approved by SAFER. This will allow us to develop our joint intern program.

6. Forest Grove: Truck 4 was returned to service after repair of deficiencies noted in annual testing – awaiting final certification.

8. Forest Grove: FF/PM Rick Ilg completed annual tests of SCBA masks. 9. Cornelius: A public AED was installed in City Hall and two AEDs were provided

to the Cornelius Police Department for their police cars. CFD acquired the two AEDs through a Walmart grant.

10. Forest Grove: Three public AEDs were installed – one in the library, one in the Community Auditorium and one in City Hall.

11. Forest Grove: Both new Type VI engines are now in service and responding to fires.

12. Forest Grove: Received grant funded portable weather station - Captain Smith will be working on installing and training crews on this while on light duty.

13. Joint: Began planning for Cornelius Fire Department 100 year anniversary in November.

14. Joint: Forest Grove and Cornelius successfully coordinated hose testing for Western Washington County fire departments - anticipated completion this week.

15. Joint: Began implementation of joint intern program after receiving approval from FEMA.

17. Joint: Implemented new policy on social media use. 18. Joint: Fire Chief published article in August issue of Fire Engineering on

Notable Calls and Events

1. Forest Grove: September 16 Commercial Fire at Best Western on Pacific, started by a discarded cigarette in the bark dust. The fire extended into the attic, activating 11 sprinkler heads. There was smoke and water damage to the west wing of the hotel and the hotel has been shut down for repairs. Estimated loss is $100,000.

2. Forest Grove: September 15 Residential fire at Main and Willamina, fire started on an exterior fence. Cause is under investigation.

3. Joint: Multiple small brush fires are occurring now due to the extremely dry conditions. All recreational burning has been banned in both City’s/Districts.

4. Joint: September 11 Memorial was conducted at Flag Park. 5. Forest Grove: On September 15 Forest Grove and Cornelius Firefighters assisted

with the Forest Grove Corn Roast (roasting all the corn), provided coverage at the Pacific University Football games (this was a last minute unscheduled game that we found out about one hour prior to kickoff), and provided medical support to the annual Lions Run.

6. Forest Grove: Forest Grove Fire and Rescue assisted Washington County sheriff’s office with the recovery of the body of a suicide victim in a remote area off of Highway 6.

7. Forest Grove: Volunteer Firefighter David Doornink coordinated the annual “Fill the Boot” for the Muscular Dystrophy association, raising over $7000 in a six hour campaign.

8. Joint: 8 firefighters from Forest Grove and Cornelius attended the State Fallen Firefighter Memorial Ceremonies held in Salem on September 20.

9. Forest Grove: City Council approved the supplemental budget to add the SAFER funded Lieutenants position. Lt. Tad Buckingham starts October 1 with this.

10. Forest Grove: Completed Chief’s interviews for shift Lieutenant position. Keith Baas was selected for promotion to Lieutenant.

Training

1. Cornelius: Crews from Cornelius and Forest Grove have been doing

familiarization tours of the new Virginia Garcia facility. 2. Joint: Division Chief O’Conner is currently coordinating a joint NFPA Pumper

12. Forest Grove: Fire Chief attended WCCCA Board of Directors meeting representing Council President Tom Johnston. Presentations on our radio system (which has to be replaced in the near future) and the five-year budget picture for WCCCA.

16. Forest Grove: Fire Chief attended PSAC meeting. Presentations on the downtown traffic study was provided by Rob Foster, along with tours of new mobile fire training simulator.

Notable Calls and Events

1. Forest Grove: Received notification from the Oregon State Fire Marshal that Division Chief Dave Nemeyer has received the “Golden Sparky Award” for 2011. This is the most prestigious award in the state for fire prevention officers. The Golden Sparky recognizes a fire service member for outstanding achievement in fire prevention or public fire safety education. The award will be presented at the joint Oregon Fire Chiefs/Oregon Fire District Directors Conference on November 9, 2012.

2. Forest Grove: Conducted written testing for firefighter candidates: the top 25 were selected to proceed to the physical ability testing on November 2. 15 of the candidates are from Forest Grove/Cornelius Fire Departments.

