BEST PRACTICE LOTOV (LOCKOUT TAGOUT VEHICLE)

PURPOSE• To ensure the safety of our employees who

perform maintenance and/or repairs on vehicles which include but not limit to: Sedans, Light Trucks or Medium / Heavy, Fire Trucks, Forklifts, and Misc. Equipment. In addition to ensure the safety of our clients and any visitors that may be in the maintenance area.

BEST PRACTICE LOTOVRESPONSIBILITIES / AUTHOIZATION / REQUIRIREMENTS

Responsibility• General Managers - Are responsible for the adherence

to First Vehicle Services Lockout / tag out program.• Authorization – Only authorized employees listed on

the list of “Affected and Authorized Employees Form” FRM-(SA-115 Affected and Authorized) are permitted to implement the First Vehicle Services LOTO Procedures.

• Technicians (authorized employees) – Must be knowledgeable of all information within FVS-SA-102A Title LOTOV and have received the required training to perform LOTOV

Tag out procedures shall be followed for the following types of situations:

• Diagnostic• Inspections• Electrical• Quick Fixes• Preventative Maintenance (PM’s)• Tire Changes

• A tag device (i.e. steering wheel cover or standard tag) shall have the authorized employees name on it and place on the steering wheel. (FRM-SA- 106)

• At least one NON-drive wheel shall be wheel chocked.

• The keys to the vehicle shall remain in the control of First Vehicle Services (i.e. on production board, in the technician’s pocket or in a key box).

Lockout procedures shall be followed for the following types of situations:

• Mechanical Repairs• Safety Related Issues

In addition to the above, if the client brings a vehicle to our service center for the following; no brakes, safety related issues, or the vehicle is not drivable, it shall be the responsibility of the authorized employee to lockout this vehicle.

LOCKOUT1.Affected employees will be notified that unit is

being taken out of service for maintenance, repair, and/or service

2.Disable unit by applying parking brake and shutting off engine.

3. Isolate unit of its energy source by removing the ignition key and locking keys out (i.e. lock it to steering wheel or lock it to mirror).

4. Stored energy in unit will be controlled by:

• Placing wheel chocks in front of and behind at least one NON-drive wheel when applicable;

• Letting engine and attached parts cool;• Maintain necessary safety latch engagement

on tilt-cabs and dump- bodied units;• Support unit by using jack stands when

applicable;

• Control stored or residual energy (i.e. a raised dump bed) by blocking or restraining (blocks or prop rods)

• Maintain use and proper engagement of safety latches with units on lifts;

• Remove battery source (i.e. lockout battery terminal) if necessary.

5. Verify that the unit has been successfully isolated from all energy sources by removing key (power source or pressing starter button, when applicable).

6. Legibly complete a “tag” and attach to unit steering wheel when applicable.
