Welcome to Open Enrollment 2012!• All full time employees working (32+hrs) MUST COMPLETE THE 2012

ENROLLMENT FORM (yes, a form is required EVEN if you decide not to take the benefits)

• Your 2012 Enrollment form is found on MPG’s Intranet under the HR Department• It’s EASY! Just follow the directions on the top of the form, SIGN and return!

• Complete this form and give to your site’s designated person• To find out who your site’s designee is, go to the 2012 Benefit info on the intranet in HR department

section or ask your Office Manager

What’s New in 2012? Long-term DisabilityDid you know?

One out of every four people in today's work force will have a disabling condition of 90 days or greater before the age of 65

•You already have an individual LTD policy? Great! Your individual plan will supplement/stack with the group plan so you can get as much income as possible.•No medical questions or health exams required•Great group rates!•Pays up to Social Security normal retirement age

– If your annual salary is greater than $100k• Own Occupation age, pays to age 65• Pays 60% of base salary up to $10,000 per month

– If your annual salary is less than $100k• Own Occupation (if you are able to return to work in a different profession), pays 24 months • Pays 60% of base salary up to $4,000 per month

What’s New in 2012? Life InsuranceGroup Life Insurance

FREE(for all Benefit Eligible Employees)

MPG has purchased

LIFE INSURANCE

for full time employees of 1 times your base salary

to a maximum of $200,000.

Voluntary Life InsuranceNew group rates

Employee– Available in $10,000 increments*

- Maximum is 5x salary, not to exceed $500,000- Guarantee issue is $50,000

Spouse– Available in $5,000 increments*

- Maximum is 50% of employee coverage not to exceed $250,000- Cannot be purchased without employee coverage

- Guarantee issue is $25,000

Child- Birth to 6 months, $500

- Age 6 months to 30 years, $5,000 or $10,000- Maximum is 50% of employee coverage

- Cannot be purchased without employee coverage

***Separate application requiredIf you had MPG’s voluntary life insurance (vli) in 2011, your coverage amount

will be grandfathered in. (If you had $300k for you and $100k for your spouse, you can apply for up to this amount without another health questionnaire)

If you did not have vli in 2011, a health questionnaire will be required for employee coverage over $50,000.
