INTRODUCTIONThis directory provides a list of the plans network
providers. To get detailed information about your healthcare
coverage, please see your Evidence of Coverage.
What is a primary care physician?A primary care physician (PCP)
is the doctor who may see you on a regular basis as your main
doctor.
Your PCP can help you:
Make smart lifestyle choices to improve and maintain your
health
Get preventive care and health screenings
Better understand your medical conditions
Coordinate any specialist care to help avoid duplicate and
expensive tests. Your plan may require yourPCP to coordinate any
specialist care you receive
Manage your prescriptions and identify possible harmful
interactionsHow to choose a primary care physician (PCP)You will
have to choose one of our network physicians to be your primary
care physician (PCP). The termPCP will be used throughout this
directory.
Choose one of our network providers to be your primary care
provider and call to find out if theyre acceptingnew patients.If
you choose a PCP who is not taking new patients, we may assign you
to a different PCP. Ifthis happens, you can switch to another PCP
who is accepting new patients.
Your PCP plays an important role in your health care and we want
you to be comfortable with the provideryou choose. Call Customer
Service at the number listed on the back cover of this directory if
you want toswitch providers.
The "network providers" listed in this directory have agreed to
provide you with your health care coverage.Check your Evidence of
Coverage to make sure your plan includes additional benefits or
that you've signed upfor optional supplemental benefits before
making appointments for dental, vision or hearing services. You
cansee a specialist without getting a referral. If you have been
going to one network provider, you are not requiredto continue
going to that same provider.
If I need emergency or urgent careA medical emergency is when
you, or any other ordinary person with an average knowledge of
health andmedicine, believe that you have symptoms that require
immediate medical care to prevent death, loss of a limb,or loss of
function of a limb. The medical symptoms may be an illness, injury,
severe pain, or a medicalcondition that is quickly getting
worse.
If you have a medical emergency, get help as quickly as
possible. Call 911 or go to the nearest emergency roomor hospital.
You can always get emergency care from the nearest available
provider, whether or not they are anetwork provider, and whether or
not you are in the service area. You or someone else should call
and tell usabout your emergency care, usually within 48 hours. If
your emergency care is provided by out-of-network

providers, we will try to arrange for network providers to take
over your care as soon as possible given yourmedical condition and
the circumstances.
After you receive emergency care you may need follow-up care to
be sure your condition stabilizes. Thisfollow-up care will be
covered by our plan.
Urgently needed care is a non-emergency, unforeseen medical
illness, injury, or condition, that requiresimmediate medical care.
Urgently needed care may be received from in-network providers, or
from out-of-network providers when in-network providers are
unavailable. The unforeseen condition could, for example,be an
unforeseen flare-up of a known condition that you have.
In most situations, if you are in the plans service area, we
will cover urgently needed care only if you receivecare from a
network provider and follow the plan rules described in your
Evidence of Coverage. However, ifthe circumstances are unusual or
extraordinary, and network providers are temporarily unavailable,
we willcover urgently needed care that you get from an
out-of-network provider. When you are outside the servicearea and
cannot get care from a network provider, our plan will cover
urgently needed care that you get fromany provider.
If you get routine care from out-of-network providers, neither
Medicare nor your health plan will pay thecosts.
Our plan does not cover urgently needed care or any other
non-emergency care if you receive the care outsideof the United
States.
Additional BenefitsYour plan may cover additional benefits that
are not covered by Original Medicare. Additional benefits
mayinclude vision, dental or hearing services. You may have to use
specific providers for some of these additionalbenefits.
Services from these providers will not be covered if your plan
does not include additional benefits. Before youmake an
appointment, check your Evidence of Coverage to see if your plan
includes additional benefits.
If you get a billAt times you may get a bill from a provider for
the full cost of medical care you received. In most cases,
youshould send this bill to us instead of paying it. We will review
the bill and decide whether the services shouldbe covered. If they
should be covered, we will pay the provider directly. Please see
your Evidence of Coveragefor more information.
If youre on Medicare and MedicaidIf youre a Qualified Medicare
Beneficiary (QMB), your doctor may not bill you for deductibles,
copays orcoinsurance for Medicare-covered services, except for Part
D prescription drug copays. Please show yourmember ID card and your
state-issued Medicaid member ID card when you see the doctor or use
other planservices. This will help ensure youre billed for the
correct amount.
Accessing your mental health benefitsTo access your mental
health benefits, call the mental health number on the back of your
member ID card.When you call, a representative will check your
eligibility and get basic information about you and yoursituation.
Depending on the help you need, a clinician may talk with you and
recommend a provider andtreatment plan. If you prefer, your PCP may
call and arrange a referral for you. Your personal information
willbe kept strictly confidential.

Organ transplant servicesYour plan has a nationwide network of
facilities that perform organ transplants (including the heart,
lung,liver, intestine, kidney, bone marrow and/or pancreas). Solid
organ transplants must be performed at a facilitythat is currently
Medicare-approved for your specific type of transplant. The
complete list of your plansfacilities that are approved by Medicare
may change throughout the year. For information on network
