Dear Mr. Luna:

Enclosed is the report for the Limited Asbestos Inspection performed at the Cysto Room in Corpus Christi, TX. The sampling was performed on August 2, 2016 by Mr. Carl Suding of Envirotest, LLC. Mr. Suding is licensed by the Texas Department of State Health Services as an Asbestos Inspector (#60-3504).

SCOPE OF WORK The scope of this project was limited to suspect asbestos-containing materials associated with the interior of the building at the above-referenced property. Nine (9) samples of suspect asbestos-containing materials were collected. No asbestos was detected in any of the samples collected.:

Suspect Materials Tested

During the inspection, sampled materials were assigned a sample number and a homogenous area number. Samples were collected of each area and the friability and condition of the suspect material was assessed. Table 1 contains the homogenous area numbers, material types, material descriptions, material locations, condition assessments, and a summary of the analytical results. Table 2 correlates each sample number to its homogenous area number. Analytical results and a list of definitions can be found following the text of this report.

Analytical Methods

All analyses were performed at our Houston laboratory using standard oil immersion and optical staining techniques. Envirotest, LLC is an American Industrial Hygiene Association (AIHA) accredited laboratory (ID #10643), a National Institute of Standards and Technology NVLAP-accredited laboratory (#101595), and licensed by the Texas Department of State Health Services (#30-0005) for asbestos laboratory analysis.

LIMITATIONS This sampling report does not guarantee that additional ACM is not present. The scope of this project was limited to the materials sampled within this report. Areas such as, but not limited to, beneath existing flooring, beneath all subflooring, inside doors, interior of all ductwork, interior of all equipment, interior of all piping flanges/valves, interior of all electrical components, behind all non-movable finish materials, and all other portions of the building not designated in the Scope of Work, were specifically excluded. The following analytical results pertain to only the samples analyzed and may not reflect the actual composition of the entire homogeneous area. Envirotest, LLC assumes no responsibility for any subsequent use or interpretations of these analytical results. List of Definitions

Asbestos-Containing Material (ACM) - any material containing more than one percent asbestos (chrysotile asbestos, amosite asbestos, crocidolite asbestos, tremolite asbestos, anthophyllite asbestos, actinolite asbestos). Demolition - the wrecking or taking out of any load-supporting structure member and any related razing, removing, or stripping of asbestos products. Disturbance - contact which releases fibers from ACM or debris containing ACM including activities which that disrupts the matrix of ACM, render ACM friable, or generate visible debris. Encapsulation - a method of control of asbestos fibers in which the surface of ACM is penetrated by or covered with a liquid coating prepared for that purpose. Enclosure - the construction of an airtight, impermeable, semi-permanent barrier surrounding asbestos to prevent the release of asbestos fibers into the air. Fiber - a particulate form of asbestos, 5 micrometers or longer, with a length-to diameter ratio of at least 3 to 1. Friable Materials - any material that when dry can be crumbled, pulverized, or reduced to powder by hand pressure. Homogeneous Area - an area of surfacing material or thermal system that is uniform in color and texture. Intact - means that the ACM has not crumbled, been pulverized, or otherwise deteriorated so that it is no longer likely to be bound with its matrix. Removal - all operations where ACM is taken out or stripped from structures or substrates, and includes demolition operations. Renovation - the modifying of any existing structure, or portion thereof. Repair - overhauling, rebuilding, reconstructing, or reconditioning of structures or substrates, including encapsulation or other repair of ACM attached to structures or substrates.

Asbestos content percentages are reported by area percent estimation. < = less than, > = greater than. Conversion of area percent to dry weight is not feasible unless the specific gravities and relative volumes of the different matrix materials are known. Accuracy and precision of the analysis is dependent upon the following items: quantity of sample analyzed, homogeneity of the sample, nature of matrix interference, sample preparation techniques, fiber size, material type, and the percent of asbestos involved. Asbestos may be detected in concentrations of <1% by area if sufficient material is analyzed. The minimum detection limit for asbestos analysis is less than one percent by area visual estimation. Inhomogeneous samples are separated into sub-samples and each layer is analyzed and reported separately, where applicable. LOCAL AMENDMENTS. THE INSTALLATION SHALL CONFORM TO THESE BUILDING CODES AND

ALL PERTINENT STATE AND NATIONAL CODES.

