Guidance Notes This form allows an application for HEKSS approval or re-approval to undertake NHS educational work for GP placement Clinical Supervisors, supervising FY2 doctors or where agreed by the Patch Associate GP Dean, providing clinical supervision for ST1 and ST2 GP Specialty trainees in a training practice supported by the GP Trainer (Educational Supervisor)
The process is based on the standards in the GMC’s “The Trainee Doctor” for specialty and GP training and the RCGP/COGPED Standards for Deaneries/LETBs guidance document. The COGPED/RCGP guidance amalgamates both the GMC and Academy of Medical Educator (AoME) standards described in the GP educator Competency framework and affords applicants the opportunity to reflect on how they meet those standards.
The application form mirrors the AoME Framework areas and also details relevant supporting evidence required in each section. The self- assessment form should be supported by evidence (a mandatory list is available) but will include trainee feedback from Exit Interviews.
Initial applications from practice-based educators usually combine a self-assessment and a GP School assessment including a visit to the practice to assess the learning environment; approval is for initial 2 years subject to a follow up visit during that period.
Providing the self-assessments and trainee reports are consistently satisfactory or higher and other regulatory visits (CQC) are of sufficient standard a formal re-application is required every 5 years.
Any self-assessment which generates a cause for concern or unsatisfactory reports from trainees or others will automatically trigger full HEE/Deanery visits. Some self-assessments may receive a visit to quality assure the system; applicants will be informed with not less than 4 weeks notification before such visits. Additional information may be requested.
Applicants will see a report on their application before a final determination of the outcome of the application is made. Appeals may be made to the Postgraduate GP Director.

A visit to the GP Practice is undertaken for all new supervisors where the practice is not already an approved training practice.
A visit may also take place where a trigger event has occurred Adverse or lack of trainee feedback Concerns from other regulators, Area Team or LMC or other stakeholders Major changes to the GP Practice
The Patch Associate GP Dean will review the Approval & Assessment application form; portfolio and visit report where undertaken and make a recommendation through the GP Trainer Selection Committee regarding appointment.
Ratification of continued approval will be received from the GP School Board.
