How does it work?

On a facility-selected weekly or twice-weekly basis: - The provider logs into Connect and enters one-click status

updates on the family-selected wellness metrics for the resident (up to 10)

- They add any additional comments they’d like and click “Save and Send” NOTE: The entire update process takes less than 3 minutes per subscribing family and can be done directly on-line or using a simple printed template and input into Connect later

The system immediately notifies authorized family members by email and text message that a new update is available for viewing at their secure Connect for Healthcare™ portal NOTE: depending on the provider’s preference, the update detail can be included in the email and text message