3. Forest Grove: Conducted annual “Be Seen at Halloween” campaign where firefighters handed out over 2000 light-sticks to trick-or-treaters. This project is funded by the Forest Grove Firefighter’s Association and the Forest Grove Police Officers Association. Later that evening these safety-orange light-sticks were seen in the hands of trick-or-treaters throughout Forest Grove and Cornelius.

4. Joint: Firefighters from both departments participated in Breast Cancer Awareness month by wearing pink shirts.

5. Joint: Both departments have responded to and extinguished multiple chimney fires in the past month.

6. Forest Grove: Lt. Tad Buckingham attended the Gales Creek Neighborhood Watch meeting and provided a presentation on our volunteer firefighter program. Two interested people requested applications.

7. Forest Grove: Conducted a successful Public Safety/Fire Prevention Open House on October 13, with approximately 800 people attending.

8. Cornelius: Conducted a successfully Fire Department Fire Prevention Open House on October 14, with approximately 300 people attending.

9. Joint: The Fire Prevention Division provided fire prevention presentations to over 2300 children at Forest Grove School District schools in both Forest Grove and Cornelius.

10. Forest Grove: October 1 major MVA at Highway 47 and B Street requiring extrication and Life Flight transport to the trauma center. Units from Cornelius and Gaston assisted.

11. Cornelius: Firefighters conducted fund raiser for Seattle Stair Climb (car wash), raising over $500 for the charity.

12. Forest Grove: FGFR provided medical and event support to two Pacific University football games during this time period.

13. Joint: The outdoor burn ban was lifted in all areas served by FGFR and CFD on October 15.

1. Forest Grove: Division Chief Ian O’Connor and Lieutenant Keith Baas attended

classes at the National Fire Academy in Emmitsburg, MD. Chief O’Connor completed a two-week class in Training Program Management; Lt. Baas completed a one-week class in Command and Control of Incident Operations.

2. Joint: Fire Chief worked with representatives of Hillsboro Fire Department to test new system of shared video conference training for both departments. This is expected to go live in November.

3. Cornelius: Captain George Robards completed 2nd of 3 weeks of the Metro Fire Officer I class.

4. Joint: The Fall Volunteer Recruit Firefighter Academy was completed, with 5 Forest Grove/Cornelius volunteers graduating on October 28.

5. Joint: Dr. Daya provided EMS training on October 15 to both departments. 6. Joint: Fire Chief attended FEMA class MGT-315 Enhanced Threat and Risk

2. Forest Grove: FF Ivan Bratchuk completed CPR/AED training for the library. 3. Cornelius: Intern Jeff Lewis completed seven new pre-plans for the department. 4. Joint: Division Chief Nemeyer and FPT Matt Johnston completed inspections of

local haunted houses in both communities. 5. Cornelius: Intern Jeff Lewis, who is on-light duty following an off-duty injury, is

working on creating preplans for high risk occupancies (using same system as Forest Grove). He has completed seven to date.

6. Forest Grove: Division Chief Nemeyer has started the process for our annual community Toy Drive, receiving commitments from KGW Channel 8, several business and local community organizations.

7. Cornelius: FPT Matt Johnston completed plan reviews for new Wilco Store, Virginia Garcia and several other businesses.

1. Joint: Fire Defense Board meeting attended October 18 – joint Fire Defense

Board meeting of Washington County and Yamhill County Fire Chiefs. Discussions on the Yamhill radio patch with Washington County units and medical response to unprotected areas.

2. Cornelius: Fire Department personnel attended grand opening ceremonies of new Virginia Garcia clinic.

3. Cornelius: Captain Steve Black has set up a fundraiser for the Seattle Stair Climb at 24 Hour Fitness, scheduled for mid-November.

4. Forest Grove: Lt. Tad Buckingham developed a Volunteer Recruit Firefighter Application handbook, submitted an OJT program to the State for the Veterans Administration, and developed guidelines for non-combat volunteers.

