1.0 PURPOSE

1.1 To provide the preferred means of reporting and responding to the following types of

emergencies at the Plant for Human Health Institute (PHHI).

1.1.1 Fire, Smoke, Explosion or Gas Leak (Section 4.6)

1.1.4 Public Safety/Civil Disturbance Events (Section 4.9)

2.0 SCOPE

2.1 The requirements of this document apply to all employees, tenants, visitors,

consultants and contractors located on the PHHI premises.

3.0 RESPONSIBILITIES

3.1 PHHI Director, Business & Research Administration is responsible for:

3.1.1 Providing resources to ensure goals of Emergency Action Plan (EAP) are

met.

3.1.2 Serves as alternate Emergency Coordinator

3.2 PHHI Faculty and Staff (Including Consultants, Contractors, Temporary Employees,

and Tenants) are responsible for:

3.2.1 Serving as Fire Wardens/Monitors when requested to do so.

3.2.2 Promptly addressing emergency situations as specified in this EAP.

3.2.3 Evacuating the building when signaled to do so.

3.2.4 Providing, or assist in obtaining, emergency medical assistance when

necessary.

3.2.5 Promptly cleaning up all spills.

3.2.6 Participating in annual fire evacuation drills.

3.2.7 Attending annual EAP training.

3.3 PHHI Laboratory Managers are responsible for:

3.5.3 Assisting PHHI Laboratory Managers and Supervisors in evaluating

incidents as required.

3.5.4 Providing annual training for appointed Fire Wardens.

3.6 Fire Wardens are responsible for:

3.6.1 Assisting PHHI and Building Supervisor in the event of an emergency as

outlined in this document.

3.6.2 Ensuring that emergencies in their areas of responsibility are reported.

3.6.3 Ensuring that their areas of responsibility are evacuated in the event of an

emergency requiring evacuation.

3.6.5 Check Restrooms and secluded areas for employees during evacuation.

3.6.6 First floor only: Retrieve the sign-in sheet before exiting building to account

for visitors.

3.6.7 Assemble at meeting place as a group from assigned floor.

3.6.8 Conduct head count of assigned floor employees at Emergency Assembly

Point.

3.6.9 Report headcount to Emergency Coordinator at Emergency Assembly

Point or by cell phone.

3.6.10 Attending annual Fire Wardens Training.

4.0 PROCEDURE

4.1 To Report A Fire or Other Emergency

4.1.1 In any Emergency, Dial 9-9-1-1

4.1.2 If you discovered a fire on your floor, if you see or smell smoke, reports it

immediately.

4.1.3 An alarm should be initiated by the following method:

4.1.3.1 Activate the manual Fire Alarm Pull Station located at the exits

on the first floor when exiting the building.

4.1.3.2 Dial 9-911 immediately and report:

4.1.3.3 “There is a fire on the XX floor of the Plants For Human Health

Institute at 600 Laureate Way, Kannapolis, NC 28081”.

4.1.3.4 Call Facility Maintenance Supervisor (see Appendix 7.1)

4.1.3.5 Alert your supervisor and all other occupants on your floor.

4.1.3.6 Evacuate immediately, closing all interior doors behind you. Do

not lock the doors.

4.2 Emergency Action Plan Coordinator and Facility Maintenance Supervisor

4.2.1 The Director, EHS is designated as the Emergency Action Plan Coordinator.

Director, EHS.

4.2.2 The PHHI Facility Maintenance Supervisor is the designated Emergency

Coordinator for the PHHI Building.

4.2.3 Both the Director Environmental Health and Safety and the facility

Maintenance Supervisor are also the designated contact persons for

communicating with fire, police, and media during an emergency.

4.3 Emergency Information and Contacts

4.3.1 First Aid Kit Location: First Aid Kits are located in all laboratories near the

laboratory exit door.

4.3.2 Emergency Eyewash Location(s): Eye Wash stations are located in

laboratories by the door.

4.3.3 Injury/Illness Reporting: Ensure that incident is reported following the

requirements of NCSU EHS Website:

reporting procedures

4.3.4 All Emergency Contacts are listed in Appendix 7.1.

4.4 Employee Alarm System

4.4.1 The fire alarm system is activated by smoke detectors or manual pull

stations located at the exits on the first floor.

