OBSERVERSHIP 2009

Dear GSMC / KEMH Interns and Residents,  

We are in the process of setting up the 2009 "Observership" Program. There are several changes in the set up. 

1. Because of limited number of available mentors only 6 - 8 candidates will be selected for the 2009 program.

Selection will be based on merit, strength of recommendation letters, scholastic and extra- curricular

activities.

2. To assure a spot, the sooner you submit all the required documents the better your chances of confirming a

position. Incomplete data will delay your registration and you could lose your priority.

 

3. Rotations will be assigned on an as available basis. We will try to meet your requests but no guarantees.

 

4. List of required documents will be sent to you separately.

5. Print out the "Confidentiality Agreement" and sign and print your name where indicated and return a scanned

copy with the rest of the documents.

6. Once we receive and review your completed application you will be informed about our decision and rotations.

REQUIREMENTS FOR OBSERVERSHIP PROGRAM 2009

1. GRADUATION CERTIFICATION

Copy of M.B.B.S. / M.D. / M.S degree certification.

2. CONFIDENTIALITY AGREEMENTS by

Vanguard Health Care & Fallon Clinic. To be signed by the candidate and the mentor.

3. CURRICULUM VITAE

No more than a three page summary of your CV along with a recent photo

4. VACCINATION and DOCUMENTATION absolutely essential.

As per attached protocol.

5. RECOMMENDATION LETTERS FROM

Dr. Lopa Mehta, Dr. Nithya Gogtay and one other GSMC / KEMH faculty member.

6. WHITE LAB COATS

Bring two white lab coats with your name monogrammed above the left pocket.

7. SECURITY and ID BADGES:

Will be obtained from Human Recourses office at SVH before starting observership.

8. VISA’S

The Observership Program, Fallon Clinic or St. Vincent Hospital cannot sponsor or get involved in any kind of correspondence or

documentation regarding VISA application.

9. PASSPORTS and VISA DOCUMENTATION:

Scan and email the first two pages of your passport and the approved VISA.

10. HOUSING:

Housing is available quite easily around the UMMS / SVH campus.

11. HEALTH INSURANCE:

Applicants must provide their own health insurance coverage.

12. DOCUMENTATION:

OBSERVERSHIP IMMUNIZATION RECORDS

The Fallon Clinic, St. Vincent Hospital and UMMS require that all visiting students and interns show proof of immunity to

Measles, Mumps, Rubella, Tetanus/Diphtheria, and Hepatitis B. Proof of a TB skin test and a digital copy of a

Chest X-ray should be submitted. Applicants must be free from symptoms of Infectious disease at the start of an

elective. The following information must be completed approved and signed by a faculty member in the Department of

Microbiology and Radiology at GSMC / KEMH.

Digital Copy of Chest X-Ray taken within 90 days of starting your elective.
