on January 14, 2012, lone star circle of care celebrated our tenth anniversary. We

have achieved a lot in our first decade. in fact, we accomplished a lot just last year,

which is what this 2011 report highlights. lscc is more than just a health care home;

we are a family made up of patients, doctors, staff and partners. like many families,

we are constantly growing, evolving and changing, and we celebrate successes and

face challenges together. every now and then, it’s nice to look back on how far we’ve

come. Ten years of service takes a lot of heart.

This year Lone Star Circle of Care celebrates our 10-year anniversary.

We were fortunate enough to be present at the dedication of the original

Georgetown Community Clinic on University Avenue, and since that day the pursuit of LSCC’s mission has truly been an

incredible journey. At every turn in the road the organization’s leaders have been guided by the very simple and shared

vision that the steadfast pursuit of increasing access to high quality primary health care is both a morally satisfying and

economically responsible enterprise objective.

We have accomplished a lot in ten years. LSCC started with only four full-time employees, and we now have more than 500

hard working staff members. Despite a 40-fold increase in the number of visits provided each year, we have maintained

only one federal New Access Point grant throughout our first decade. Our unique public-private partnership model has

enabled LSCC to raise more than $53 million in grant funding and provide nearly $50 million worth of uncompensated

care to our patients.

The one small community clinic that opened on January 14, 2012, has become a network of 25 clinics spanning three

counties and serving nearly 100,000 individuals. Together, LSCC’s board leaders, staff, partners and supporters have

created an incredibly valuable and highly leveraged community asset that makes the world a better place than it was

before we came into being.

What defines an organization like this one that has accomplished so much? Since its inception, Lone Star Circle of Care has

been endowed with a unique spirit and mission bond that has remained at the core of everything we do. LSCC approaches

each and every endeavor with tenacity and a knack for innovation. These characteristics allow us to overcome challenges

and achieve many things that others might hesitate even to attempt. Through it all, we are driven by the needs of the

patients we serve and the people in this region that we know are still in desperate need of our services.

In ten years we’ve proven that LSCC excels at creating the art of the possible. Thanks to the unwavering dedication of

our employees, board members, partners and supporters, LSCC is well positioned to continue our success into the next

decade and beyond, providing truly accountable care to those who are most in need.

our family is GroWinG

In ten years, Lone Star Circle of Care’s health care

“family”—the patients in our health care home—has

grown tremendously. By now there have been close

to 100,000 members of our family (with over 73,000

individuals seen in 2011 alone), and with all that growth

there have been some changes. Our family is more

geographically diverse now, with more patients than ever

before coming from areas outside Williamson County.

We also have more seniors in our family than we used

to, most likely because we now offer internal medicine

services in clinics especially designed for those age 60

and older. But one thing that hasn’t changed about our

family members is that when they joined our health care

home, it was probably because they needed access to

care but couldn’t find it elsewhere. We were here for them

when no one else was.

lscc Provided more Than 75,000 visiTs To Travis counTy residenTs in 2011.

viSiON SERviCES

For years, Lone Star Circle of Care’s most vulnerable

patients had difficulty obtaining affordable vision care

access. Prescription glasses can be prohibitively

expensive for some, and regular eye exams are part of

the standard of care for patients with chronic diseases

such as diabetes. For all those who are underinsured,

affordable access to vision services is essential to

maintaining quality of life and productivity. In early 2011,

LSCC began offering optometry services for our patients,

including eye exams and discounted prescription eyewear.

LSCC MOBiLE APP

As a patient-centered health care home, LSCC is

dedicated to ensuring that our patients have access to

the information they need about LSCC when and where

they need it. As the world of communication technology

changes, so do the needs of our patients. For many LSCC

patients today, a mobile phone is the primary source

of internet connectivity—and information—for them

and their families. In order to stay connected with our

patients, LSCC launched a mobile “app” in 2011, which

provides convenient access to information about our

clinics, services and doctors. At the touch of a button,

users can request an appointment at an LSCC clinic, get

directions to their appointment, and more. The LSCC app

is free on iPhone and Android, and our mobile-optimized

web site is compatible with any browser.

PATiENT NAviGATiON CENTER

LSCC’s in-house Patient Navigation Center has grown

and evolved considerably since its inception in 2009,

adding staff and a number of patient-centered services.