CONTRACTOR IS ADVISED TO VISIT THE JOB SITE PRIOR TO BID AND PRODUCTION OF SHOP

DRAWINGS, AND FAMILIARIZE HIMSELF WITH THE SITE CONDITIONS AND FACILITIES, TO

ACCOMMODATE ANY DIMENSIONAL DIFFERENCES BETWEEN OR AMONG THE DRAWINGS,

SPECIFICATIONS, EXISTING CONDITIONS AND THE ACTUALLY PURCHASED EQUIPMENT

CONNECTIONS FOR PIPING, DUCT, CABLE AND CONDUITS; ALL MODIFICATIONS MUST BE PRE

APPROVED BY THE DESIGN ENGINEER OR OWNER'S REPRESENTATIVE. FAILURE TO DO SO WILL

NOT RELEASE THE CONTRACTOR FROM THE RESPONSIBILITY OF FURNISHING AND INSTALLING

COMPLETE AND WORKING SYSTEMS COVERED BY THIS CONTRACT.

IN CASE CONFLICT OCCURS BETWEEN OR AMONG DRAWINGS SPECIFICATION, CODES,

STANDARDS AND LOCAL GOVERNMENT REGULATIONS, THE MOST STRINGENT REQUIREMENT

SHALL TAKE PRECEDENCE; ANY MODIFICATIONS NECESSARY MUST BE PRE-APPROVED BY THE

ENGINEER.

WALL MOUNTED SWITCHES SHALL BE MOUNTED AT 46" AFF TO THE CENTER OF THE DEVICE,

UNLESS NOTED OTHERWISE. ELECTRICAL RECEPTACLES AND TELEPHONE/DATA OUTLETS

SHALL BE MOUNTED 18" AFF, UNLESS NOTED OTHERWISE. CABLE TV OUTLETS AND TV

RECEPTACLES SHALL BE MOUNTED AT 80" AFF, UNLESS NOTED OTHERWISE. ELECTRICAL

DEVICES MOUNTED ABOVE COUNTERS SHALL BE INSTALLED 2" ABOVE COUNTER BACKSPLASH

TO THE BOTTOM OF THE DEVICE BUT NO HIGHER THAN 44" AFF. COORDINATE MOUNTING

HEIGHTS WITH ARCHITECT/ENGINEER PRIOR TO INSTALLATION.

ALL WIRING SHALL BE COPPER AND SHALL BE INSTALLED IN EMT CONDUIT. MINIMUM CONDUIT

SIZE SHALL BE 1/2".

FLEXIBLE CONDUIT (MC CABLE) IN LENGTHS NOT EXCEEDING 72" MAY BE UTILIZED FROM

JUNCTION BOXES TO LIGHT FIXTURES AND TO EQUIPMENT ABOVE CEILING OR IN MECHANICAL

ROOMS.

ALL CIRCUITS SHALL BE RUN WITH AN INSULATED GROUNDING CONDUCTOR.

ALL CONDUIT PENETRATIONS OF FIRE-RATED BARRIERS SHALL BE PROTECTED WITH UL LISTED

FIRE BLOCKING MATERIAL AND/OR SEALANT.

PROVIDE SLEEVES AND FLASHINGS REQUIRED FOR CONDUIT PENETRATIONS; PROVIDE

ESCUTCHEON PLATES FOR ALL CONDUIT PENETRATING FINISHED WALLS & CEILINGS.

ALL MATERIAL INTENDED TO BE REINSTALLED SHALL BE STORED AND PROTECTED FROM

DAMAGE BY THE CONTRACTOR. ANY DAMAGED MATERIAL SHALL BE REPLACED BY

CONTRACTOR.

ALL ELECTRICAL PANELS AFFECTED BY THE WORK OF THIS CONTRACT SHALL HAVE THE PANEL

SCHEDULES UPDATED. PANEL SCHEDULES SHALL BE TYPEWRITTEN OR COMPUTER

GENERATED. HAND WRITTEN PANEL SCHEDULES ARE NOT ACCEPTABLE.

PROVIDE JUNCTION BOXES AND CONDUIT FOR THE INSTALLATION OF WALL MOUNTED

TELEPHONE/DATA OUTLETS.

ALL RECEPTACLES SHALL BE HEAVY DUTY, HOSPITAL GRADE TYPE.

PROVIDE RED DEVICES AND COVERPLATES WHERE RECEPTACLES AND SWITCHES ARE

CONNECTED TO EMERGENCY POWER CIRCUITS.

CONTRACTOR SHALL PROVIDE CAT6 DATA/TELEPHONE CABLING FROM NEW OUTLETS SHOWN

ON SPECIAL SYSTEMS PLAN TO EXISTING IT CLOSET. TERMINATE CABLING AT NEW OUTLETS

AND TERMINATE WITHIN HOSPITAL NETWORK PATCH PANEL IN IT CLOSET. QUANTITY OF

CABLES/OUTLETS IS DESIGNATED ON DRAWINGS AT EACH OUTLET. COORDINATE EXACT

REQUIREMENT WITH HOSPITAL IT MANAGER.

ALL ELECTRICAL WORK SHALL BE PERFORMED IN ACCORDANCE WITH NEC 2008 - ARTICLE 517 -

HEALTH CARE FACILITIES.
