PROPOSAL/PARTICIPATION FORM SUBMISSION GUIDELINES
1. Competition Description:
a. Open to all registered participants of the 25th MAOISCTE who are Malaysians and practicing
in Malaysia ONLY.
b. All submissions must be registered as a participant of the 25th MAOISCTE, 27th-29th April
2019 @ DoubleTree by Hilton, Kuala Lumpur.
c. Announcement of winners will be during the Gala Dinner of the 25 th MAO Conference on
27th April 2019
d. Only 5 submissions will be shortlisted to receive the 3M Orthodontic Materials
e. Submission of the Proposal/Participation form is needed in order to receive the 3M
Orthodontic Materials listed
f. All those who have received the materials must complete the project as stipulated in the
guidelines
g. Prizes will be given to the winner and runner-up amounting to a total of RM2,000 worth of
3M Products & Orthodontic Materials
2. Project selection:
a. Orthodontic Clinical Pearls, case summary + 1 minute how-to video (maximum)
3. Materials will be provided only after vetting of submission
4. Proposal/Participation forms should be completed as instructed and emailed to the emails below:-

5. Only complete Proposals sent in by 28th February 2019 will be considered for the finals competition
6. 3M reserve the right to disqualify any pre-submission proposals should it be incomplete or not be up
to the required standards. Decisions will be at the discretion of the organizers.
CASE SUMMARY RULES & REGULATIONS
1. Full presentation must be submitted in MS Word & PDF by 10st April 2019
2. Only 1-2 pages allowed
3. Number of words should not exceed 1500 words including acknowledgement & references
4. Each page must follow the example and format given below
5. Case Summaries must be simple and informative.
6. The Case Summary must include:
a. Introduction
b. Simple Case Report/Steps
c. Discussion & Conclusion
d. Acknowledgement
e. References
7. All photos must be original, clear and related to the case.
