Assume that the Product ( say Pump ) is used for fire fighting purpose.

Loss of Primary Function may be One of the Potential Failure Modes. Should it

be treated as one of the Legal Non-compliances ?

Nice one !

Let’s understand one important aspect here. The product designed is not creating any safety issue by itself.

It is the situation, wherein the product is applied and hence, it can not be considered in an FMEA analysis as Legal.

Hence, while evaluating its Severity, it would be ideal to consider this situation as “Loss of Primary function” only.

The corresponding severity ranking is 8, but NOT 10 – which is meant for Legal.

What is the Difference between “Error” & “Variation” ?

Let us consider a Manufacturing process.

Variation is Omni present and can not be avoided. However, excess variation may lead to an error, but all variations are not errors.

Variation will decide the fate of a process / product. Variation may be caused because of Chance causes and Special causes.

GenerallyGenerallyGenerallyGenerally there may not be any error, when the variation is due to chance cause of variations only. Hence it is believed that, this variation will not vitiate the fitness for use and hence it is not considered as an “error”.

Special cause variations may lead to Error. What we discuss now are General principles, but specific process to be studied for final judgment.

Shall we consider the Machine Capability study as one time activity ?

Mostly yes !

At the time of Machine approval, we shall conduct and convince on Machine’s variation pattern through a Machine Capability study – which is known as Cm & Cmk.

Afterwards, the regular Process capability studies will take care of Machine variations too and hence no need for a separate Machine capability study.

Assume that the Machine undergoes a significant Maintenance process and we are now interested to assess the Maintenance quality. Now, a machine capability study may be of use, again.

Out of 10 design samples, let’s assume that ONE product failed in a field trial.

How severe is this effect ? What kind of importance to be given for this ?

Depending upon the nature of Failure, we have to attach the importance accordingly.

A rare incident in Delhi attracted the attention of the Whole nation, recently ! If we really look that in the ppm level, the incident is of no big significance at all !

However, the crime has happened in the Capital of a Nation and hence it has been given highest level of focus.

Nature of failure will decide its importance. If the failure is on Safety or Primary function, the importance will be more than that of other types of failures.
