    1.0 Communication:1.1 Booking of patients schedule for Operation.

    1.1.1 Calls are received from the AED or OPD or ward and documentedin a logbook then an OR request is sent after approval from theHead of the Anesthesia Department. It will be received by the ORstaff nurse stating the date and time received including thereceiving nurses signature and ID number.

    1.1.2 The scheduled book cases and additional cases are beingdocumented in a Daily Operating Room List Form and beingupdated at the end of shift and used as an indicator forimprovement.

    1.1.3 For Locum Doctors, they book and schedule cases through phonecall after approval from the Head of the Anesthesia Departmentthen documented in a Logbook and operation requested from AEDonce patient is seen by the doctor upon admission.

    1.1.4 The anesthesiologist on-call on duty will be contacted in extensionnumber 2117 and 2334.

    1.1.5 Approved scheduled Rota list for all OR Staff (Anesthesiologist andnurses/midwives) must be followed strictly and will stay up to theend of their duty.

    1.1.6 Any revision/changes of the scheduled Rota must be approved bythe head of the department with date and time and concerned areasmust be informed by the OR desk staff.

    change their official duty must have a formal communicationthrough a letter or Exchange of Duty Form. The department headmust make sure that he will be available to be reached for anyemergency calls.

    1.2 There is an existing nurses call in all surgical rooms including RR/PACUas a means of communication between all OR staff in cases of emergency.

    1.3 Communication between the ward nurses through endorsement of patientfor Operation includes the two identifiers.

    1.3.1. Verification of patients identification through:

    1.3.1.1 Patients statement of full name (3 names)

    1.3.1.2 Correct Medical Record Number of the patient with his/herID Band and file.

    1.3.1.3 Name of the Attending Surgeon.

    1.4 Communication with other departments telephone call to the respective

    departments (e.g. Laboratory, X-Ray) then a request is made and givenafter being entered to the WIPRO System.

    1.5 Patients who have to stay long or are delayed in the Post Anesthesia CareUnit A call is made to the respective ward where the patient is admittedand at the same time the Nursing Supervisor on duty is then informedstating the cause of delay or the need to stay longer. Through them, theywill inform the patients family or relatives.

    1.6 Communication between parents for children who will undergo operation

    parents are informed that they are allowed to go with their childrenfrom the receiving area up to the induction room provided that they wearshoe covers or clogs and that they will have to go out after the patient ispushed to the operating room.

    1.7 Communication regarding collection of post-operative patients fit fortransfer back to the ward or area is done through phone calls to thedesignated ward/area.

    1.8 Communication of Doctors to other Specialties a referral form is securedby the Attending Physician stating the reason for referral. (e.g. Surgeon toCardiologist)

    1.9 Formal and Informal Staff Unit Meetings documented through minutesof the meeting and an attendance (Logbook) is secured.

    1.9.1 Formal scheduled meetings

    1.9.1.1 Monthly and Weekly Meeting (as per schedule)

    1.9.2 Informal Emergency or Urgent Meetings

    1.10 Nursing Continuing Education Lectures (as per schedule)

    1.11 Unit Base Educational Lectures (as per schedule)

    1.12 Patient Education Conducted by Patient Educator (as per schedule)

    1.13 Communication with the Medical, Non-Medical and PharmacyWarehouse a request is made and can also be entered and updated

    through WIPRO.

    1.14 Communication with other departments with regards to Memorandums

    1.14.1 It is forwarded through the Intranet and posted on the Billboards.

    1.14.2 Scheduled operations are written on the board for staff,anesthesiologist and surgeons information (room allocation, patientrecord number, age, proposed operation, attending surgeon, anesthetistand type of anesthesia).

    1.14.3 For any work to be done (e.g. repairs), a work request form is beingfilled up and forwarded to the engineering department and the requestdone copy will be kept in a file for future references.

    1.14.4 Communication with Biomedical Engineering for equipments incase of need in the OR are made by informing them through phone callsand also entered in the WIPRO system.

    1.14.5 Communication with the Laundry department for linens (bedsheets, blankets, pillow and pillow cases) to be used inside the OR is madethrough phone calls in emergency and scheduled times.

    1.14.6 Communication to CSSD regarding collection of used instrumentsin the scheduled agreed time as posted in the billboard and issuance ofsterile packs, etc.

    1.14.7 Instruments needed for major surgery not available in the hospitalbeing done by outside doctors will be securing a gate pass through MMS.
