في الوطن العربيShort Introduction about the Diseases in the Arab Nation

يصعب معرفة و حصر األمراض المنتشرة في الوطن العربي و ذلك لشح المعلومات

و الموثقة عن هذه األمراض.االحصائيةكما أن نسبة انتشار هذه األمراض تختلف من

دولة ألخرى

It is not easy to know and conclude the diseases scattered in the Arab Nation due to the poor

accurate statistical data about these diseases. Also the percentage of these diseases is different for

each country in the Arab nation

تنقسم هذه األمراض الى: أمراض الدم الوراثية كفقر الدم•از العصبي كضمور العضالت الجذعيهأمراض الج•مراض أأمراض التمثيل الغذائي المعروفة بال•

االستقالبية خاصة الغدة الكظرية والغدة ,أمراض الغدد الصم•

These diseases can be classified into the following:

• Hereditary Blood diseases, such as Anemia • Neurologic disease as dystrophic myotsnica• Metabolic Disorders• Indocrine Disorder , such as Adrenal glance and

thyroid

معظم هذه األمراض تنتقل بالوراثة المتنحية و التي l في زيادة أعدادها. l كبيرا يلعب زواج األقارب فيها دورا

Most of these diseases are transferred to the new generation by marriages between

relatives

همية الدور الذي يتوجب على أومن هنا يتضح عاتق مخابر الزواج القيام به من تخفيض عدد

االصابات السنوية الجديدة وخلق التوعية الالزمة ألخطار هذه األمراض

So it is clear the importance of Pre- Marriage labs on the mission of

reducing the number of new annual illness and to create the public awareness of the

danger of these dieses

فحوصات ما قبل الزواج ....أين تبدأ وأين تنتهي ؟Pre-Marriage tests .....Where to Start &

يمكن أن تقسم فحوصات ما قبل الزواج إلى ثالثة أقسام رئيسية :

فحوصات لتجنب األمراض الوراثية .1.

فحوصات لمعرفة ما اذا كان أي من الطرفين يحمل 2.lأمراض قابلة للنقل من طرف إلى آخر عن طريق ا

االتصال الجنسي أو المخالطة الالصقة.فحوصات لمعرفة قدرة المقبلين على الزواج على 3.

إنجاب األطفال .

Pre–Marriage tests can be classified into 3 main Groups:

4. Tests to avoid the Hereditary diseases5. Tests to know if any of the marriage couples is carrying

diseases that can be transferred by sexual relation or due to close social relation.

6. tests to the ability of fertility.

To give more details we have to discuss each part

القسم األول: الفحوصات الوراثية التي يجب عملها إلى جانب الثالسيميا

الفحوصات الوراثية التي يجب عملها إلى جانب الثالسيميا هي باقي فحوصات الدم

رها ، فثالسيميا الدم هو مرض ينقل عن طريق الوراثة و يؤثر في عمر وتكس�

كريات الدم الحمراء، و لعل مرض الثالسيميا ليس هو المرض الوراثي الوحيد

الذي يصيب الدم ولكنه األكثر انتشار هنا في سوريا.

1st Group : Tests to avoid the Hereditary diseases.Hereditary examination that must be done beside the

Thalassemia tests is the Hemolytic Anemia. Thalassemia is a Hereditary disease which affect the life

time of blood cells.Thalassemia is not the only Hereditary disease that affect

the blood but it is the most spread disease in Syria.

/ مصاب بالتالسيميا, أما 7000حيث يوجد في سوريا حوالى /الحاملون للمرض فيبلغون حوالي مليون ونصف المليون شخص, أما في حلب فإن عدد المصابين بالتالسيميا حتى

l منهم 849نهاية الشهر الماضي هو / 155/ شخصاl يعالجون في 694يعالجون في وحدة منبج و / / شخصا

/ حالة إصابة جديدة 50مركز حلب مع العلم أنه توجد /l في مركز حلب فقط . و لكن هناك مرض بالمرض سنويا

l خاصة في دول الخليج العربي و هو ال يقل عنه انتشارا.s و هو الخضاب المعروف بـ األنيميا المنجلية

In Syria there are 7000 Thalassemia patient but the carrier for the Thalassemia are about 1.5 million. The number of Thalassemia patient In Aleppo only until the end of last

month was 849 patient, 155 are treated in Menbij Thalassemia unit, 649 Patient are treated at Aleppo Thalassemia center.

Knowing that there are 50 new Thalassemia cases in Aleppo Annually.

By the way there another wide spread disease in the Gulf Area which is Sickling Anemia that concerned with HbS.

فينصح المقبلين على الزواج أن يفتoشوا في qوراثيا qأمراض العائلة فان وجدوا أن هناك مرضاموجودا بالعائلة فينصح باستشارة أخصائي وراثة

عن الطرق الوراثية لنقل هذا المرض و كيفية فحصه.

Couple to get married should search in their Families for the presence of Hereditary Diseases and they are

advised to consult the Hereditary specialist to do some tests and have advice .

القسم الثاني: فحوصات األمراض القابلة لالنتشار عن طريق

االتصال الجنسي2nd Group : Tests to know if any of the couples is carrying

diseases that can be transferred by sexual relation or due to close social relation.

