D e a r F r i e n D s ,The photograph on the cover says it all! �01� was a year of capacity-building and infrastructure

investment, all the while maintaining our commitment to our mission and the community we

serve. The highlight of the year was on September �1, �01�, when we were joined by hundreds

of patients, founders, providers, staff, donors, volunteers, and community representatives in

cutting the ribbon on the expansion of our Mashpee facility.

The opening of our new building illustrates what CHC

is all about: quality, comprehensive, and affordable care.

Community Health Center of Cape Cod, like community

health centers in general, performs exceedingly well in

nationwide studies, formal reviews, and in the eyes of the

patients we serve. With the introduction of pharmacy,

imaging, and walk-in care, our continuum of care continues

to grow. By accepting the public insurances that many

private practitioners do not, and offering a sliding fee schedule for those with no insurance at

all, our services are affordable and accessible to all - regardless of who they are, where they live,

what language they speak, or the complexities of their care.

What does the road ahead look like? With substantial evidence that community health centers

provide first-rate care, the focus is shifting to health centers as the fiscally responsible solution

because of our ability to balance quality with cost containment and efficiency.

We are ready for the challenge! In the midst of uncertainty at both the state and federal

levels, we are restructuring how we provide care, towards a model that emphasizes prevention

and facilitates patient involvement in decision-making. We are fortunate to have the ingredients

for success in place, including a skilled and dedicated staff, a strong management team, an

engaged Board of Directors, a generous community, and more than 14,000 patients who have

entrusted their care to us.

We thank them, and we thank you for your interest in Community Health Center of Cape Cod.

q u a l i t yAs the nation turns its attention to the implementation of the

Affordable Care Act, the spotlight is on community health centers,

which are expected to care for as many as 40 million people.

Community Health Center of Cape Cod is a proven leader in

providing quality, cost-effective care.

A study by Kaiser Health News examined the nation’s 1,�00

community health centers’ performance on six quality of care

measures against national averages. The results of this study

showed that Community Health Center of Cape Cod exceeded the national average for the

services we provide!

In December, �01�, the Health Resources and Services Administration (HRSA), our federal

grant oversight agency, conducted a formal site survey over three days. The survey reviewed

every aspect of Health Center operations, from services to governance to management and

finance. The results were outstanding. The Health Center met 100 percent of the program

requirements, with no deficiencies, which is an extremely rare occurrence according to

survey team members. The team also recognized CHC’s “benchmark” quality and patient

access programs as well as the dedication and enthusiasm of staff.

Today’s healthcare system can be confusing. At CHC, as a patient-centered medical home,

we work with patients to keep them healthy and to help them find their way through the

system. Part of this is to provide care that is comprehensive, which means the care they

need, when they need it, in keeping with their preferred language and cultural beliefs.

The care people need - In �01�, we introduced a number of important new services for our

patients. Our on-site pharmacy is a great convenience for all our patients, with the added

comfort of knowing that their provider and pharmacist work together as part of their

healthcare team. Walk-in care is available for our patients’ urgent needs such as flu, cold,

sore throats, and other minor illnesses or injuries. In concert with Falmouth Hospital, state-

of-the-art x-ray and ultrasound services are provided at our expanded Mashpee location, with

mammography to be available in the future.

When they need it – Services are available days, evenings, and Saturdays, in three locations

throughout our service area.

In keeping with their preferred language and cultural beliefs – Signage, forms, educational materials, and

patient satisfaction surveys are available in Portuguese and Spanish, the two most common

preferred languages at CHC after English. Bi-lingual (in Portuguese) certified medical

interpreters and other staff assist with patient visits, and interpreters for other languages are

just a phone call away. We work closely with consumer partners to make sure that services

are provided appropriate to the language and traditions of every patient.

a F F o r D a b l eCape Cod ranks ninth out of 14 counties in Massachusetts in median household income and

many residents struggle to balance the costs of housing, food, transportation, and health care.

True to its mission, CHC cares for an overwhelming majority of low income residents.

Seventy-six percent of CHC patients earn at or below �00 percent of the federal poverty level

(FPL). For an individual, this is $��,980; for a family of four, it is $47,100 per year. As shown

below, we care for the majority of low income individuals in East Falmouth, Mashpee, and

Falmouth village.

Access to affordable dental care on the Cape significantly improved with

the expansion of our dental clinic from three to 10 chairs. We now have the

capacity to care for 8,000 patients, up from �,�00. If our patients do not

have dental insurance or it only provides partial coverage, we offer a sliding

fee structure based on income.

The new pharmacy at CHC means that patients with inadequate or no insurance no longer

have to make the choice between medications and rent or food. As a federally designated �40

B pharmacy, we are able to purchase pharmaceuticals at significantly reduced costs. Patients

with private insurance purchase their medications at the same cost they would pay a retail

pharmacy. We use the savings to provide medications to

eligible patients at a cost they can afford.

At 10.4 percent, the rate of uninsured people on Cape

Cod mirrors that of urban areas such as New Bedford,

Brockton, and Lynn, according to a recent report by

the Blue Cross Blue Shield Foundation of Massachusetts.

For these individuals, our sliding fee schedules are the

difference between receiving medical, behavioral health,

and dental services and going without. Massachu-
