Preface

ISN gathers and manages a comprehensive range of health, safety and environmental (HSE) performance data from contractors and suppliers in capital-intensive industries. In addition to HSE, procurement, quality and compliance information, annual and quarterly HSE perfor-mance statistics are collected through ISN’s web-based system, ISNetworld, then reviewed and verifi ed by ISN’s Review and Verifi cation Services (RAVS) for accuracy and completion.

The main purpose of this report is to provide ISNetworld member contractors, suppliers and Owner Clients pertinent HSE performance information useful for benchmarking their organi-zation’s performance against industry peers.

Collectively, more than 43,000 contractors and suppliers actively report information through ISNetworld, representing more than 10 billion work hours of activity in 2011 alone. A large proportion of the 43,000 contractors (which represents about 5 billion work hours) belong to the U.S. Oil and Gas industry. A rich source of data such as this provides an opportunity for developing industry-specifi c benchmarking statistics that are unique for their quality, insight and comprehensiveness.

This report specifi cally covers information pertinent to the U.S. Oil and Gas industry for the year 2011 – utilizing data from contracting companies who work in the U.S. Upstream (Exploration and Production), Midstream (Pipeline and Terminals) and Downstream (Refi ning) industries. ISN intends to develop similar HSE performance indicator reports annually, as well as for other industries and countries in the future.

The health and safety Key Performance Indicators (KPIs) collected and presented in this report include:

• Total recordable injury and illness rate (TRIR)

• Total nonfatal recordable injury and illness rate (Nonfatal TRIR)

• Days away, restricted or job transfer rate (DART)

• Fatality rate per 100,000 employees

• Total number of fatalities

These KPIs are organized into two tables: Table 1 provides detailed health and safety performance indicators aggregated by NAICS code. For added granularity, Table 2 provides performance indicators grouped by ISN Work Type designation. All values in this report are aggregated to preserve the anonymity of contractor and supplier companies. Appendices 1 through 3 provide defi nitions, calculations and analysis procedures.

It is ISN’s hope that users will fi nd the report useful for benchmarking their HSE performance against that of their peers’ and for driving continual health and safety performance improve-ment eff orts in the workplace.

Appendix 1: Defi nitions

North American Industry Classifi cation System (NAICS) Code: The standard statistical classifi cation codes underlying all establishment-based Federal economic statistics, classifi ed by industry for the United States, Mexico and Canada. Under NAICS, a specifi c industry is represented by a two, three, four, fi ve or six-digit numeric code. See the U.S. Census Bureau’s NAICS Web site1 for more information.

ISN Work Type: A detailed list of commonly performed work classifi cations and activities available in the ISNetworld2 database for selection by contractors. ISN Work Types cover multiple industries, and contractors are required to select at least one work type that best describes their activities or specialization.

Recordable Injury and Illness: An occupational injury or illness that requires medical treatment more than simple fi rst aid and must be recorded and reported under the U.S. Occupational Safety and Health Administration (OSHA) recordkeeping requirements. See 29 CFR Part 19043 for OSHA recordkeeping requirements and defi nitions.

Days Away Injury and Illness: A recordable injury or illness resulting in one or more days away from work.

Restricted Activity Injury and Illness: A recordable injury or illness resulting in restrictions or limitations on an injured worker’s work activities.

All data used in this report is obtained directly from contractor companies’ OSHA 300 and 300A forms and are reviewed and verifi ed by ISN Review and Verifi cation Services (RAVS).

1. Incident Rate Calculations for Individual Contractors

1.1. Total Recordable Incident Rate [TRIR] represents the number of recordable injuries and illnesses per 100 full-time workers and is calculated as:

Appendix 3: Analysis Procedure Summary

The procedures used in preparing and analyzing the data presented in Tables 1 and 2 are summarized below.

1. Data Cleaning and Preparation1.1. Remove records where the total number of exposure hours is missing or equal to zero.

Only records from U.S. contractors in the Oil and Gas industry are utilized.

1.2. Identify invalid records due to missing data (such as missing Total Number of Record-able Injuries and Illnesses) for each incident rate calculation.

1.3. Remove records where Exposure Hours (EH) per Employee is less than 250 or more than 5,000.

1.4. For each valid record, calculate injury and illness and fatality rates (TRIR, Nonfatal TRIR, DART and Fatality Rate) according to the formulas for individual contractors in Appendix 2.