5. Joint: Lt. Phil Duncan and Lt. Tad Buckingham conducted needs assessment meetings with all volunteers.

6. Joint: Forest Grove and Cornelius both held their monthly child safety seat clinics.

16. Forest Grove: Forest Grove Firefighters Association met on October 9 and decided to again conduct the July 4th Fireworks Program. The association is now a non-profit that you can make tax deductible donations to them.

17. Joint: Began testing of new iPad app for both departments (Active911) that forwards calls and mapping info to each officer’s iPad.

18. Joint: Fire Chief attended presentation by PGE on their storm response and emergency response plans.

19. Forest Grove: Forest Grove RFPD meeting held - presentation by Rob Foster on Downtown traffic study.

20. Cornelius: Completed review of contract between City of Cornelius and Cornelius Rural Fire Protection District, received agreement from attorneys and City staff – contract forwarded to Cornelius Rural Fire Board for action. Brought contract to the October meeting of the Rural Fire Board members. Working on two minor revisions.

21. Joint: Completed update and review of Forest Grove/Cornelius contract for Fire Chief Services and submitted Council reports to both City Recorders.

22. Cornelius: Completed annual evaluations for all Cornelius career staff. 23. Joint: Fire Chief attended Washington County EMS Ops meetings, receiving

reports on private ambulance response times - discussion on EMS system enhancements.

24. Joint: Captain Marc Reckmann completed research into new Class A/B foam. This has been approved for a joint purchase with a significant savings for both departments.

25. Forest Grove: Captain Joe Smith completed a redesign and reorganization of the EOC trailer, and completed training of grant-funded weather station that has been added to the EOC.

26. Joint: Captain Steve Black and Sharon Cox have developed a joint uniform ordering and inventory system to reduce costs and improve coordination. Ordering of needed uniforms is underway.

27. Forest Grove: Lieutenant Tad Buckingham started on October 1 in his new position funded by SAFER for volunteer recruitment and retention.

28. Forest Grove: Division Chief Dave Nemeyer is working with Human Resources on the recruitment of a new firefighter – over 160 applied; 100 have been invited for the written test scheduled for October 25.

29. Forest Grove: Lieutenant Tad Buckingham interviewed and hired a Pacific University Work Study intern for FGFR.

Notable Calls and Events

1. Forest Grove: Forest Grove: Structure fire in large (5000+ square foot) house on Clapshaw Hill drive, started by dryer fire on December 1. Damage limited to smoke damage and dryer.

2. Cornelius: Cornelius Fire Department assisted with set up and provided transportation for Santa Claus at the November 30 Holiday Tree light ceremony.

3. Joint: Forest Grove and Cornelius Fire personnel participated in Holiday Light Parade, decorating Truck 4, Boat 4, and Heavy Brush 4. The 1906 FG Steamer was pulled by three draft horses as part of the parade. Afterwards the departments successfully held their annual children’s party for both departments.

4. Forest Grove: Significant motor vehicle accident at Martin Road and Highway 47 with three critically injured patients. Medic 4 transported one, assisted by Cornelius Fire units.

5. Forest Grove: Arson car fire on Fernhill Road – car exploded on crew arrival and spread burning gasoline 10’ in all directions on November 27.

6. Cornelius: Responded with law enforcement to a 6 hour hostage event on Saturday, November 3 where the assailant had poured gasoline over his girlfriend and his house. The girlfriend escaped, but he barricaded himself in the house. Unified command established with law enforcement with units assisting from Forest Grove Fire and Rescue.

7. Joint: Hosted FDNY Lieutenant Joe Torrillo at Forest Grove Auditorium for presentation on lessons learned on 9/11 – Lt. Torrillo was a survivor of the World Trade Towers collapse.

8. Forest Grove: Volunteer firefighters provided medical standby coverage for the annual Verboort Sausage Dinner.

9. Cornelius: Significant motor vehicle accident on November 19 on Susbauer Road. Two large trucks collided head on, with one individual entrapped and requiring over 20 minutes to cut out of the vehicle. Crews from Forest Grove assisted with extrication, one patient was transported to a regional trauma center.

