SUMMARY INFORMATION RELATING TO THIS POSITION

POST: CLINICAL FELLOW IN PAEDIATRIC CARDIAC SURGERY

BASE: ROYAL HOSPITAL FOR CHILDREN

This clinical fellow post in paediatric cardiac surgery is based solely at the Royal Hospital for Children, Glasgow. The position is for a period of 6 months but applicants seeking shorter or longer terms may be considered. The post is not a West of Scotland Region Scottish Deanery rotation and cannot be recognised for training. However, it offers full exposure to the training opportunities offered to West of Scotland paediatric cardiac surgical trainees. The successful applicant will contribute to the ward care of cardiac services patients and attend theatres. Active participation in the unit educational program and weekly clinical meetings is expected. Commitment to the unit 1:3 on call rota is required. The post would suit any paediatric cardiac surgery aspirant wishing to gain exposure to either field.

To practice medicine in the UK you need to hold both GMC registration and a licence to practice.

The Hospital

The Royal Hospital for Children, Glasgow, is the largest paediatric teaching hospital in Scotland. It provides care, not only for children resident within Greater Glasgow, but is also a tertiary referral centre for children from the West of Scotland and, in some sub-specialties, from the whole of Scotland.

There is a complement of 244 beds in the Royal Hospital for Children and 50 neonatal intensive care beds. All paediatric medical and surgical subspecialties are represented, including general medical paediatrics, cardiology, neonatology, neurology, nephrology, respiratory medicine, endocrinology, gastroenterology, immunology and infectious diseases, dermatology, haematology/oncology, audiology, ophthalmology, ENT surgery, orthopaedics and general paediatric and neonatal surgery. The hospital provides the Scottish national ECMO service for paediatric and neonatal patients. It is the centre for all paediatric cardiac surgery and the majority of the Paediatric Intensive Care for Scotland

The hospital provides the major Undergraduate Paediatric Teaching facility for the University of Glasgow and accommodates all or part of the University Departments of Child Health, Child and Adolescent Psychiatry, Medical Genetics, Human Nutrition, Paediatric Pathology, Paediatric Biochemistry and Paediatric Surgery.

Paediatric Cardiology

The hospital provides the national paediatric cardiac centre for Scotland, serving a population of 5.5 million and undertaking all types of cardiac surgery and catheter intervention with the exception of cardiac transplantation. Consequently the management of cardiac patients forms a significant percentage of PICU workload and experience. The paediatric cardiology ward is a dedicated 14-bed cardiac care unit, of which four beds are designated HDU level. Usual ward medical staffing consists of two NTNs in paediatric cardiology, two paediatric trainees and an FY2. The service runs a highly developed Advanced Nurse Practitioner Program, currently staffed by 2.0 ANPs. They are responsible for some ward duties, the pre-operative assessment clinic and a large number of non-clinical commitments.

Fellows are involved in the preoperative preparation of patients undergoing elective cardiac procedures as well as the ward based postoperative management. They contribute to the daily PICU cardiac ward round, learning more about the ward/PICU interactions. There is also regular involvement in the assessment of infants referred into the neonatal unit with cardiac conditions.

There are up to 9 weekly outpatient clinics. There is a fully functioning cardiac catheterization laboratory with diagnostic and interventional procedures undertaken on a regular basis. There will be a 1:5 out-of-hours commitment responsible for first line management of the Paediatric Cardiology patients between 09.00 and 21.30 (after 21:30 this cover is provided by the Hospital at Night team to which the cardiology middle grade do not contribute).

Paediatric Cardiac Surgery

The paediatric cardiac surgical department chairs Ward 1E with cardiology. It provides the national paediatric cardiac surgery centre for Scotland. The post holder will be expected to have an FRCS or equivalent and have an interest in cardiac surgery. They will be involved in pre-operative and operative management of the patients along with the post-operative intensive care. The 1:6 rota currently is required for 24 hour cover of the cardiac surgical service. The team consists of 3 Consultant Congenital Cardiac Surgeons, speciality doctor and the paediatric cardiac surgical fellows. The Unit provides a complete range of cardiac services excepting transplantation. Informal visits and contact is encouraged and can be arranged by contacting Mr Kenneth MacArthur or Mr Walid Gabir.

Neonatal Intensive Care

The Neonatal Intensive care has 50 cots, the unit provides regional level 3 NICU services and in addition sub-specialty expertise in neonatal surgery, cardiac surgery, ECMO, neonatal airway surgery, Vein of Galen treatment (UK/European service) and the congenital diaphragmatic hernia service. The Neonatal ECMO for Scotland and the North of Ireland is based in the unit. Neonatal cardiology admissions for the whole of Scotland are admitted to this unit for further assessment and management in conjunction with the cardiac service.

TEACHING

The Clinical Fellow will be expected to participate in and contribute to the weekly formal educational teaching program in paediatric cardiac service. The post provides an opportunity to benefit from the same teaching paediatric and paediatric cardiac trainees receive.

STUDY AND TRAINING

Study Leave within United Kingdom is in accordance with the Terms and Conditions of Service and applications should be submitted to the relevant Clinical Director. There is a good medical library within RHC and extensive library facilities in Glasgow.

POSTGRADUATE MEDICAL EDUCATION COURSE AVAILABLE LOCALLY There are frequent lectures within the University of Glasgow and the Royal College of Physicians and Surgeons of Glasgow relevant to postgraduate medical education. Special courses and seminars are arranged within the hospital from time to time. Formal in-post training within the hospital is arranged by the Medical division, by the postgraduate Tutor and by the Medical Staff Association.

RESEARCH OPPORTUNITIES

There are opportunities for clinical research and include laboratory and animal facilities if required. Any research protocol must first be agreed by the Hospital Ethical Committee before it can be instituted.

TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

TYPE OF CONTRACT Fixed Term

GRADE AND SALARY Clinical Fellow£ 31,220 - £ 41,305 per annum (pro rata)

New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.

HOURS OF DUTY Full Time 40.00

SUPERANNUATION New entrants to NHS Greater Glasgow and Clyde who are aged sixteen but under seventy five will be enrolled automatically into membership of the NHS Pension Scheme.  Should you choose to "opt out" arrangements can be made to do this via: www.sppa.gov.uk

REMOVAL EXPENSES Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.

EXPENSES OF CANDIDATES FOR APPOINTMENT

Candidates who are requested to attend an interview will be given assistance with appropriate travelling expenses. Re-imbursement shall not normally be made to employees who withdraw their application or refuse an offer of appointment.