2. Injury and Illness Rates, Fatality Rates and Fatality Counts by NAICS CodeThe injury and illness and fatality rate calculations by NAICS code for the cleaned data set include the following steps:

2.1. Calculate Total Number of Contractors (∑iNCONTRACTOR ) for each two, three, four, fi ve and six-digit NAICS code, i. For instance, calculations presented for two-digit NAICS codes will include all records with any of the three, four, fi ve and six-digit NAICS codes that are subsumed in the two-digit industry classifi cation.

2.2. Remove the whole row for the NAICS code if the total number of contractors calcu-lated in Step 2.1 above is less than 10.

2.3. For each two, three, four, fi ve and six-digit NAICS code, calculate exposure hour weighted injury and illness and fatality rate averages (TRIR, Nonfatal TRIR, DART and Fatality Rate) using formulas for groups of contractors in Appendix 2. Also calculate the sum of total count of fatal work injuries by NAICS grouping.

2.4. For each rate and fatality count computed in Step 2.3 above, if there are fewer than 10 contractors used in the calculation, suppress the reported rate or fatality count by replacing it with a dash (“-”).

2.5. If the largest contractor contributes more than 70% of the total exposure hours for a given NAICS code, fl ag the total number of exposure hours with a symbol (“†”) for that NAICS code.

2.6. If the fatality rate is greater than or equal to 100 for a given two, three, four, fi ve and six-digit NAICS code, suppress the reported fatality rate by replacing it with a double-asterisk (“**”).

3. Injury and Illness Rates, Fatality Rates and Fatality Counts by NAICS Code without Those Records with Rates in the Highest 0.5th Percentile

When the number of records for a given NAICS code is small, extreme observations may unduly infl uence the reported rates and counts. Thus, Tables 1 and 2 also include the results of a sensitivity analysis conducted to evaluate the eff ect of those records with rates in the

highest 0.5th percentile. The analysis steps include the following:

3.1. For each rate computed previously in Step 1, calculate rates for each NAICS Code by excluding contractors that are in the highest 0.5th percentile of the entire dataset using the steps outlined in Step 2.

3.2. Calculate the percentage diff erence between the injury and illness rate from Step 2.3 and 3.1 for each two, three, four, fi ve and six-digit NAICS code.

3.3. Identify those rates that show a 10 percent or greater diff erence with an asterisk (“*”). For example, a table entry of 2.3 (1.8*) implies that 2.3 is the rate calculated with the entire dataset considered; 1.8 is the rate calculated with the highest 0.5th percentile extreme values removed; the diff erence between 2.3 and 1.8 is greater than or equal to 10 percent.

4. Injury and Illness Rates, Fatality Rates and Fatality Counts by ISN Work TypeThe injury and illness rate, fatality rate and fatality count calculations by ISN Work Type for the cleaned data set include the following steps:

4.1. Calculate Total Number of Contractors (∑jNCONTRACTOR ) for each ISN Work Type, j. This is simply a count of the number of contractors that fall into the work type designation. A contractor can be categorized as more than one work type and thus may be included in the counts for numerous work types. However, unlike the calculations for NAICS codes, higher level work types do not include contractors at a more disaggregated level. For example, calculations presented for work type 1.0 – General Contractor do not include records in any of the work types at a lower level (e.g. 1.0.1 – Turnaround or 1.0.2 – Rou-tine Maintenance).

4.2. Remove the whole row for the work type if the total number of contractors calculated in Step 4.1 above is fewer than 10, unless the work type has sub-work types with greater than or equal to 10 contractors.

4.3. For each ISN work type, calculate exposure hour weighted injury and illness and fatal-ity rate averages (TRIR, Nonfatal TRIR, DART and Fatality Rate) using formulas for groups of contractors in Appendix 2. Also calculate the sum of total count of fatal work injuries by ISN work type grouping.

4.4. For each rate and fatality count computed in Step 4.3, if there are fewer than 10 contractors used in the calculation, suppress the reported rate or fatality count by replacing it with a dash (“-”).

4.5. If the largest contractor contributes more than 70% of the total exposure hours for a given ISN work type, fl ag the total number of exposure hours with a symbol (“†”) for that ISN work type.