4.4.2 The system incorporates air conditioning system controls, fire zone

annunciators, and controls for simultaneously unlocking all stairway doors

and card activated doors.

4.4.3 The alarm system central panel is located on the South side of the PHHI

Building by the Double doors.

4.5 Emergency Shutdown Procedures

4.5.1 Equipment used in the laboratory work spaces should be shut down by

laboratory personnel, if time allows, as they evacuate the workspace.

4.5.2 No employees are designated to stay behind and perform emergency duties

4.7 Response to Medical Emergencies

600 Laureate Way, Kannapolis, NC 28081”.

Your name and phone number from which you are calling.

Contact Facility Maintenance Supervisor and provide the incident information

Notification Let other people in the area know about the incident and request assistance.

Actions (until

EMS arrives)

Do not move a sick or injured person unless instructed to do so by medical personnel, unless there is

immediate danger or unless absolutely necessary.

Persons qualified to administer First Aid, CPR or apply the AED may do so only when necessary and in

accordance with the limits of their training per the American National Red Cross or American Heart

Association First Aid Manual.

them of the incident.

Actions Step 1: Leave the Control Spill Area

Evacuate personnel from the immediate spill area. Block off immediate spill area – close corridor doors, use

lab carts, wastebaskets, etc.

Eliminate any fire hazard, especially if the spill is flammable or combustible – turn off burners, electrical

equipment, etc. Gas shutoffs are located at the corridor laboratory entrance. Electrical breakers are located

in the corridor adjacent to the laboratory.

Post sign, “Spill Area – Keep Out”

Alert other personnel in laboratory and adjacent areas of a chemical spill including the Department Head.

Step 2: Help Injured Personnel

Move injured persons from spill, remove contaminated clothing, flush, skin with water, use eyewash and/or

safety shower, etc. Seek medical attention if chemical is splashed in eyes, and/or there are burns or

respiratory problems. If person are found in the laboratory that are unconscious due to the chemical spill, do

not enter the area. Call 9-911 and report a medical emergency due to a chemical exposure. Ensure that

incident is reported following the requirements of NCSU EHS Website:

Make preliminary evaluation of hazard and identification of risks and decide whether Facility Maintenance

Supervisor should be called. If spill can be handled without respiratory protection, continue with the clean-

up.

If there is an unconscious person, do not enter the room. Call 9-911

Step 4: Clean Up Spill

Use absorbent pillows, pads, or vermiculite to

stop spill from spreading under refrigerators,

cabinets, equipment, drains, or corridors. Then

use pads around the perimeter to dam the spill and

adsorb the rest of the liquid.

Place soiled absorbent material into container

with plastic lining or plastic bag (use biohazard

waste container if necessary).

Seal the plastic bag for disposal.

Wash and deactivate the spill surfaces of trace

amounts of the spilled chemical. Apply acid

neutralizer to acid spills.

Contact the Procurement Manager for disposal

pick up.

Replenish materials in spill kit.

Step 4: Clean Up Spill

Wear appropriate PPE for working with blood and

body fluids.

Cover spilled material with paper towel soaked in

bleach or other disinfectant.

Allow disinfectant to decontaminate the spill material

and area for at least 10 minutes.

Pick up contaminated materials and place in Regulated

Medical Waste Container.

Step 5: Critique Incident with Director, Environmental Health and Safety to prevent future spills.

Tornado Earthquake Water/ Power Outage

IMMEDIATELY. The safest place is an

interior room on the lowest level away

from windows.

• Stay as safe as possible

during an earthquake.

• Be aware that some

earthquakes are actually

foreshocks and a larger

earthquake might occur.

• Minimize your movements

to a few steps to a nearby

safe place and if you are

indoors, stay there until the

shaking has stopped and

you are sure exiting is safe.

If indoors

• DROP to the ground;

• take COVER by getting

under a sturdy table or other

piece of furniture; and

• HOLD ON until the

shaking stops. If there isn’t

a table or desk near you,

cover your face and head

with your arms and crouch

in an inside corner of the

building.
