• 6esign calculation for slab system shall be performed and submitted to company for approval.

• Certified and 'ell trained manpo'er shall be deployed for all types of form 'or( erection.

• &nspection shall be done by the supplier!third party prior to proceeding.

• Certificate shall be obtained from supplier for safe 'or(ing load and stability to submit to company.

• Care shall be ta(en to ensure that any form 'or( system is installed and supported correctly 'hen applied

on site conditions may vary. Disual inspection shall be carried out by the construction manager use for

each element to ensure support provided is ade=uate for actual site condition. ,very leg of scaffolding

should be chec(ed for tightness: rigidity and proper bracing both 'ays.
