♦ 23 CFR 650.313(g) Quality Control and Quality Assurance requires each state to assure that systematic Quality Control (QC) and Quality Assurance (QA) procedures are being used to maintain a high degree of accuracy and consistency in the inspection program
Without QA/QC procedures in place, FHWA would find INDOT non-compliant, NBIS Metric #20
INDOT designated Beam, Longest & Neff (BLN) as Quality Assurance Officer (2011 – Present)

Bridge File Documentation / Reports
Note enough deficiencies to provide justification for condition rating.
Verify deficiencies noted are consistent between sketches, comments, and executive summary
Bridge deck rating for adjacent box beam bridge without deck to be based on beam condition not wearing surface condition
Approve bridge data within 60 days of inspection
Use text and abbreviations that are easy to follow

Critical Findings Structural or safety related deficiency that requires
immediate follow-up inspection or action

When in the Field……♦ Take extra time in field to verify structure member sizes, bridge dimensions, and section loss
♦ Document section loss and remaining section
♦ Verify condition at each phase of inspection, including annual inspections

♦ If significant deficiencies or change in condition visible, calculate rating quickly and recommend load posting changes
♦ Verify remaining section or section loss noted matches load rating calculation
♦ Include shear and moment in load rating calculations
♦ Verify load rating calculations or summary are signed and include Load Rating Engineer(LRE) information

When in the Bridge File…♦ Inspection agency to make sure load rating information added to bridge file in BIAS, does not have to include all pages of the calculation
♦ Make sure load rating matches BIAS data, must unapproved initial reports after load rating report approved
♦ Make recommendations for posting changes if needed. Remember to recommend advance posting signs.
