limits.

• Tobacco questions – may be funding requirements• Verify with Tiffany• CC maybe changing policy
• Driving questions – may be funding requirements• Verify with Wendy• CC an issue

• and NPC or options for other and OT.• Some campus groups present as very different.
• Specific Organization Leadership Question• More Recovery information?
• Do you have any family members that are sober and in recovery?• Do you have any family members that are currently struggling with
alcohol or drug addiction?
• T9• Student aware of Univ. policy on sexual violence• Continue while at this campus.• Stalking or electronically invasive.• Gender of Perpetrator. Maybe not, just focus on prevalence.• Are we asking these question too many times, do we already have
this information from somewhere else.

reviewed at, the August PIP Meeting• Consent Form due back by September 4rd • Decision to add extra questions must be made by
September 18th (yes or no)• If decide yes, school-specific extra questions are
due before October 2st
• IRB Process will begin in September (non-extra questions) or October (schools with extra questions)
