US Bank Focus Card
• Payroll cards are essentially
prepaid debit cards that are loaded
up by Immanuel every pay period.
The cards can make purchases at a
point of sale and be used at ATMs
to withdraw funds loaded on them or
taken into US Bank for cash
withdrawal.

New Payroll Cards

– IRS 1099 form received for the HSA account.
– Reports all distributions from your HSA on Form 1099-SA.
– IRS requires you to prepare Form 8889 and attach it to your tax return. Check with your tax preparer for more information if needed.
– If you were participating in our HSA prior to June 1st and used funds from your accounts in 2015, you will be receiving two 1099 forms. One form from Mutual 1st and one from Wells Fargo.

New ACA Regulations
Overview
• Three new tax forms relevant to individuals, employers and health insurance providers.
– 1095-A, if you had coverage through the marketplace
– 1095-B, if you had coverage through an employer with less than 50 employee
– 1095-C, if you had coverage through a large employer
• These forms help determine your compliance. There may be a penalty fee charged if you didn't have health insurance in 2015.

Form 1095-C, provides information about the coverage your employer offered and whether or not you chose to participate. You need this form to complete your tax return.
New ACA Regulations
Reminders • Immanuel provides tuition reimbursement for FT employees up to a maximum of
$4,000 per fiscal yr. and regular PT employees up to a maximum of $2,000 per fiscal yr. This was increased in July to further help employees!
• If you elect the Flexible Spending accounts for Dependent Day Care or Medical or Limited, you must reapply for calendar year 2016.
• Special Enrollment on the Supplemental Spouse Life policy! During this open enrollment only, all spouses can get up to the lesser of $75,000 or 50% of the employee’s supplemental life amount.
• Special Enrollment on the Voluntary Cancer policy! If you do not currently have the cancer policy and would like to get the cancer policy, you will not have to complete a health questionnaire. There is a guarantee issue for this open enrollment only.
• Wellness Reimbursement on the cancer policy--$50 on the Low policy and $100 on the High policy. Outpatient physician reimbursement on Accident policy--$25 on the Low policy and $50 on the High policy.
Reminders Continued…
• Immanuel provides a 401k plan with a 3% safe harbor contribution to eligible employees and matches $0.50 on the $1 of the first 6% of employees contributions after eligibility is met.
• There have been a few questions about a recent letter and the Verizon employer discount of 22%.
• EVERY Benefit Eligible person must enroll or they will not have insurance on 1/1/2016!