Through one toll-free number, patients have access to

centralized scheduling at any LSCC location, specialty

referrals, chronic disease management, follow-up and

more. Contacts with patients are facilitated via phone,

email, chat and text messages. The Patient Navigation

Center allows us to keep patients engaged as active

participants in their own health and also help them

access the services they need across the health care

continuum, within the LSCC network and beyond.

BEN WhiTE hEALTh CLiNiC ExPANSiON

LSCC’s Ben White Health Clinic plays a critical role

in providing a health care home for some of the most

vulnerable underserved patients in Austin, including

those who are homeless, many who have complex

medical conditions, and hundreds of legally sponsored

international refugees from war-torn countries. In

order to increase our capacity to provide behavioral

health services and add more space for primary care,

LSCC expanded the clinic in 2011, more than doubling its

primary care capacity and adding a third-floor space for

psychiatry and behavioral health services. The location

now provides a complete health care home for nearly

18,000 individual patients.

dENTAL ExPANSiON

The need for affordable dental care in Central Texas

is extraordinary. Many patients are forced to neglect

their oral health because they simply can’t afford the

necessary treatment. Throughout Lone Star Circle of

Care’s history, the demand for our dental services has far

surpassed our capacity. In order to help meet the need,

LSCC completed a major expansion at our Georgetown

Dental Center at the end of 2011, adding 11 exam rooms

and nearly tripling the square footage. This expansion

will allow LSCC to provide 70 percent more dental visits

in 2012 than we have in previous years.

hEALTh CENTER AT TAYLOR

For years, LSCC has served many patients from Taylor,

Texas, and other rural areas of eastern Williamson

County at our clinics in Round Rock and Georgetown.

Through a partnership with Scott & White, in February

2011 LSCC was able to begin operating a family medicine

clinic in Taylor adjacent to Scott & White Hospital. LSCC

Health Center at Taylor provides much-needed primary

care access for uninsured and underinsured people in

that community and the surrounding area.

as a requirement of our status as a federally Qualified

health center, every year lscc undergoes an extremely

thorough a-133 federal audit. The financial data presented

here has been fully audited and verified. in 2011, lscc

passed our audit with zero findings and no management

recommendations for the second consecutive year.

PARTNER COLLABORATiON ANd ExPANSiON

In 2012 and beyond LSCC will continue to focus on

expanding our nationally recognized health care home

model across the Central Texas community. Thanks to

the unique and powerful partnerships that LSCC has

with the leading health care systems and foundations in

our region, we are well positioned to continue increasing

access to care. LSCC will work in collaboration with our

partners to fully leverage the opportunities created by the

Texas 1115 Waiver across our region. Key partners also

will enable LSCC to increase capacity in the northern

areas of Central Texas, where there continues to be

substantial need.

EMBRACiNG ThE fUTURE Of hEALTh CARE

It is difficult to imagine a more volatile yet exciting time

in the health care industry. As the national dialogue

surrounding health care reform goes on, LSCC will

continue to aggressively pursue our core mission

objectives, using innovative and forward-looking

strategies to advance the time-honored and critical role of

community health centers in the provision of accountable

health care. We will apply best practices, advanced

technologies and a collaborative approach to the creation

of more high quality, accessible health care homes for

everyone in our community.

MEMBER NAviGATiON SERviCES

Since 2008 LSCC has been a pioneer in developing the next

generation of health information technology solutions

and applications as well as patient-centered health care

delivery. A major cause of the high cost of health care can

be traced to an absence of true care coordination across

the continuum and a lack of focus on preventative and

primary care services for most patients, especially those

who are underserved.

Over the next two years, LSCC will make substantial

investments in the development of a truly integrated

delivery network that is focused on keeping people healthy

and tailoring both treatment and wellness programs

to each individual, or member. This revolutionary,

coordinated care will improve health outcomes, increase

capacity and create new access points to care, while

also reducing avoidable emergency room encounters

and hospitalizations. LSCC’s Patient Navigation Center

will evolve into a comprehensive Member Navigation

Center that will utilize proactive health and wellness

solutions as well as care coordination in a way that can be

scaled to ultimately serve millions of members of a true

accountable care organization.
