REGISTRATION INFORMATION Same Prices As 2011!

Each applicant must complete a separate registration form. Two-day attendees must register for one seminar each day. Prices listed are per-day rates. Early registration is encouraged! Registrations received after February 27 will be accepted on a space-available basis. Registration and payment must be received by February 27 to receive the pre-registration fee. Each seminar is a full-day program with the exception of the Board/Trustee Seminar which is a half-day program. Credit card payments are accepted only for online registration. To register online, go to the Annual Conference tab at www.LeadingAgeOK.org.

PASSPORT REGISTRATION This entitles the attendee access to any of the seminars for both days. Simply mark the “Passport” registration and select all seminars you would like to attend. You will receive all applicable seminar materials for the seminars you have selected. To request CEU credit, mark the seminar you plan to attend each day and the corresponding CEU credit requested. This registration will ensure seating availability at all indicated seminars. Cost: see registration form.

MEALS Registration fee includes

Continental Breakfast, Lunch Buff et, and Afternoon Refreshments.

CANCELLATION POLICY Cancellations received prior to February 27 will receive a refund, less a $20.00 processing fee. Cancellations received prior to March 6 will receive 50% credit toward future LeadingAge Oklahoma seminars. Notify LeadingAge Oklahoma by March 6 of any substitutions. All cancellations or substitutions must be e-mailed to [email protected].

CEU INFORMATIONCONCURRENT SEMINARS - Attend the designated full-day seminar to receive appropriate CEU credit.

The CEU key (box on right) indicates the continuing education available at each seminar. Seminar titles represent the specifi c discipline referenced. For those seminars that encompass multiple disciplines, the following key is used.

Refer to the registration form to designate CEU credit request. CEU Certifi cates will be distributed at 4:45 p.m. on March 13 and March 14.

NURSING HOME ADMINISTRATORSLong Term Care Administration Seminars on March 13 and 14 have beenapproved for 12 continuing education clock hours by NAB/NCERS - approval #1332012-12.00-8636 (6 hours CEU credit for each LTC Administration program, for a total off ering of 12 hours CEU credit for Nursing Home Administrators.) This educational off ering has been reviewed by the National Continuing Education Review Service (NCERS) of the National Association of Long Term Care Administrator Boards (NAB) and approved for 12 clock hours.

HOME CARE ADMINISTRATIONLong Term Care Administration Seminars on March 13 and 14 have been submitted for review by the Oklahoma State Department of Health for 6 hours CEU credit each for a total of 12 hours CEU credit for Home Care Administration.

DIETARY The Dining Services Seminar has been submitted for 6 hours Continuing Education by the Certifying Board for Dietary Managers.

ASSISTED LIVING/RESIDENTIAL CAREThe Assisted Living Seminar on March 13 and the Long Term Care Administration Seminar on March 14 have been submitted for 12 hours Residential Care Administrators Continuing Education Credit through the Downtown College Consortium. All attendees will receive the necessary certifi cate for CEU verifi cation. If you prefer to have your CEUs catalogued by the credentialing institution, there is an additional fee. This is optional. The CEU credentialing fee is $30/6 hours, $40/12 hours which is charged by the credentialing institution.
