Per the Duke Committee on Health Information Policies and Services (CHIPS), all documents which are or will become part of the medical record are to contain two identifiers. The subject’s name and Medical Record Number (MRN) are the preferred identifiers. The subject’s name and date of birth may also be used. Be sure to include two identifiers on each page of the informed consent form (ICF). For all other research documents which are stored in the study file, at least one identifier should be present on every document. This will decrease the chance that the data could be lost or attributed to the incorrect study subject.