ومن هذه الفحوصات هي عمل وظائف الكبد و التهاب الكبد ب Bالوبائي

و HIV باالضافة الى فيروس نقص المناعة المكتسب جCو االخرى االلتهابات الجنسية

Some of these tests are HbsAg & HCV in addition to the HIV and any other sexual infections

القسم الثالث: الفحوصات الالزمة لمعرفة إن كان هناك قدرة على

اإلنجاب من الطرفين3rd group :tests of fertility for both Male &

female

يجب أن تشمل هذه الفحوصات فحص الحيوانات •المنوية عند الرجل لمعرفة عدد الحيوانات المنوية ونسبة الحيوانات المنوية السليمة و نسبة الحركة

أو نسبة اإلعاشيةالفاعلة فيهاكما ينصح بعمل هرمونات الذكورة لالطمئنان على •

الوضع الصحي لإلنجاب . أما لألنثى فينصح بشدة FSHعمل هرمون

• These tests should include Sperm Quality Analyses for Male (Sperm Concentration, Motility & Morpholgy) in addition to some fertility hormone

• FSH Hormone for female.

Our experiment in EDLIB concerning the Pre-Marriage Lab

:إن تجربتنا في محافظة ادلب تتلخص بما يلي

We can conclude the results of our experiment in Edlib as

follows:

تم البدء بالعمل بتحاليل ماقبل الزواج على مرحلتين:

في شهر أيار و حزيران و تموزالمرحلة األولى كنا مقتصرين فقط على إجراء تحليلين هما تعداد

اطب و الخالدم العام و الزمرة الدموية لكل منحيث بلغ عدد المراجعين للفترة المخطوبة

ت ابلغ عدد حال حالة زواج و1888المذكورة أعاله حالة وبذلك تكون نسبة 150تنافر الزمر الدموية

%.7.94ت تنافر الزمر للعدد المفحوص احال

We started the wok in our lab in 2 stages:1st stage include the period from May to the end of

July, during this period we were performing 2 tests only

which are CBC & Blood grouping, the number of marriage cases during this period was up to 1888 case, from

these cases we found that the number of adersion factions

was 150 which mean 7.94% from the total marriage cases

كانت نسبة المصابات بفاقة دموية عند شكالها المختلفة أب % 35اإلناث حوالي

كانت فاقة عظمىإال أن النسبة ال Michrocytic Anemiaدموية

The percentage of general Anemia cases for the female was 35% but

most of these cases where from the Michrocytic Anemia type

2nd Stage was during the period August and September

In this period we preformed all the tests that has been Declared

by the MOH and by the Committee of Pre– Marriage clinical Examination, where we have started to run the HB-V tests. HPLC technology has been considered according to the MOH Approval . This is a high performance technic, where it Is the reference method globally for the HB-V .

Results Accuracy:The HPLC system from Chromsystems is

characterized by a High Accuracy of the results. In addition we are able To get knowledge about all types of normal and Abnormal Hemoglobin

though one run for the sample Using just 1 reagent which is HB-V Kit. this will save a lot of

time and cost that is not available from any other company .

الحاالت الكلي HBV 45من عدد 2.08 حامل سليم للفيروس %

الحاالت الكلي HCV 70.32 حاالت حامل سليم للفيروس %

من عدد الحاالت الكلي

Male: • 9 cases of Minor Thalassemia which

represents 0.41% from the total samples• 45 cases of HBV carrier which represents

2.08% from the total samples• 7 cases of HCV carrier which represents

0.32% from the total samples

االناث:•من عدد 0.37 حاالت 8السيميا صغرى ث %

الحاالت الكلية HBV21 من 0.97 حامل سليم للفيروس %

عدد الحاالت الكليةHCV 5 من عدد الحاالت الكلية0.6 حاالت %

Female: • 8 cases of Minor Thalassemia which

represents 0.37% from the total samples• 21 cases of HBV carrier which represents

0.97% from the total samples• 5 cases of HCV carrier which represents

0.6% from the total samples

: و خالصة نتائج Results & Conclusion:

جل أن لديهما تنافر زمر من أبالغ كال المخطوبين إيخفف من األمر الذي احتياطات ما حول الوالدة

.وفيات والمشاكل الصحية للمولودينال

Informing the couples that they have adersion factions so they can take precautions before

baby’s delivery.This will participate in reducing the number of

death &

health problem for the new born.

و ك نسبة من المصابين بالتالسيميا الصغرى اهن لهذا الموضوع.يجب االنتباه بشكل جيد

There are a cretin percentage of minor Thalassemia patients, an issue we should take

care of this issue

وهو قد رتفاع اصابة الذكور بالعامل االستراليإ الذكور قد اليشير الى ان عملية الختان عند

تخضع لرقابة جيدة من قبل الجهات الصحية وهو ر.مر خطيأ

High percentage of males having HBsAg which could indicate that circumcision operation is

not well monitored by the Health authority

which is critical issue

من كل هذا يتبين لنا أن فحوصات ما قبل الزواج تبدأ بفحص الدم لمرض

الثالسيميا ولكنها تمتد إلى أكثر من ذلك بناء على رغبة المقبلين على الزواج لمعرفة الكثير عمن يودون اإلقتران

بهم .

We can conclude that the Pre-Marriage tests start with the blood tests for

the Thalasima patient but it could be extended to much more according to

the wish of the couples getting married, so they can get more
