PMH: known case of Diabetes mellitus and Hypertension since 2 yrs. VITALS: PR: 66/min B.P: 180/90 mm of Hg CVS : s1 s2 + + RS: Clear ABD: Soft Labs: DATE 1 EOSINOPHILS 01% BLOOD SUGAR (F) 79mgs/dl HDL cholesterol 29mgs/dl VLDL cholesterol 18.8mgd/dl Current medication: Tab.nitro glycerine stat Inj.pantoprazole 40mg OD Inj.ceftrioxone 1gm OD CAD is due to sub intimal deposition of atheromas in small and medium-sized coronary arteries 1. the current therapy for the problem to reduce fat accumulation in blood vessels and decrease the blood volume to reduce the work load of the heart Need for Therapy: therapy is required. To 1.Prevent the platelet aggregation. And reduce atheroma formation and there by reduce the chest pain. 1.Tab.nitro glycerine stat Inj.pantoprazole 40mg OD Inj.ceftrioxone 1gm OD Tab.amlodipine 5mg OD Tab.glyciphage 500mg BD Tab.ecospirin 75mg OD Tab.clopidogrel 75mg OD Tab.atorvastatin 40mg HS 2. blood pressure should moniter while administering nitro glycerine, 3.ECG and ECHO Cardiogram is recommended. 1. Continue emphasizing medication adherence.
