The history of services and supports for
persons with disabilities started with
segregating people in institutions, like a big
hospital where you live all the time. Person
Centered Planning is a way to make sure
people with disabilities are included as a
full citizen. You have the same rights,
dreams, and goals as everyone else.

Person centered planning is supposed to help you
think about what a good life for you looks like. This
includes things that are important TO you. Big
dreams and goals can be turned into smaller steps
to make them easier to do. Other people like
friends and family can help you come up with ways
to make the things you want in life happen. And,
you can always change your mind about what you
want and do something new or different.
