Create a survey using survey monkey

Sign up to Survey Monkey or skip to survey monkey home if you already have an account

Choose start from scratch to start your survey

Add a title to your survey

Choose your question type i.e. slider, drop down, multiple choice etc. Then record each question, you can change the question type for each individual question

Preview your survey and test it out

When you’re happy with your survey click next

You can choose to save your survey as a PDF

Select how you would like to send the survey i.e. via email, web link. Determine the recipient of the

survey responses, they will receive the notifications

Copy the weblink to send to your community. You could send it via an email or post it on your Facebook page

You will receive an email when a response is received

You have the choice to view the responses in a graph or as an individual response