10. Cornelius: Chimney fire response November 7, requiring over an hour to extinguish and overhaul.

11. Cornelius: Captain Steve Black coordinated a fundraising event for the Leukemia Society with a 24 hour stair climb at 24 Hour Fitness, raising $374. They have raised over $1000 so far total.

12. Joint: Both departments have started their annual Holiday Toy Drive. Donations are being accepted.

13. Forest Grove: 25 people made it to the interviews for the current firefighter vacancy. After two-days of interviews, five were selected to go to Fire Chief Interviews scheduled for December 4.

Training

1. Forest Grove: 2. Joint: Fire Chief attended class on project management. 3. Joint: Began planning for February AIC/Officer testing process. 4. Joint: Conducted confined space drills for A and C shifts. 5. Joint: Completed mandatory bloodborne pathogens training and immunizations

for both departments. 6. Cornelius: Completed training on new multi-gas air monitors. These monitors

detect hydrogen sulfide, carbon monoxide, LEL and oxygen level. 7. Cornelius: Captain George Robards completed the Metro Fire Officer I

academy. 8. Joint: On-going training with Northwest Natural Gas at their Sherwood facility,

training crews in the use of new gas monitors donated by NW Natural and response to natural gas incidents.

9. Joint: Live fire training conducted by Division Chief O’Connor on November 6 for C Shift with Mobile Fire Training Unit. Currently getting several warranty issues repaired on the unit, and working on installation of shore line, scheduled for completion in mid-December.

1. Cornelius: Captain Steve Black gave presentation to the Cornelius Boosters club. 2. Cornelius: FPT Matt Johnston acquired 35 smoke alarms from OSFM to place on

engines for community distribution. 3. Forest Grove: Fire Prevention completed final inspection for certificate of

occupancy for Juniper Gardens Apartments. 4. Forest Grove: Chief O’Connor working on implementation of training module in

new records management software. Go live date scheduled for January 1. 5. Joint: Regional EMT B class scheduled to start January 2013. 6. Forest Grove: Hillsboro Fire will be covering Forest Grove fire on Dec 8 so

volunteer and B shift crew can attend RIT training in Sherwood using props developed by TVF&R.

Projects and Administration 1. Cornelius: Cornelius Rural Fire Protection District met November 27 – Contract

amendments were approved with next step to take contract before Cornelius City Council.

2. Forest Grove: Public Safety Advisory Committee held their monthly meeting, with presentation provided by FGPD on mental health issues in our community.

3. Forest Grove: FF Geoff McFarland conducted another successful Child Safety Seat program (monthly program).

4. Forest Grove: Fire Department features in positive News-Times article about grant successes and use of technology in the department.

5. Cornelius: Obtained chassis in Idaho for Type VI project. Working on delivery and next actions for building this engine.

6. Forest Grove: Division Chief David Nemeyer is updating the fire department web site.

7. Cornelius: Captain Reckmann ordered and received the two new ventilation chainsaws – working on placing them in-service. Also placed new flashlights in-service for all personnel.

8. Cornelius: New MSA gas monitors are in-service (grant funded). 9. Forest Grove: Electrician completed boring, conduit and electrical box for mobile

fire training unit – Kidde sent a rep to do some warranty repairs, several issues still outstanding.

10. Forest Grove: Have received tentative indicators that we have received a four-year SAFER grant to fill the Captain/Fire Inspector position that was frozen in 2009.

11. Cornelius: Received approval from FEMA for a grant amendment to fund a Type VI engine identical to those recently completed in Forest Grove. Must be completed by February 23, began process of building these engines.

12. Forest Grove: Decision made to maintain minimum staffing of three due to recent high overtime usage because of injured officers. Will re-examine at end of December to determine if we can return to minimum staffing of four.

13. Forest Grove: Completed physical ability test for new firefighter applicants. All 25 applicants passed.

14. Joint: Went live with new web-based records management system for both departments.

15. Joint: Completed planning meeting with Lt. Woodford and Captain Reckmann to begin joint purchasing of turnouts and PPE to improve quality, coordination and reduce costs.

16. Forest Grove: Forest Grove RFPD held monthly meeting – updates on Cornelius Fire Chief contract provided, and received approval to sell sign-making machine.

