COMPREHENSIVE ASSESSMENT OF AN INDIVIDUAL OR COUPLEAn Aging Life Care Specialist will complete a comprehensive assessment on the Care Recipient, focusing on medical, physical, environmental, safety, legal and financial and will make recommendations based on those findings. Couples rates may apply.

ASSESSMENT FOR PLACEMENTAn Aging Life Care Specialist will complete an assessment to determine the level of care for placement and then assist the family in finding facilities that can accommodate the level of care needed.

FACILITY OVERSIGHT AND ADVOCACYAn Aging Life Care Specialist can assist and oversee all aspects of the Care Recipient’s move and transition into a facility. An Aging Life Care Specialist can also continue ongoing care management services at any facility.

HOME SAFETY ASSESSMENTAn Aging Life Care Specialist will go to the home and perform a safety and environmental evaluation to reduce the risk of falls and identify any potential problem areas to help the Care Recipient maintain independence.

COMPREHENSIVE CARE MANAGEMENTA team headed by an Aging Life Care Specialist will provide ongoing care management services on an as-needed basis — letting you step away from daily caregiver duties and get back to your life and the rest of your family.

SAFETY NET PROGRAMAn Aging Life Care Specialist will perform an initial comprehensive assessment and then follow up with a phone call monthly, and one face to face visit per quarter to check on the Care Recipient and monitor for any changes. This gives the family peace of mind that they can call someone if a concern or crisis arises but currently the Care Recipient’s needs are not enough to require services at this time. The hourly rate will apply after 4 hours per quarter are used.

MEDICAL POWER OF ATTORNEYAn Aging Life Care Specialist will provide the paperwork, ensure it’s notarized appropriately, and act as the Care Recipient’s Medical Power of Attorney. The Care Recipient must be enrolled in the Safety Net Program.

SHORT TERM CARE MANAGEMENTAn Aging Life Care Specialist will monitor a Care Recipient for a short time. For example, this is beneficial if the family goes out of town and wants someone to check on their loved one, or needs transitional services for approximately 30 days following hospitalization.

PRE- AND POST- SURGERY / HOSPITALIZATION PROGRAMAn Aging Life Care Specialist will attend pre-operative and post-operative visits to help manage the process from the planning of the surgery through recovery. An Aging Life Care Specialist can also monitor the Care Recipient in the hospital, during transfer to another facility, and upon discharge, help transition back to the Care Recipient’s home and reduce the risk of re-hospitalization.

SERVICES WE CAN PROVIDEAll services are performed by or under the direction of an Aging Life Care Specialist who is a Registered Nurse (RN) with experience in the kinds of issues you and your loved one are facing.

One of the most difficult decisions a family can face is whether a loved one who has a medical condition or is taking medication is able to drive safely. Diversified Nurse Consultants offers the DriveABLE Cognitive Assessment Tool, which helps patients and their doctors understand whether and how their driving skills may have been compromised. When this assessment is performed in the physician’s office, it can be covered by Medicare. FOUR SIMPLE STEPS TO PEACE OF MIND.Here’s how we can help. A free phone screening will allow us to gain an initial understanding of your situation, after which we follow four steps.

First, one of our Aging Life Care Specialists will perform a holistic medical, social, legal and financial assessment.

Next, your Aging Life Care Specialist will develop a specific, individualized plan of care, and work to put it into place.

Your Aging Life Care Specialist serves as an advocate with providers, insurers, trusts and others to ensure that everything goes as it should.

And finally, your Aging Life Care Specialist will provide ongoing monitoring and continually assess and update the plan of care as needed.

In short, we take all the time-consuming and challenging tasks off your shoulders.
