1.0 Title:Organizational Plan for Operating Room Services
2.0 Conditions:Applies to all medical, nursing and ancillary
staff assigned to work in theOperating Room Department.
3.0 Purpose:To provide functions and resources that contributes
to the achievement of thephilosophy, purpose, goals and objectives
of the Operating Room Department.
4.0 Procedure:4.1 The Head of Anesthesia and OR Department is
the representative body ofthe medical and nursing staff,
accountable to the governing body through theMedical Committee.
4.2 The OR is under the Director of Nursing Service. In the
absence of theDirector of Nursing, the Deputy Director of Nursing
is responsible for thereview, evaluation and monitoring of medical
and nursing staff practice andfunctions within the department and
the final approval for all of these will be the
responsibility of the Director of Medical Affairs.
4.3 The OR Nurse Supervisor reports directly to the Deputy
Director ofNursing and has the responsibility and accountability
for new staff orientationand ascertaining educational requirements
for existing staff. The OR NurseSupervisor provides Standards of
Care and Practice through collaboration withthe OR Head Nurse.

4.4 The OR Head Nurse supervises the rotation of nurses in each
departmentto ensure that all staff are familiar with each procedure
and special instrumentsfor 3-6 months basis. Daily assignment is
given to each staff in specific procedureapproximately 3 nurses in
each room assignment. Requisition and distribution ofstock from
warehouse and pharmacy is assigned in monthly rotation to
eachdepartment.
4.5 The OR Head Nurse reports directly to the Nurse supervisor
assigned inOR who has the responsibility and accountability for the
review, evaluation and
monitoring of OR Nursing staff. The OR Head Nurse implements
Standard ofCare and Standards of Nursing Practice through
collaboration with the ORNurse Supervisor.
4.6 Head Nurse/Charge Nurse in Anesthesia organizes with work
betweenthe Nurse Anesthetist and the Anesthesiologist for utilizing
the shortage of staff.