17. Forest Grove: Forest Grove City Council approved continuing the contract for Fire Chief Services with the City of Cornelius. This is now complete.

18. Joint: Fire Chief met with new vendor for possible provision of medical exams for both fire departments and the City of Forest Grove. Next action is to evaluate the services they provide by having some personnel have their annual medical exams at this facility.

19. Forest Grove: Lt. Tad Buckingham completed the development of a Volunteer Recruit Firefighter Application Handbook. Also developed a Veteran’s OJT program with the State – awaiting Federal approval.

20. Fire Chief attended ARMUP review, where the use of fire department ambulances (during times of system overload) is reviewed. The system is going well, although there is still a problem with receiving early notification from MetroWest of Level 0 status (this is when there are no ambulance resources in the County).

21. Cornelius: Cornelius Firefighter Association meeting held November 16, with planning for the upcoming Crab Feed. Decision was made to combine this event with a celebration of the Cornelius Fire Department’s 100th Anniversary. There was also a decision to review and update the constitution and bylaws by the members attending.

22. Joint: Fire Chief completed IS 704 and IS 247 emergency management (NIMS) training.

23. Joint: Continuing implementation of new RMS system – overall going well. 24. Forest Grove: Met with Forest Grove Police Department to discuss planning to

provide their CPR and First Aid training. Classes scheduled for officers with FF Ivan Bratchuk in December.

25. Joint: On-going planning for participation in several upcoming holiday events by the Fire Department, including Toy Drive, tree-lighting ceremonies (both Cities), holiday light parade and fire department children’s party.

1. Joint: Volunteer Firefighter Jason Horner lost his four-year old son in an accident that occurred in Dallas, Texas. Fire department personnel assisted family with services, reception held at Forest Grove Fire Station.

3. Forest Grove: Residential fire on December 14 at Maple and 17th. Started by in proper wiring, $25,000 in damage.

4. Forest Grove: House fire on Dilly road on December 24, started in kitchen. Two children and one adult were transported for minor smoke inhalation. Estimated loss of $50,000.

5. Forest Grove: Apartment fire on December 28 on Elm Street, started by overloaded extension cords, loss of approximately $15,000.

9. Joint: Set up EOC for possible wind storm event on December 16, 2012 (event never materialized).

10. Division Chief Dave Nemeyer was presented with the Golden Sparky Award by the OSFM on December 10, 2012.

Training 1. Forest Grove: Lieutenant Chris Woodford completed the class curriculum design

for Maritime Boat Operator: Awareness and Operations. 2. Forest Grove: Completed shore line installation for fire training simulator. 3. Joint: Lieutenant Tad Buckingham completed first draft of active shooter

protocol and joint training with FGPD and CPD. 4. Joint: Began implementation of new training records management module. 5. Joint: Completed gas monitor training with Northwest Natural. 6. Joint: Provided rope rescue and confined space rescue training.

Fire Prevention/Community Risk Reduction

1. Cornelius: Captain Steve Black gave presentation to the Cornelius Boosters club. 2. Forest Grove: Forest Grove and Cornelius firefighters provided CPR training to

over 50 members of the Elks.

Projects and Administration 1. Forest Grove: Placed nine new APX portable radios in service (grant funded). 2. Forest Grove: Received formal notification of SAFER grant award for Captain's

position. 3. Forest Grove: Completed hiring process for new firefighter. Volunteer firefighter

Chad Toomey starts January 14, 2012. 4. Cornelius: Acquired new grant funded chassis for Type VI build for CFD and

painted. 5. Cornelius: Captain Steve Black working with Red Cross to establish a shelter

location in Cornelius. 6. Forest Grove: Fire department hosted the City Holiday party with the police

department. 7. Forest Grove: Fire Chief participated in Salvation Army bell ringing event, raising

over $300. 8. Joint: Both departments participated in the Holiday Parade, tree lighting

ceremonies, and holiday party's. 9. Joint: Fire Chief attended Fire Defense Board and Washington County EMS Ops

meeting. Joint Rural Fire District meeting scheduled for February 12 in Cornelius. 10. Strategic plan update is posted here:
