The use of Kloehn cervical gear is considered in regard to Class II skeletal correction, Class II dental correction, slow apical base expansion as a means for creating arch length, as an anchorage source to correct anterior overbite, and its direct application to the mandibular dental arch. Owing to the great versatility and effectiveness of the appliance, the author believes that Kloehn cervical gear is the most significant appliance developed to date in orthodontic-orthopedic history. The direct distal movement of the upper or lower molars as in the correction of a dental Class II or Class III; 2. The indirect distalization of the upper molars as a consequence of the downward and back- ward displacement of the maxilla in the ortho- pedic correction of a skeletal Class II; 3. A combination of 1 and 2 with posterior molar movement within the body of the maxilla, while the maxilla itself is being dis- placed bodily downward and backward on the cranial base. Distalization of molars may yet take another form as in the use of growth to gain arch length. In this instance, the molars are merely held in space while anterior teeth and the corpus of the jaw translates downward and forward with growth. Posttreatment superimposition shows a distaliza- tion of molars when, in fact, the molars are probably held in space within the dental trough while the jaw moves forward with growth, thus, needed arch length is gained without buccal tooth expansion or incisor advancement, both of which are highly susceptible to relapse. This article reviews the direct and indirect benefits of the Kloehn cervical gear appliance in orthopedic and orthodontic treatment 1. An at- tempt will be made to be objective, but it will be difficult not to expound heart-felt prejudices resulting from a 40-year love affair with this marvelous appliance. A list follows that enumerates, in the author s opinion, those instances in which the Kloehn appliance is clearly superior to ll other appli- ances and techniques, and why it might be the most significant and versatile appliance devel- oped in orthodontic-orthopedic history. The Kloehn appliance, in the author s opin- ion, has no peer in regard to: 1. Class II skeletal correction; 2. Class II dental correction with no mandibular anchorage loss; 3. Slow apical base expansion both in the trans- verse and anteroposterior (A-P) dimension; 4. As anchorage for gaining mandibular arch length; 5. For anchorage to correct anterior overbite; 6. To produce a growth-simulated treatment in adults including providing anchorage in a maximum retraction extraction case; 7. Used directly to the lower arch it is effective (a) for Class III interceptive treatment, (b) to restrain lower dental arch advancement, and (c) to move buccal teeth posteriorly. models, it is virtually impossible to tell the

difference between those patients in whom RPE

was used to widen the apical base and those in

whom slow continuous growth was induced at

the midpalatal and intermaxillary sutures via the

expa nded face bow.

Because of the potential of Kloehn cervical

gear to inhibit forward move men t of the maxilla,

to redirect anteroposterior growth and to pro-

mote transverse, anteroposterior, and vertical

growth, it seems appropriate to refer to the

Kloehn cervical appliance as the Kloehn growth

regula tor KGR). The KGR illustrated in Figure

5 shows a Kloehn face bow that has been altered

to achieve slow maxillary apical base expansion.

The in ner bow has been e xpand ed 8 to 10 mm. If

the bow is kept expan ded thro ugho ut treatment,

significant changes in apical base width, even

rivaling those achieved with rapid palatal expan-

sion, will predictably result. Figure 5B shows

where the outer bow is bent up off the horizon-

tal. The vertical bend in the oute r bow varies

from abo ut 10 ° at the start o f trea tmen t increas-

ing to 25 ° to 30 ° near the end of treatment. The

appliance may be used with or without an ante-

rior buccal shield. The shield might enhan ce the

stretching of the orobuc cal musculature to stimu-

late growth of these struc tures to their full

genetic potential.

When the inner bow is expanded, there is a

definite expansive force delivered to the molars

that would tend to make the molar crowns roll

outwardly. However, this tendency is completely

overcome by the pull of the cervical strap be-

cause the outer bows, whether they are straight

or bent up off the horizonta l, always lie above the

level of the source of the traction force. The

downward force from the outer bows actually

translates to a torque force that would cause the

crowns to roll to the lingual, the net effect being

a bodily lateral force to the molars. Considering

the great anchorage offered by the palatal roots

and period ontal structures and the lesser anchor-

age from the buccal roots, there is a force

transmitted to the intermaxillary and midpalatal

sutures to cause a slow apical base expansion by

sixth and seventh uses of the KGR. The sixth use

is a growth-simulated treatment in the adult, the

seventh being the application of the face bow

directly to the mandible to distalize molars and

other buccal teeth in the growing or nongrowing

individual. The author believes that this treat-

ment is unique but that it can be reproduced.

Because few orthodontists apply cervical traction

to the mandibular dental arch and even fewer

would do so expecting to move molars more

than 5 mm, the patient shown is of considerable

interest. It had been the intent to be less heroic

by removing lower first premolar teeth but the

patient asked whether it was possible to move

teeth into the space previously occupied by the

much earlier extraction of her third molars.

Figure l lA shows the severe lower anterio r

crowding as well as a full Class III molar relation-

ship with a crossbite extending from the second

premo lar to the second molar. The patient s
