Congratulations on your hard work and your commitment to the process of learning to live independently. We’re excited to have you join us and look forward to working with you and your family on goals that we will establish together as you work towards autonomy and independent living. We're eager to learn more about you, hear your insights into what you have learned from the past and talk about what you know you need to be successful moving forward. This Family Agreement is the plan we will refer back to time and time again so that you know where you stand with your family and with your personal commitments. We use it as a road map to help you build your life while in Portland. Before getting into the details it sometimes is helpful to review the bigger goals. The biggest goal that our members seek is independence. Research on Developmental Relationships from Search Institute and our own experience has proven that by practicing the following traits young adults reach their goals. Each of the following subjects will be rotating themes for our weekly group therapy sessions. Autonomy to Identity Members successfully inherit an increasing amount of responsibility for the planning and management of their lives and strengthen their sense of capability and identity. Determination to Resilience Members delve into and manage the challenges and pressures of the adult world while building healthy coping strategies to thrive and succeed in future trying situations. Organization to Responsibility Members strengthen their ability to structure and follow-through with attending to their major needs and finer details Reflection to Stability Members practice the ability to observe themselves and think about how in turn, others observe them as a means of maintaining intra and interpersonal stability. Balance to Harmony Members practice balance between work and recreation and achieve greater harmony in their lives. Planning to Momentum Members conceptualize long and short-term plans for themselves to help build greater momentum in moving forward with the various aspects of their lives. Connection to Belonging Members build on social practices to strengthen their familial, peer, professional and community connections to foster an enhanced sense of belonging to various “tribes” in their world.

Onward Transitions works with you and your family to set up an agreement about the type of experience you are looking to have in Portland. Together we will talk about and write down some expectations and responsibilities that we will all share. We will draw from four key objectives (education, work, service, and personal development to help you craft the life you are looking to lead now and in the future. Personal Development is divided into the following categories: Nutrition/cooking Finances Health and wellness-nesting,interpersonal counseling/coaching/mentoring Support groups, clubs, and social groups Other personal responsibilities The following pages will break down each of these objectives into tasks that are measured by the amount of independence you are demonstrating. It does this by looking at a spectrum of dependence-interdependence-independence. Not all of these tasks are meant to necessarily be managed independently, especially in the beginning of your move to Portland. The hope is that, over time, you will address the majority of these tasks more towards the interdependence-independence side of the spectrum. Please do not be overwhelmed by the amount of information on these pages. Some of the tasks may be obvious and you may already be doing them. You, your family and your therapist will discuss each task and alter it so that it is more focused on your needs and particular place in life. Again, it is a road map for achieving your individual goals and we will look at it frequently during your time working with us at Onward Transitions. You will need to add, subtract, modify and maybe pass over some items that you see here. Think of it as a comprehensive worksheet for you to help put together your plan before and after your move to Portland.
