I, the undersigned applicant, hereby certify that one of following conditions applies to meplease selectone:

I have foreign nationality and have never held R.O.C. nationality; meanwhile I am not an overseas Chinesestudent by the time of application.

* *)R.O.C. nationality mentioned above is defined in accordance with Article 2 of Nationality Law.

1. According to Article 2 of the Nationality Law, a person shall have the nationality of the

Republic of China under any of the conditions provided by the following

Subparagraphs:

i. His/Her father or mother was a national of the Republic of China when he/she was

born. *The situation of mother does not apply to those who were born before

February 9 (including the 9th), 1980.

ii. He/She was born after the death of his/her father or mother, and his/her father or

mother was a national of the Republic of China at the time of death.

iii. He/She was born in the territory of the Republic of China, and his/her parents cantbe ascertained or both were stateless persons.

iv. He/She has undergone the nationalization process.

Preceding subparagraph 1 and subparagraph 2 shall also apply to the persons who were

minors at the time of the revision and promulgation of this Act.

I have never studied in Taiwan as an Overseas Chinese Student and I have not been accepted for admissionplacement by University Entrance Committee for Overseas Chinese Student in the year of application.

()All the documents I have provided (including diploma, passport, and other relevant documents whetheroriginal or photocopy) are legal and valid. Should any of the documents be found to be altered or toviolate any university regulation, my admission/enrollment will be cancelled and no proof of attendancewill be issued.

The proof of the highest education level achieved provided (for Bachelors degree applicants, your highschool diploma; for Masters degree applicants, your Bachelors degree diploma; and for Ph.D. degreeapplicants, your Masters degree diploma) is valid and legally awarded in the country from which Igraduated. The diploma provided is equivalent to the degree conferred by accredited schools anduniversities in R.O.C. If this is found to be fraudulent, including those through alterations and forgery,

and in violation of any regulation, my admission will be refused, my student status revoked, and notranscript or diploma will be issued.

Once accepted, the admission notice is valid under the condition that I have submitted the following

documents which should be authenticated by an overseas Embassy or Mission of Republic of China (Taiwan)

upon arriving in NTUST for registration.(1)Original diploma(2)

Original authenticated documents of the highest (**) academic degree diploma and transcript in English.

** Master programrecipient: please submit bachelor degree diploma;

Ph.D program recipient: please submit master degree diploma.

(3))Financial statement providing sufficient funds to cover your study in Taiwan.

(4)A certificate of medical and accidental insurance issued outside Taiwan covering the period from the date of entry into

Taiwan to at least four months upon reporting to NTUST.

I have NOTbeen dropped out of or expelled from any university or college in the Republic of China dueto unsatisfactory conduct grade, failure in academic achievement or confirmed sentence in criminal cases.Should I breach any of the regulations, I will be denied admission and my student status will be revoked.

If I am approved to make Initial Household Registration, Moving-in Registration, Acquisition orRestoration of Nationality during my study in Taiwan and have thus ceased to possess the status as aforeign student, I shall be expelled from National Taiwan University of Science and Technology (NTUST).

I have read all the regulations in the admission brochure and commit to obey the rules enumerated in the

said admission brochure.

I authorize this University to verify all of the above information provided. After being admitted into NTUST,if any of the information provided are found and proved to be false after enrollment, I have no objectionsin accepting the consequence as cancellation of my admission and deprivation of my student status. If abovecondition is found after graduation, I accept to hand in my diploma for cancellation and my graduationqualification be revoked.
