Director of Education

Welcome

Hello, it is good to have a chance to let you know what's happening in BASICS Education and I hope you will find this instructor Newsletter useful. My point of view with a couple of things which may be interesting to keep you updated at the present time.

The first of these is that I recently attended a meeting at the Faculty of Pre-Hospital Care in Edinburgh where all the current material in the PHECC Manual was reviewed. It was intended that this review would only be a minor update, but it became clear as we went through the material that there were major changes needed to the PHEC Manual and I suspect when it is eventually produced in the near future this will be more of a rewrite than just a review. I find this personally very stimulating as it suggests that the whole evidence base and practice of pre-hospital care is moving forward and at a good pace. This, I think, helps to keep things interesting. I hope that by the time the instructor day comes around in September I will have more concrete information to give you about the changes that will be present in the new manual and hence changes that will be introduced on the courses.

The second subject I would like to mention is that of assessment on courses. Initially we had no assessment on courses and then as we started to run courses under licence such as PHEC, MIMMS and PHPLS etc, assessment was introduced. Last year NES insisted that we have assessment of some kind on all courses and rather than have summative assessment on all courses, we introduced formative assessment. This has been highly successful and much appreciated by many of the candidates. I personally am now coming to the opinion that formative assessment is much more useful to most of our course participants than summative assessment will ever be. In fact, even more controversial, I would suggest that summative assessment is as much use to our courses as an isolated blood sugar is to assessing good diabetic control. Probably worth mentioning in these times of financial austerity, that summative assessment also comes at very considerable cost . We have mentioned all of the above to NES and have been asked to produce a paper for them on the subject. This is to be completed by the end of June. If anybody has any strong feelings on the matter and would like to express them to either myself or Brenda so that we can take these forward to NES, we would be interested to hear your thoughts.

Course UpdatesBASICS Scotland Immediate Medical Care Courses (IMC Part 1 and IMC Refresher):

•Courses booked for 2010-11 and 2011-12 are all IMC Part 1 and IMC Refresher courses. The individualised feedback is now established, with all participants receiving a post course letter stating any specific faculty advice for any skills and attitudes issues identified during the course. We have also introduced a pre- and post MCQ which is a simple tool to feed back on knowledge. This does not need to be passed or failed.

PHEC: •The manual of core material is in the process of rewrite. Instructors were asked to contact Colville direct with any sections/elements to bring to the attention of the writing/editing team at the faculty of Pre-hospital care within the Royal College of Surgeons (Edinburgh). Once the new manual is available, we will assess and review the instructor material to ensure information is consistent, so long as relevant and based on best evidence. We plan to continue to teach the military cricothyroidotomy kits on both PHEC and IMC courses as well as the pelvic splints.

PHPLS: •The course manual has been rewritten and we have run the first course based on the material in March, the second course happening in April. Hal Maxwell is taking feedback to ALSG based on his experience delivering both the military version of the course and BASICS Scotland is currently the only civilian organisation running PHPLS in the UK at present.

GIC: We have 39 people waiting to attend a GIC from BASICS Scotland courses but our planned course for 6-8th May 2011 had only 7 of these confirm their wanting to attend. As this was not a viable number, the course was cancelled in favour of running a full PHEC course for 24 participants. Brenda also directs GIC’s for ALSG in Manchester so PHEC, PHPLS and BASICS Scotland IMC IP’s can attend if so wished. We are delighted to report that ALSG have accepted and approved our application to have BASICS IMC courses recognised as appropriate provider courses to identify instructor potential. In addition, they have accepted that PHEC IC’s may do one IC practice on an IMC course (Part 1) but one must be on a PHEC course to ensure IC’s have summative assessment experience. This will help us allow our IC’s to progress through their two teaching practices within the recommended 2 years post GIC. We do plan to run a GIC again in 2012, and hope that we should be able to make this a viable course as there will be a greater number of IP’s on our waiting list by then.

MIMMS/HIMMS: BASICS Scotland continues to run HMIMMS courses at the request of NHS Boards.

BASICS Office UpdateWith the launch of the new website imminent, we have had a photographer in taking professional photos of

Sandpiper House, BASICS Scotland equipment and van and trailer.

2010 Resuscitation Guidelines: We have incorporated the 2010 guidelines into all our courses. The changes are relatively minor from the 2005 version.

Manual of Core Material: As mentioned on Page 3, the manual used for the PHEC and IMC course is being rewritten in the next few months.

Health and Safety: One or two instances of instructors and participants getting cuts/needle stick injuries during teaching sessions necessitates adding reminders on courses to beware of sharps! Similarly, we need to remind both ourselves and participants to be aware of moving and handling issues. This now incorporated into the introductory lecture for all courses.

BASICS Scotland Website: Over the summer months we plan to upload all the teaching materials for PHEC and BASICS IMC courses to the instructor area of the website. The instructor area is password protected so instructors will only be able to access materials for the courses they can teach on.

For any ALSG licensed courses (PHPLS, MIMMS, HMIMMS and GIC) all materials continue to be accessed from their VLE area for registered instructors. Our plan is to have a handful of instructors “test” the web access then we anticipate having this up and running for all instructors by the Instructor Day in September where we propose the demonstrate the site and area.

Equipment Update; Military Cricothyroidotomy Kits: We are now teaching this instead of the mini trach kits on courses. The reception of this kit from course participants is overwhelmingly positive, We have kits on order to start to hand to those who have been trained and wish to replace their mini- trach kits in their Sandpiper Bags. We tend to not be prioritised for delivery as any military orders are given priority over us! However, we have just had our first delivery – very kindly funded by The Sandpiper Trust. Anyone wishing to receive an initial kit to replace their mini-trach and having used one in training please contact Chelsee at [email protected] and your name will be added to a recipient list.

The plan is to take the kits to courses to hand out as participants and instructors are trained so long as they have a Sandpiper Bag. Thereafter we hope to place orders for more so that responders can send BASICS Scotland a cheque for any replacements kits. As these kits are not available to NHS/procurement we are very fortunate to be able to access these direct on behalf our responders.

Responder Equipment Update: Over the past few months, responders with VLS and Island responders have received pulse oximeters, pelvic splints and EZI0 drills again with grateful thanks to the Sandpiper Trust. We are now monitoring responders activity through the BASICS responders Patient Report Forms- so we can monitor what equipment is used regularly and what skills are used most.

Consistency in Teaching: Participant feedback from several courses has raised the issue of inconsistencies in teaching – mostly on BASICS IMC/PHEC courses. It is hoped the availability of the most up to date teaching material on the website should go a long way to addressing this problem.

Difficult Airway Society “APP”: from Drew Inglis for those interested. Free Iphone App, IDAS, not the best (Drew’s words) but has airway algorithms that are useful.
