WHAT IS AN ADVANCE DIRECTIVE?
An advance directive is a legal document that explains to your
physician, health care professionals, family and
friends, what your wishes are about your health care
This document is prepared before there is ever a need for it
In Nevada, there are different types of advance
directives available
TYPES OF NEVADA ADVANCE DIRECTIVES
Durable Power of Attorney for Health Care
This document will give the person you designate power to make
decisions concerning your medical care and treatment, in the event
you are unable to do so
Declaration
This type of instrument is used to direct your attending physician to withhold or withdraw any form of medical treatment that is meant
only to prolong the process of dying, in the event you are
determined to have an incurable or irreversible condition
Do-Not-Resuscitate Order
This document informs all health care professionals who treat you,
that you do not desire them to use CPR if your heart stops beating or
you stop breathing
Physician Order for Life-Sustaining Treatment
This detailed legal document spells out the different types of
life-sustaining medical treatment
to which you consent or refuse, depending on the situation
WHAT IS REQUIRED TO CREATE THE DURABLE POWER OF ATTORNEY?
First, you should choose your attorney-in-fact, to act
on your behalf
